7749-C-38 Division TECHNICAL SPECIFICATIONS 1.0 SCOPE 1.1 General This Specification covers the complete furnishing, fabrication, delivery, unloading and storage of materials as well as the labor, equipment and supervision necessary for the construction of the Shield Building slip"'formed walls, temporary lightning protection, miscellaneous embedded items and blockouts.

1.2 Work Included The WORK included in this Contract consists of furnishing all labor, tools, materials, equipment, instruments, supervision and services necessary to con struct the Shield Building walls including th~ following:

2.3 All referenced specifications, codes and standards shall be the latest revision in effect on the date of the Contract.

2.4 Discrepancies in and between applicable codes, standards, these Specifications and/or design drawings shall be brought to the immediate attention of the ENGINEER, whose decision shall be final.

3.0 SCHEDULES 3.1 See paragraph 6.0, "Table of Key Start and Completion Dates", of Section VIII.

Specitic Conditions, of this Contract Document for Construction Schedule Requirements. A second schedule is given in this table for Alternate 1.

4.0 DESCRIPTION

OF CONTAINMEN'T STRUCTURES 4.1 General The Containment Structures consists of the following two separate structures:

4.1.1 The Containment Vessel (not in contract 7749-18) will be a low leakage 2

steel shell which will contain the energy and material that could be released by an accident. The Specification for the Containment Vessel is referenced as Specification No. 7749-C*37 in Paragraph 7.2.

4.1.2 The Shield Building shall be a medium leakage rate concrete structure completely surrounding the Containment Vessel and providing biological shielding and protection for the Containment Vessel from adverse atmospheric conditions and external missiles.

4.2 Shield Building The Shield Building shall be a vertical reinforced concrete right cylinder with a shallow dome roof.

2 The Shield Building will completely enclose the Containment Vessel. An annular space is provided between the walls of each structure adequate to permit construction operations. Clearance under the roof dome is provided for a temporary false work system to support a 2*foot thick concrete shallow roof 2

dome. The Containment Vessel top head will support precast concrete forms and their supports plus a 50 psf minimum construction live load. Support pads will 3

be provided on the vessel head.

7749-C-38 Division 4.3 Blockouts, Openings and Embedments Construction openings, blockouts and embedments shall be of size and location as shown on the arrangement drawings. The blockouts will be filled in later by others, except that the equipment hatch, personnel lock and escape lock, blockouts will be constructed by this CONTRACfOR.

\\2 4.4 Design Drawings 4.4.1 The design drawings describe the Shield Building and form a part of this Specification. They show the concrete dimensions, reinforcing requirements, penetrations. openings and site conditions. Additional and/or revised drawings and details will be issued by the ENGINEER as required.

Section IX, List of Drawings of the Contract Document enumerates the drawings for the Shield Building which are part of this Specification and Contract Document.

4.4.2 The CONTRACTOR shall abide by. and fully comply with, the true intent and meaning of the drawings and shall not endeavor to avail himself of any unintentional errors or omissions, should such exist, nor fail to complete and perfect every part of the WORK intended though incompletely shown or expressed for the sake of brevity or for avoiding undue repetition.

4.4.3 All WORK shall also conform to such other drawings issued in explanation of details or to cover conditions encountered during or prior to the execution of the WORK or resulting from revisions.

5.0 DESIGN AND WORKMANSHIP 5.1 Except as stated herein or shown on the design drawings, the WORK shall conform to the "Specifications for Structural Concrete for Buildings", ACI Standard 301, Building Code Requirements for Reinforced Concrete. ACI 318, Concrete Chimneys (ACI 307) and the Ohio Building Code.

7749-C*38 Division 5.3 The storage, batching, handling and hauling facilities of the concrete supply contractor shall be inspected by the CONTRACTOR at his option to determine the adequacy and capacity of the plant to handle the WORK efficiently.

5.4 Substitution of materials at variance with the referenced standards and specifications and with this Specification will not be permitted except upon written authorization from the CONSTRUCTION MANAGER and/or ENGINEER.

5.5 The CONSTRUCTION MANAGER will provide horizontal control for the Shield Building centerline and a bench mark for vertical control on the foundation slab.

