SUMMARY

On November 18, 2009, a Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise evaluation was conducted by the U.S. Department of Homeland Security (DHS)/Federal Emergency Management Agency (FEMA) for the 10-mile Emergency Planning Zone (EPZ) around the Clinton Power Station (CPS). The purpose of the exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.

The most recent exercise at this-site was conducted on September 12, 2007. The qualifying emergency preparedness exercise was conducted on January 13, 1987.

DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Illinois, the risk county of DeWitt and the support counties of Macon, McLean and Piatt participated along with State Government.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperation and teamwork on the part of all the participants were evident during this- exercise.

This Final Report contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence interviews and activities:

After Action Report/Improvement Plan Clinton Power Station Except where noted in this report, the State and local organizations demonstrated knowledge of and adequately implemented their emergency response plans and procedures.

There was one Deficiency identified during this exercise for the State of Illinois. The State received a Deficiency finding under Protective Action Decision-Making Criterion 2.b.2 - a decision-making process involving consideration of appropriate factors and necessary coordination is used to make Protective Action Decisions (PADs) for the general public including the recommendation for use of KI if Offsite Response Organization (ORO) policy.

The Deficiency assessed was based on our findings that both the performance and the decision-making processes used by the Unified Area Command (UAC) in the selection of Parkland College as the sole Reception Center resulted in a portion of the public in Sub-Area I being directed to use evacuation routes along the same path the plume was traveling, where they could have received unnecessary radiological exposure, thus compromising health & safety. There were no Deficiencies identified for the Counties of DeWitt, Macon, McLean or Piatt. Section 3 of this report, entitled "Analysis of Capabilities" presents detailed information on the Deficiency.

There was one Area Requiring Corrective Action (ARCA) identified during this exercise for DeWitt County. DeWitt County received an ARCA finding under Direction and Control Criterion 1.c. I - Key personnel with leadership roles for the ORO provide direction and control to that part of the overall response effort for which they are responsible. The ARCA assessed was based on our findings that within the DeWitt Emergency Operations Center (EOC) the key personnel with leadership roles did not consistently demonstrate effective direction and control of the overall response effort for which they were responsible.

There were no ARCAs identified during this exercise for the State of Illinois or the Counties of Macon, McLean or Piatt. Section 3 of this report, entitled "Analysis of Capabilities" presents detailed information on the ARCA.

There was one Plan Issue identified during this exercise for Radiological Emergency Assessment Center (REAC). The REAC received a Plan Issue under Emergency Information and Instructions for the Public and the Media criterion 5.b.1 - OROs provide accurate emergency information and instructions to the public and the news media in a timely manner. Residents of the Clinton 10-mile EPZ were not notified that they may need to ingest KI after such a recommendation was made for emergency workers and immobile persons. Information 5

After Action Report/lInprovement Plan Clinton Power Station contained within the Illinois Plan for Radiological Accidents (IPRA) and Radiological Task Force (RTF) procedure RTF-PARS-2 is not consistent regarding KI information that is provided to the public.

There were no Plan Issues identified for the State of Illinois or the Counties of DeWitt, Macon, McLean or Piatt. Section 3 of this report, entitled "Analysis of Capabilities" presents detailed information on the Plan Issue.

INTRODUCTION - EXERCISE BASIS On December 7, 1979, the President directed FEMA to.assume the lead responsibility for all offsite nuclear planning and response. DHS/FEMA's activities are conducted pursuant to 44 Code of Federal Regulations (CFR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness", 351 "Radiological Emergency Planning and Preparedness" and 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning."

These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

The FEMA Rule 44 CFR 350 establishes the policies and procedures for DHS/FEMA's initial and continued approval of State and local governments' radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.

DHS/FEMA's responsibilities in radiological emergency planning for fixed nuclear facilities include the following:

  • Taking the lead in offsite emergency planning and in the review and evaluation of RERPs and procedures developed by State and local governments;
  • Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments;
  • Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal

After Action Reportlhnprovement Plan Clinton Power Station Register, Vol. 58, No. 176, September 14, 1993); and Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

U.S. Department of Agriculture; U.S. Department of Commerce; U.S. Department of Energy; U.S. Department of Health and Human Services; U.S. Department of the Interior; U.S. Department of Transportation; U.S. Environmental Protection Agency; U.S. Food and Drug Administration; and

- U.S. Nuclear Regulatory Commission.

