RIV000027 Submitted: December 22, 2011 Condition Report search for the word "Wall Thinning" associated with Contention 11, FAC The attached document is a summary report using words Wall Thinning of all condition reports; no culling for relevance has been done. This is not the approach taken for every other word search where the condition reports were reviewed and the actual CR was sent for your consideration. The reason for this is that there are 171 condition reports, and a brief scanning of the CRs indicating they seem to be relevant.

If you need a print out any condition report, please let me know and we will send it to you.

Wall measurement around the Unit 1 Integrated Liquid Waste Handling (ILWH) System release flow meter (CT-971) are below the minimum allowable. For a 3" schedule 40 pipe, the nominal wall thickness is 0.216". For the area immediately downstream of the orifice 2" away, wall thickness was measured to be 0.098". Other results are as follows:

I was able to get the UT information for you this evening. I took several readings upstream and downstream. I took readings at 0, 90, 180, & 270 at a distance of 2" and 6" from the weld. I also took several readings on the "pump" side of the wall on both painted and non-painted carbon steel pipe. Here are the results in inches:

Location 0 90N 180 270S 6" upstr .173 .156 .155 .168 2" upstr .160 .155 .151 .216 2" dnstr .110 .122 .098 .098 6" dnstr .149 .146 .134 .145 Pump Side .184 - .200 Painted

.200 Not Painted The localized wall thinning is probably attributed to the orifice operation over time. A rough estimate of the remaining wall appears to be adequate. However, Civil Structural Engineering should perform a structural evaluation of the piping system.

This inspection was associated with CR-IP2-2004-001 07 and CR-IP2-2003-05895 investigation. The documented condition impact accuracy of the flow instrumentation.

Great catch by iTi eddy current inspection team. During eddy current inspection of the 21A condenser tube bundle they identified some titanium tubes with unusually accelerated wall thining at one particular location. They entered the condenser to look at the steamside of the tubes and find reason for wall thinning. They found an MSR drain/blowdown line on the south east corner of the condenser with part of the pipe wall blown out allowing steam impingment on the condenser tubes.

Description:

Condition: Significant pipe wall thinning detected while performing FAC exam on CHECWORKS component HD-05.1 C-02R (expander downstream of valve 4HD-LCV-1117) under WO IP3-05-24892.

Discussion: Significant wall thinning is defined as a thickness less than 60% of pipe nominal wall thickness. Nominal pipe thickness for this component is 0.280 inches. Minimum measured thickness is 0.127 inches. This equates to 45.35% nominal wall remaining. This component has been evaluated as acceptable for continued service through the next refuel cycle.

Description:

During FAC inspection IP3-06-23430, component RHD-02.15A-02E, wall thinning was found on a reheater drain system 6" elbow.

Minimum wall thickness found was 0.10'1". The minimum required thickness for continued operation is 0.233". Nominal wall thickness for this component is 0.432". The wear is concentrated in the elbow extradose. The pipe downstream of the elbow shows some wear in the first 6", but then returns to nominal wall thickness downstream.

The location of this elbow is immediately downstream of a pipe leak found on 7/23/06, CR-IP3-2006-02270. which is to be replaced during 3R14 via WO IP3-06-18240.

During FAC inspection IP3-05-24843, component EX-02.14-07P was inspected and wall thinning was found on the upstream elbow, EX-02.14-06E during the pipe to upstream elbow weld scan. Upon further examination of the weld scan area, additional thinned areas were found on the extradose of the elbow. As the elbow is heavily corroded on the outside surface, this 'best effort' UT exam showed wall thickness of less than 0.170", however a properly prepped surface is required for accurate thickness measurements.. Minimum required thickness for this component is 0.256". The elbow was replaced in 1987 with FAC resistant chrome-moly material and is not expected to wear to this extent due to FAC.

The carbon steel pipe EX-02.14-07P showed little wear and is acceptable for continued service.

CR-IP3-2007-01516 documents the fact that during FAC inspection IP3-06-23456 on component FAC-07-VCD-09, wall thinning was found on a Vent Chamber Drain System 3" diameter elbow and its downstream pipe and reports an immediate action of, "Generated Work Order IP3-07-17176 and Scope Add Form to replace this elbow and pipe durina 3R14."

In the course of planning Work Order IP3-07-17176, found that Engineering Design Drawing No. 9321-F-29003-4 requires a 2" 45-degree elbow to be welded to the vent chamber discharge nozzle of Moisture Separator Reheater MSR-31 A and with dimensions as shown at coordinates H-2 of that drawing.

Contrary to that requirement, a 2" x 3" expander has been welded to the vent chamber discharge nozzle with dimensions other than as shown at coordinates H-2. It appears that this has resulted in an interference with the insulation on a proximate vertical riser pipe as well as exacerbating the Flow Accelerated Corrosion phenomenon at the 90-degree elbow.

The minimum wall thickness found was 0 .148" for the 2" pipe and 0.24 7" for the 3" pipe. The minimum required thickness for continued operation is 0.141" for the 2" pipe and 0.181" for the 3" pipe per calculation IP-CALC-07-00199. These pipes are acceptable for continued service.

The piping was inspected by UT measurements through the existing paint, requiring a conservative correction factor to be subtracted from the as measured thickness. The calculation also used conservative assumptions in the analysis of this piping. UT measurements found the piping to be uniform thickness with no areas of localized thinninn Since the minimum measured thickness is greater than the required thickness, the piping is structurally sound and is operable.

During the 1 year inspection of the 31 SJAE primary elbows A I B I C I D, work order 51477594-01, wall thinning was found on the D elbow.

There is no immediate operability concern.

The minimum thickness found, 0.148" is greater than the minimum required thickness of 0.090" per calculation IP3-CALC-CAR-01996, Rev. 0.

The location on the elbow of the minimum thickness (0.148") in the current inspection, was 0.191" during its previous inspection in December, 2006.

Based on the wear of this elbow since the previous inspection, design engineering needs to review and update the calculation IP3-CALC-CAR-01996, to determine the minimum required thickness and the remaining service life of this elbow.