The CONTRACTOR shall establish lines and grades from these control points necessary for the accurate installation of the WORK. All lines and grades shall be checked before placing of concrete by the CONTRACTOR.

6.0 MATERIALS All materials shall be new, unused, and of U.S.A. manufacture. The CONTRACTOR shall be required to furnish satisfactory evidence and/or notarized certificates stating that the materials furnished conform to the standards or stipulations stated below or elsewhere in this Specification or on the design drawings. All materials shall conform with the applicable ASTM Specification, and/or these Specifications, whichever is the more stringent.

7.0 STANDARD SPECIFICATIONS 7.1 Contract Specifications The following Contract Specifications are hereby made a part of this Specification and Contract Document. They shall be binding upon all parties as if they are an integral part of this Specification. In the event that a conflict between Specifications arises, the ENGINEER shall make the final judgement.

8.0 INSPECTIONS. SAMPLES AND TESTS

8. I General I

Inspections. samples, and tests will be made as required in these paragraphs :lnd the Contract Specifications. Documentation will be provided as required in Section XII. Quality Control Requirements, of this Contract Document.

8.2 Concrete Concrete, freshly batched by others, will be sampled and tested by the CONSTRUCTION MANAGER or his authorized representative without charge to the CONTRACTOR. The CONTRACTOR shall cooperate fully in providing samples in the amounts required for tests without charge to the OWNERS, or 12 their authorized representative. Exhibit 6 Power and Industriai Specification hu. 7749-C-38 Division 9.2 Curing The Shield Building concrete, except as otherwise approved by the CONSTRUCTION MANAGER, shall be cured by the liquid membrane method.

The liquid membrane curing compound shall conform to Clear Seal No. 150 as 3

manufactured by Grace and Company, Cambridge, Massachusetts, or approved equal. Application shall be in accordance with the manufacturer's instructions:

The liquid membrane shall be applied to all slip-form wall surfaces. The membrane shall be applied to within 2 feet (two feet) of the bottom of the slip-form after the concrete surface has been finished.

9.3 Repair of Concrete Repair of imperfections in formed concrete shall be completed within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> after removal of forms. Fins shall be neatly removed from exposed surfaces.

Concrete that is damaged or honeycombed must be removed to sound concrete by chipping, and in areas of wall prominently exposed to view, an initial cut of 1 inch in depth shall be made by a diamond saw around the honeycombed area and the inside of the area chipped to sound concrete replaced with drypack, mortar, or concrete as hereinafter specified. Drypack filling shall be used for holes having at least one surface dimension less than the hole depth, for holes left by the removal of fasteners from the ends of form tie rods and for narrow slots cut for repair of cracks. A coating of an approved epoxy resin concrete adhesive shall be applied to all concrete surfaces, except tie rod holes, immediately before plaCing drypack or concrete repairs. The epoxy resin concrete adhesive compound shall be Thiopoxy 62, A. C. Horn product as manufactured by Grace and Company or approved equal. Application shall be in accordance with the manufacturer's instructions. All materials, procedures, and operations used in the repair of concrete, shall be as directed by the CONSTRUCTION MANAGER. All fillings shall be bonded tightly to the surface of the holes, shall be sound and free from shrinkage cracks and dummy areas, and shall match the adjacent concrete in color and texture after the fillings have been cured and dried.

10.0 SLIB-FORM WORK The cylindrical wall of the Shield Building from the foundation ring to the spring line of the dome shall be constructed by continuous slip-forming except that forms may be stopped at the end of the week's work. Concrete for slip-form work shall not be considered as mass concrete. Cement used in slip-form work shall be Type II below elevation 585' and Type I above elevation 585'.

10.1 Design The CONTRACTOR shall design the formwork, shoring, and scaffolding for the following loads in addition to the loads from concrete weight and pressure, platform dead, live, equipment, and jacking loads:

Construction 10.2 C'C'.;2 Forms shall be constructed in accordance with the applicable Provisions of ACI 347 except as modified herein. Forms shall be of wood, metal, structural hardboard or other suitable material that will produce the surface finish required in paragraph 9.1. Forms shall be constructed to conform to the shape. form, line and grade required. They shall be sufficiently rigid to prevent deformation under load and be so designed as to be removed readily without injuring the concrete.