Representatives of these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.

Formal submiission of the RERPs for the Clinton Power Station to FEMA Region V by the State of Illinois and involved local jurisdictions occurred on November 25, 1986. Formal approval of these RERPs was granted by FEMA on August 5, 1987, under 44 CFR 350.

A REP Partial Participation Plume Exposure Pathway Exercise was conducted on November 18, 2009, and evaluated by DHS/FEMA to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Clinton Station. The purpose of this exercise report is to present the exercise results and findings on the performance of the Offsite Response Organizations (ORO) during a simulated radiological emergency.

The findings presented in this report are based on the evaluations of the Federal evaluation team, with final determinations made by the DHS/FEMA Region V RAC Chairperson, and approved by the DHS/FEMA Headquarters.

  • NUREG-0654/FEMA-REP-1, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants,"

November 1980;

  • FEMA "Radiological Emergency Preparedness: Exercise Evaluation Methodology; Notice" as published in the Federal Register Notice, Vol. 67, No. 80, dated April 25, 2002.

Section 1 of this report, entitled "Exercise Overview", presents information pertaining to the team that planned and coordinated the exercise. This section also provides listing of all participating jurisdictions and functional entities that were evaluated.

Section 2 of this report, entitled "Exercise Design Summary", contains the purpose and design of' the exercise, a description of the plume pathway EPZ and presents basic information and data relevant to the exercise scenario.

Section 3 of this report, entitled "Analysis of Capabilities," presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based, issues-only format. This section also contains: (I) descriptions of all Deficiencies and ARCAs (if any) assessed during this exercise, recommended corrective actions, and the State and local governments' schedule of corrective actions, if applicable, for each identified exercise issue; and (2) descriptions of unresolved ARCAs assessed during previous exercises and the status of the OROs' efforts to resolve them.

Section 4 of this report, entitled "Conclusion" presents the DHS/FEMA summary of overall exercise conduct and results as evaluated against the requirements of 44 CFR 350.

After Action Report/Improvement Plan Clinton Power Station EMERGENCY PLANNING ZONE (EPZ) DESCRIPTION The Clinton Power Station (CPS) is located approximately seven miles northeast of the City of Clinton in Harp Township, within DeWitt County, Illinois. DeWitt County is located near the geographical center of the State of Illinois. The topography of the area is predominantly rolling prairies.

The 10-mile EPZ contains a total population of 12,880 (2000 census) within the risk counties of DeWitt, Macon, McLean and Piatt. Nearly two-thirds of the total EPZ population (8,576) resides in the communities of Clinton, Weldon, Wapella and DeWitt. The land use within the EPZ is predominantly agricultural, with cash grain crops of corn and soybeans. The closest industries are located in the community of Clinton.

Recreational facilities outside the municipalities include the Clinton Lake State Recreation Area and the Weldon Springs State Park. The park encompasses approximately 370 acres and contains a 28-acre lake with camping, fishing, and picnicking facilities. Weldon Springs is one of the two sizeable lakes within ten miles of the site. Clinton Lake is a 4,895 acre man-made cooling lake within the CPS property lines. Recreational facilities accommodate camping, boating, hunting, fishing, and hiking.

The nearest major highways are State Highways 54, 10, and 48, all of which intersect the plant location. Other major thoroughfares are U.S. Highway 51, located about six miles west of the plant; and Interstate Highway 74, located about eleven miles northeast of the plant. The nearest railroad is the Illinois Central Gulf Railroad (ICG), which crosses the site east to west and comes within about three-fourths of a mile to the north of the reactor centerline. Another ICG track is located approximately three and a half miles south of the CPS.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region V Office evaluated the CPS Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise conducted on November 18, 2009, to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans (RERP) and procedures to protect the public health and safety during a radiological emergency involving CPS. The purpose of this report is to present the results and findings on the performance of the Offsite Response Organizations (OROs) during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix E "Exercise Plan."