Joints shall be mortar tight and arranged to conform to the pattern of the design required. Local defects, such as chipped plywood or kinks in steel forms, will not be permitted. Lumber to be reused shall be thoroughly cleaned and inspected before re-use.

lO.3 Construction Tolerance lO.3.l Out-Of-Roundness The walls of the completed Shield Building shall be substantially round.

The maximum plus or minus deviation from a true circular form, measured radially on the inside of the building, shall not exceed 3 inches (three inches),

10.3.2 Plumbness The cylindrical wall of the completed Shield Building shall be plumb within 4 inches (four inches) from top to bottom and shall be not more than 1 in£h (one inch) out of plumb in any 2S feet (twenty-five feet).

10.3.3 Azimuth and Elevation The location of any embedded items shall not deviate from the true.

azimuth or elevation shown on the drawings by more than 1 inch (one inch) measured along the outside surface of the building. Blackouts shall be located within 2 inches (two inches) tolerance in both azimuth and elevation.

2 10.3.5 Embedded Plates, Anchors and Inserts Embedded Plates, anchors, and inserts shall be accurately placed and securely held in position. A maximum tolerance of+/- I" (plus or minus 2

one inch) in azimuth., elevation or radius will be allowed.

10.4 Coating I

Forms of surfaces which will be exposed shall be coated with an approved parting agent before the reinforcement is placed. Surplus coating shall be removed prior to concrete placement. Forms for non-exposed surfaces may be thoroughly wetted with water in lieu of oiling, except that in cold weather with probable freezing temperature, the parting agent shall be used.

Parting agent shall not be used in the slip-form operation.

/3 10.5 Inspection Immediately before concrete is poured, all forms shall be carefully and thoroughly inspected to ensure that they are properly placed and braced, sufficiently rigid and tight, thoroughly clean, properly treated and free from foreign materials.

The CONTRACTOR shall, at his own cost, design all concrete mixes using the aggregates and materials available through the "Central Concrete Mix Plant" Contractor. The mixes shall be designed in accordance with ACI Standard 613 to produce a required strength of 15 percent over specified strengths for each class of concrete with maximum sizes of aggregate, slump and air contents as specified.

The CONTRACTOR shall furnish the CONSTRUCTION MANAGER with mix designs for approval prior to the manufacture of concrete. Such approval shall not relieve the CONTRACTOR of his responsibility for compliance with the proviSions of this Specification. Where directed by the CONSTRUCTION MANAGER to achieve higher early strength, or for other reasons, additional cement shall be added to the design mix. Calcium chloride shall not be used.

11.4 Temporary Lightning Protection This CONTRACTOR shall provide temporary lightning protection for the Shield Building while it is being erected and until the time the permanent protection is provided by others.

The protection shall be in accordance with requirements of the Underwriter's Laboratories and the National Fire Protection Association.

All materials shall be furnished by this CONTRACTOR except the required cable which will be furnished by the OWNERS.

11.5 Embedded Conduit for Wiring by Others The CONTRACTOR shaH furnish and install all embedded conduit and boxes for future wiring shown on drawing 7749-E-401 E.

I 1.6 Installation 0 f Electrical Conduit 11.6.1 Metallic Conduit 11.6.1.1 All metallic conduit shall be rigid, hot dipped galvanized steel conduit conforming with ANSI Standard C80.1, "Specification for Rigid Steel Conduit, Zinc Coated".

7749-C-38 Conduit embedded in concrete shall be rigidly supported and I 2 braced to avoid shifting during pouring of concrete.

Conduits shall have long-sweep field bends wherever possible 12 but shall in no case have smaller radium bends than are recommended by the manufacturer. All field bends shall be made with a bending machine or other approved device which will not reduce the internal diameter of the conduit or injure the protective coatings. The bend shall be free of kinks, indentations, or flattened surfaces. Heat shall not be applied. Standard bends shall be used where conduits tum out of the concrete and at conduit terminations.

Conduit shall be cut in an approved manner and threaded ends run on a suitable threading machine. Burrs shall be carefully removed with file, butt reamer or other approved means. Field threads on steel conduits shall be painted with an approved paint before fittings are attached.