2.3 Scenario Summary Appendix F "Scenario Details," contains a summary of the Exercise Scenario, a simulated sequence of events, that was used as the basis for invoking emergency response actions by Offsite Response Organizations (OROs) in the Clinton Power Station REP Partial Participation Plume Exposure Pathway exercise on November 18, 2009.

This exercise scenario was submitted by the State of Illinois and Exelon Nuclear and accepted by DHS/FEMA Region V on October 21, 2009.

During the exercise, in addition to information and data provided through the CPS onsite scenario, controllers from the State of Illinois provided "inject messages" containing scenario events and/or relevant data to., those persons or locations who would normally receive notification of such events. These inject messages were the method used for invoking additional specific response actions by OROs.

The details of the scenario are included in Appendix F "Scenario Details."

After.Action Report/lnmprovement Plan Clinton Power Station SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the November 18, 2009, REP Partial Participation Plume Exposure Pathway Exercise conducted to test the offsite emergency response capabilities of State and local governments in the EPZ surrounding the CPS.

Each jurisdiction and functional entity was evaluated based on its demonstration of exercise criteria delineated in Federal Register Notice: Vol. 67, No. 80, dated April 25, 2002. Detailed infonrmation on the exercise criteria and the extent-of-plýy agreements used in this-exercise are found in Appendix E "Exercise Plan" of this report.

Presented below are definitions of the terms used in this report relative to the criteria demonstration status:

  • M - Met: The status of a REP exercise Evaluation Area Criterion indicating that the participating Offsite Response Organization (ORO) demonstrated all demonstration criteria for the Evaluation Area Criterion to the level required in the extent-of-play agreement with no Deficiencies or ARCAs assessed in the current exercise and no unresolved prior ARCAs.
  • D - Deficiency: An observed or identified inadequacy of organizational perfonriance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant.
  • A - Area Requiring Corrective Action (ARCA) - An observed or identified inadequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety. Listing of the demonstrated exercise criteria under which one or more ARCAs was/were assessed during the current exercise or ARCAs assessed during prior exercises remain unresolved. Included is a description of any ARCAs assessed during this exercise and the recommended corrective action to be demonstrated before or during the next biennial exercise.

  • P - Plan Issue - An observed or identified inadequacy in the ORO's emergency plan or implementing procedures, rather than in the ORO's performance.
  • N - Not Demonstrated - Exercise criteria that were not demonstrated as scheduled during this exercise and the reason(s) they were not demonstrated.
  • Prior ARCAs - Resolved - Descriptions of ARCAs assessed during previous exercises that were resolved and the corrective actions demonstrated, in this exercise.
  • Prior ARCAs - Unresolved - Descriptions of ARCAs assessed during prior exercises that were not resolved in this exercise. Included is the reason the ARCA remains unresolved and recommended corrective actions to be demonstrated before or during the next exercise.

3.2 Summary Results of Exercise Evaluation The matrix presented in Table 3.1, on the following pages, presents the status of all exercise criteria from Federal Register Notice: Vol. 67, No. 80, dated April 25, 2002, which were scheduled for demonstration during this exercise by all participating jurisdictions and functional entities. The criterion status box is blank if it was not scheduled for demonstration.

This subsection provides information on the evaluation of each participating jurisdiction and functional entity in a jurisdiction-based, issues-only format.

The DHS has developed a standardized system for numbering exercise issues. This system is used to achieve consistency in numbering exercise issues among DHS Regions and site-specific exercise reports within each Region. It also is used to expedite tracking of exercise issues on a nationwide basis.

The identifying number of Deficiencies, ARCAs, and Plan Issues includes the following elements, with each element separated by a hyphen (-).

  • Plant Site Identifier - A two-digit number, corresponding to the Utility Billable Plant Site Code (13 for Clinton Power Station).

Exercise Year- The last two digits of the year the exercise was conducted.