Conduit runs shall be made up with all required condulets, couplings and other required fittings. Fittings shall be made up with full thread engagement, providing structural rigidity and low electrical resistance to ground across joints.

Metallic conduit run into boxes shall have two locknuts and a bushing unless otherwise shown on the drawings.

During installation, due precaution shall be taken to protect the conduits and conduit threads from mechanical injury.

The ends of conduits shall be sealed in an approved manner during installation whenever the work is interrupted, and runs shall be sealed upon completion by the use of caps and discs or plugs. The seals shall be maintained, except during inspection and tests, until the conductor is pulled in.

Conduits shall be inspected before conductor is pulled in.

Conduits shall be inspected before conductors are installed and thoroughly cleaned of water and dirt by means of compressed air, swabs, or other approved methods. Conduits shall be checked for freedom from obstntctions by pulling a mandrel of the proper size through the conduit. All boxes and fittings shall be kept closed and free from dirt, moisture, and debris.

II.7 Outlet, Terminal, Junction and Pull Boxes

11.7.1 All boxes shall be made of steel and double hot dipped galvanized after fabrication.

11.7.2 All conduit boxes shall be so located that covers and openings are easily accessible. They shall be installed parallel with building lines and where embedded shall be flush with the surface of the finished floor, wall, or ceiling. The CONTRACTOR shall, at no extra cost to OWNERS, remove and reset all boxes not properly installed or shifted out of line during concreting, wherever required or as directed.

12.0 ARCIDTECTURAL TREATMENT 12.1 The cylindrical wall of the Shield Building shall conform to the configuration shown on the design drawings.

13.0 SUBMITTALS Submittals shall be made in accordance with Form ED 6021,Section VIII. Specific Conditions, of the Contract Document.

13.1 Drawings 13.1.1 Forming and Shoring Drawings:

Drawings depicting the method of forming and shoring the Shield Building.

13.1.2 Placement Drawings:

Sections, plans and details showing location of rebar. blockouts and embedments.

13.1.3 Detail Drawings:

Drawings for details of construction such as bar list and bending diagrams. embedment fabrication assemblies, anchor bolts and weld details.

13.1.4 Drawings for Approval:

Forming and shoring drawings shall be submitted for approval by the CONSTRUCTION MANAGER.

Placement and detail drawings for reinforcing steel and embedded metals shall be submitted for approval by the ENGINEER.

3 13.1.5 Drawings to be Certified:

Placement and detail drawings for design coordination. shall upon completion of the detailing be certified to be correct with no further changes required. No alterations will be made to the details after certification without the approval of the CONSTRUCTION MANAGER and/or the ENGINEER.

13.1.7 One copy of each drawing, when finally approved, will be returned to the CONTRACTOR by the CONSTRUCTION MANAGER and/or the I ENGINEER and will be properly identified as approved.

3 13.1.8 The CONSTRUCTION MANAGER and/or the ENGINEER will check drawings for compliance with the design and construction only. The ENGINEER will not approve detail drawings, if, in his opinion, the WORK is not in accordance with this Specification.

13.1.9 Performance of any WORK and/or fabrication of any materials prior t-o obtaining approval of CONTRACTOR'S drawings is at the CON TRACTOR'S risk. Approval of CONTRACTOR'S drawings shall not relieve him from responsibility for deviations from the drawings and this Specification unless he has, in writing, called the ENGINEER'S attention to said deviations at time of submittal and received written approval of said deviations.

13.1.10 Record Drawings:

Concurrently with the progress of installation of material, the CONTRACTOR shall keep a set of record drawings consisting of a marked set of the ENGINEER'S design drawings. They shall denote and dimension accurately all changes in elevation, location and size of material deviating from the contract set of drawings. At completion of the WORK, the CONTRACTOR shall submit to the CONSTRUCTION MANAGER "as-built" shop and erection drawings corrected to show the WORK as finally installed. These shall be furnished as required in paragraph 38.0,Section V of this Contract Document.