Nov 2007) notes that "REAC will also recommend...that members of the general public with KI in their possession for such a contingency are advised to consider use of the drug through the JIC." Procedure RTF-PARS-2, "Potassium Iodide (KI) Recommendations," notes at Step 4.2.C that the REAC Manager is to "remind the Joint Information Center Radiological Liaison that the media should be advised to infonn the public that those with KI in their possession should consider consuming the drug at this time." The REAC Manager notified the JIC; however, no subsequent action was taken, nor was contemplated, by the JIC staff.

CONDITION: CPS is centrally located within Sub-Area 1, which includes the entire 5-mile radius around the plant. The initial wind direction was from the west at 260 degrees, so any release would travel in the direction of IL Route 54, the designated evacuation route to the reception center at Parkland College. During the evacuation the wind shifted from 260 degrees to 228 degrees, thus drifting the plume directly over IL Route 54. People to the west of CPS were advised to evacuate Sub-Area 1 via IL 54 East or IL 10 East to Parkland College. TACPs were located around the perimeter of the three (1, 3, and 4) affected Sub-Areas, but not within them, and therefore would have been ineffective as a means of preventing public exposure to radioactive material as people evacuated Sub-Area 1 via the designated evacuation routes. Anyone traveling along IL 54 East would have been traversing through the plume pathway, would have been immersed within the plume, and would have been accumulating unnecessary radiological exposure.

POSSIBLE CAUSE: There was insufficient coordination between the UAC and the Radiological Emergency Assessment Center (REAC) in determining which designated facility would be utilized as the reception center.

REFERENCE:

NUREG-0654/FEMA-REP- 1, J.9, 10.m EFFECT: A portion of the public in Sub-Area I was being directed to use evacuation routes along the same path that the plume was traveling thereby receiving unnecessary radiological exposure,thus compromising health & safety.

RECOMMENDATION: Review the decision-making process for the selection of the reception centers that would minimize or eliminate unnecessary radiological exposure to the general public.

CORRECTIVE ACTION DEMONSTRATED: The Illinois Unified Area Command (UAC) successfully demonstrated its capability to use a decision-making process in a timely manner including considering appropriate factors to activate Reception Centers and provide evacuation routes. The successful demonstration occurred during the re-demonstration drill to rectify the Deficiency detected during the Clinton 24

After Action Report/Improvement Plan Clinton Power Station Power Station (CPS) Radiological Emergency Preparedness Exercise conducted on Wednesday, November 18, 2009. The re-demonstration drill was held on January 28, 2010 at the Illinois State Emergency Operations Center (State EOC).

The state of Illinois had the necessary staff pre-positioned and activated at the State Incident Response Center (SIRC), Radiological Emergency Assessment Center (REAC) and the UAC which was staged in a separate area of the State EOC. The Illinois Emergency Management Agency (IEMA) controller provided a briefing and hand-out containing general information regarding the situation and the actions taken up to the point of play just prior to the re-demonstration drill. This information included Nuclear Accident Reporting System (NARS) forms of State Message No. 1, Site Area Emergency (SAE) and State Message No. 2, General Emergency (GE).

The GE NARS information included: the GE declared by CPS at 1008, radioactive gas release occurring, wind direction from 100 degrees with a speed of 6.0 mph, Protective Action Recommendations (PARs) of evacuation for Sub-Areas 1, 7 and 8 as well as the recommendation to administer Potassium Iodide (KI) per procedures.

The UAC Incident Commander (IC) held a briefing with the UAC staff to communicate the conditions of the emergency. The brief included discussion of Sub-Areas 1, 7 and 8 to be evacuated and the possibility of Sub-Areas 2 and 6 could be affected should the wind shift. The IC further stated that the IEMA Radiological Assessment Field Teams (RAFT) staging area was Maroa, IL, the need for the UAC to plan for evacuation of the affected Sub-Areas, establish communication with Department of Transportation (DOT) to evaluate any impedirments to evacuation and request that Illinois State Police (ISP) and the affected counties prepare to post Traffic Access and Control Posts (TACPs).