13.2 Schedules 13.2.1 Detailing and Fabricating Schedule:

The CONTRACTOR shaH submit a fully itemized detailing, equipment and material procurement, fabricating (and construction) schedule in the form of a critical path method. The CONTRACTOR'S schedule shall include a monthly breakdown showing the time required for detailing, procurements, fabricating, delivery of materials to the site, and construction.

The CONTRACTOR shall submit a monthly progress report including a current schedule.

13.3 Records The CONTRACTOR shall furnish records of mill test reports, concrete delivery tickets and other test certificates as specified herein.

13.4 Field Procedures and Facilities 13.4.1 The CONTRACTOR shall submit plans and an outline of all phases of the slip forming and erection procedures. He shall locate on the plans all hoists. derrick and guy anchors, showing details of any special foundations required. The plan shall indicate storage and field fabricating areas and the movement of materials. Procedures for floating forms and methods-~--form~~----~

construction tolerances shall be fully described. All procedures and methods are subject to approval by the CONSTRUCTION MANAGER 13.4.2 The CONTRACTOR shall indicate what means of protection shall be used for protection of the WORK and the protection of other work and personnel during his operations.

The CONTRACTOR shall submit his design mix requirements to the CONSTRUCTION MANAGER for approval. The intention is to have the concrete supplier provide a suitable concrete mix in the quantity reasonably required by this CONTRACTOR to perform his WORK.

14.0 MEASUREMENT AND PAYMENT 14.1 The Lump Sum Price for the complete WORK for the Shield Building shall include all items specified herein. The unit prices bid in Paragraphs 2.2 and 2.3 of the Proposal shall be used for additions to or deletions from the Lump Sum bid in Items 2.1.1 and 2.1.2 respectively of the Proposal. Only net changes in quantities as directed by the CONSTRUCTION MANAGER or the ENGINEER shall constitute additions to or deletions from the Lump Sum bids.

DB-PF-03009 requires periodic Containment Vessel and Shield Building Inspections (every 3.33 years). During this Inspection, one small, area of distressed concrete (minor flaklng/spalling) was observed. This area has been previously identified in the maintenance rule evaluation of structures report and eRn 07-29203. Comparing the pictures from the maintenance rule inspection to the current pictures, shows no evidence of growth or change in the appearance of the affected area. This Is also being tracked by Not.1I600424184 and WO# 200288911.

The northeast drain cover on the shield building roof was found broken. The drain appeared to be working properly as there was no evidence of blockage.

Problem Statement:

During an inspection of the Containment Vessel and Shield Building, two small areas of distressed concrete (minor f1aking/spalling) was observed. The two areas both appear to be approximately 6" x 10" wide and 2" deep, and are located on the NW side of the Shield Building (upper right corner of the former RV Head entry cut out).

Evaluation:

These two locations have previously been reviewed/identified by Design Mechanical/Structural Unit during the performance of Maintenance Rule Evaluations of Structures during October 2005. The evaluation identified the same two locations Identified in this CR and stated the spalling was minor and does not pose any structural concerns.

Review of the pictures taken back on 10/14/05 and those attached to this CR taken 10/25/07 indicates that two spalling locations have not signifioantly changed. However, to protect the conorete's rebar from potential environmental induced corrosion, Notification No. 600424184 has been initiated to have Maintenance Rework these two areas. The two spalied areas of concern shall have any loose material removed and be reworked to their original condition per Davis-Besse Specification C-401Q, The two locations in question are located within the architectural flute at 292.5 degrees (Ref. Design Drawing C-110) and at an approximate elevation of 615'-0", which is 30 feet above grade level (Le. just to the right of the upper right corner of the recently replaced concrete located at the prior opening used to support the RV Head replacement).

A formal Corrective Action is not required to correct this deficiency per NOP-LP-2001, Section 4.1.6, which states the following:

The Work Management (WM) process and associated electronic database (I.e. SAP Notification) is the only other database that may be used to track resolution of Adverse Conditions in accordance with the limitations of this prooedure. The WM Process is considered part of the FENOC Corrective Action Process for resolving priority 600 to 800 maintenance orders originating from a CR. These Items are low significance/risk and will be tracked through resolution in the WM process.

Defect is actually in the Containment Shield Building. There is also a similar defect of about the I

same size located approximately 18" below and 7' to the left of the reported one. Per DB-OP-00018, N

o Inoperable Equipment Tracking Log, if the cumulative total of all known openings in the Shield Building Negative Pressure Area exceeds 100 square inches, the Emergency Ventilation System cannot maintain the required negative pressure and TS 3.6.5.2 for Shield Building Integrity must be entered. Currently, there are no openings in the Shield Building negative pressure boundary that are being tracked in the Inoperable Equipment Tracking Log. In the extremely unlikely event that the crack is uniform in size through the wall and extends all the way through the Shield Building wall, these two defects (approximately 5" x 1/2") represent a very conservatively estimated opening of 5 square inches. In addition, Containment Purge is in service, maintaining a negative pressure on the Mechanical Penetration rooms; therefore, any potential leakage will be into the negative pressure area and exhClusted through filtered and monitored ventilation. As a result, Shield Building integrity and the basis for TS 3/4.6.5.2 is being maintained and "Operable" is marked "Yes". Further investigation is required.

The Shield BuHding has a small area where the concrete is spalling.

EVALUATION:

The small areas of spalling concrete on the exterior of the shield building have been evaluated by OEMS.

These areas were identified in a Maintenance Rule walkdown for this structure performed in June of 1999. This evaluation concluded that thespalling was minor in nature and that it was not a structural concern. Therefore, this condition is "REWORK", MDT # Y0561 has been initiated to remove any loose material (I.e. eliminate any possible safety hazard) and rework the spalled area per Spec. C-401 Q.

APPARENT CAUSE:

The apparent cause of this condition is that the shield building construction opening required hand finishing on the exterior. This area has apparently developed cracks, which have gradually grown larger causing the spalling of concrete.. Minor surface detereioration, such as this condition, is a common occurrence on concrete structures..

Exhibit 10 POSSIBLE LIGHTNING STRIKE ON THE CONTAINMENT The following is list of events identifying the concern, investigation and resolution to the lightning strike on the containment building.

  • A server thunder storm occurred during the l/lte morning 011 May 10, 1995 Scverltl p(lUple witnessed lightning strike the lOp of the containment building and observed "something" falling from the containment.
  • Concurrent with the strike, the scismic trigger Wftll activated and scvcralnhulllli 011 the control room annunciator panel "nashed".
  • Operations ~',ent up on the Aux. Building roof and tound severnIsmall chunks of concrete lying in close proximity to each other at the Northwest corner of the roof The chucks of concrete arc shown in photographs # I and 1#2.
  • Operations requested assistancc from the Civil/Stmclural Unit (ltpprox. 1300 I-Irs) in cvaluiltion the condition of the containment.
  • The Civil/Structural Unit reviewed the concrete samples lind identitied to the Shifl Supervisor, Shift Manager and the Manager of Operatinns that due to the size and small quantity of concrete that a structural problem did not exit and that the concrete is most likely from the concrete cover over the rebar. [t was also identified that several surfaces of the concrete samples exhibited weathering (discoloration) that indicated it had been previously cracked before falling. The Manager of Operations requested that Design Engineering inspect the damaged area to confirm their uS!lumptions and recommend a rCJ,air (Note: It "Tornado Watch" WIIS in effect at this time IUld as soon as it was felt it was safe to climb to the top. engineering would so.).

In addition, it was agreed by Engineering and Operations that no seismic event occurred and that the activation of the seismic trigger was a result of either loud thunder of stray voltage.

  • Wednesday night (5-10-95) Jim Freels of Operations climbed to the tOl' of the containment roof to look for concrete damage. Ue located the origin of the concrete that fell. The location is the outer portion of the parapet wall on the Wast side as shown in photographs 3 and 4.
  • On the morning of S-J2.95. Operations, Design Engineering, Plant Engineering, the NRC residences and Safety climbed to the of the containment to inspect the concrete damage where photographs 3 and 4 were taken. Engineering confirmed the area had preexisting cracks above and below. There were no signs of a lightning strike (i.e.

charged. burned concrete). Design Engineering identified similar fltillire and repair on adjacent flute on the North side of the containment and identified that a grout repair was all that was required since no slnlctural damage occurred The peak seismic recorder on top of the containment was inspected and the data was retrieved.