The UAC team, consisting of the IC, Planning Chief (PC) and Radiological Response Chief (RRC) discussed the possible evacuation routes for Sub-Area I and which reception centers should be designated to receive the public. They noted that the public located in Sub-Area 8 and Sub-Area I west and north of Clinton would evacuate north to Horton Field House, public located in Sub-Area 7.and Sub-Area I west and south of Clinton, would evacuate south to Stephen-Decatur Middle School and public located in Sub'-Area I east of Clinton would evacuate east to Parkland 25

The discussion then focused the evacuation routes and based on plume travel and reception center locations, the IC determined that Parkland College would not be needed. This was challenged by the RRC noting that all three should be opened so no evacuees would traverse the plume. The RRC suggested that they should direct Parkland College to be designated as a reception center so that evacuees east of Clinton would travel east to Parkland College and be traveling in the opposite direction of the projected plume. The PC was asked to contact ISP and verify that all evacuation routes were clear and passable and that no routes were restricted.

At 1036 hours0.012 days <br />0.288 hours <br />0.00171 weeks <br />3.94198e-4 months <br />, the IC contacted the REAC Manager for a briefing of radiological conditions and discussed the UAC plan for evacuating the public from Sub-Areas 1, 7, and 8. The briefing included current and projected radiological conditions, division of Sub-Area 1 for evacuation, the designation of Highway 54 as the dividing line between north and south in Clinton, the thought process by which the IC chose which facilities to be activated as reception centers, directing RAFT to open the designated reception centers, and that-Emergency Workers within the 10-mile Emergency Planning Zone (EPZ) to take KI. During discussions with the REAC.

Manager the IC reaffirmed that Parkland College would not be required to be activated as a reception center.

The area around the CPS was restricted as ano-fly zone, rail traffic had been restricted and not allowed in the CPS EPZ and local recreational areas had been evacuated. The IC and State EOC Manager also discussed weather conditions, possible effects if weather conditions changed and ensuring that there had been contact with the National Weather Service for the most recent weather pattern predictions.

The RCC again suggested that Parkland College should be activated as a reception center and the PC agreed based on his calculation of the number of evacuees and the capacity of Horton Field House and Stephen-Decatur Middle School as reception centers. As the call ended, the RRC asked what the current conditions were and noted that he did not have the understanding that Parkland College had been directed to activate as a reception center. He concurred with activation of Parkland College and stated that evacuation routes need to be evaluated as to not direct evacuees to traverse through the plume. The UAC staff then discussed the current evacuation plan, routes of travel and verification that no impediments to evacuation existed.

After Action Report/lInprovernent Plan Clinton Power Station SECTION 4: CONCLUSION The U.S. Department of Homeland Security/Federal Emergency Management Agency identified a Deficiency finding during the Clinton Power Station's Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise, conducted on November 18, 2009.

A Deficiency is defined by DHS/FEMA as an observed or identified inadequacy of organizational performance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant.

The State received a Deficiency finding under Protective Action Decision-Making Criterion 2.b.2 - a decision-making process involving consideration of appropriate factors and necessary coordination is used to make Protective Action Decisions (PADs) for the general public including the recommendation for use of KI if Offsite Response Organization (ORO) policy.

The Deficiency assessed was based on our findings that both the performance and the decision-making processes used by the Unified Area Command (UAC) in the selectionof Parkland College as the sole Reception Center resulted in a portion of the public in Sub-Area I being directed to use evacuation routes along the same path the plume was traveling, where they could have received unnecessary radiological exposure, thus compromising health & safety.

Because of the potential impact this finding has on public health and safety, a remedial exercise was conducted and successfully completed by the State on January 28, 2010.

1. Contraindications for the issuance of KI for ingestion Summary: During the Clinton Power Station Exercise conducted November 17, 2009, a controller inject message was given to a law enforcement representative located in the EOC. The inject stated that a law enforcement deputy in the field had concern regarding the order to ingest Potassium Iodide (KI), specifically 1OSAT TM. The deputy in the field had a known allergy to shellfish and the deputy was pregnant. The law enforcement representative in the EOC reviewed the Issuance of KI to Emergency Workers procedure, and then reaffirmed the recommendation for the deputy to ingest the KI tablet.

Description:==

The exercise evaluator questioned if the response that was provided by the EOC law enforcement representative was accurate. The exercise evaluator conferred with the EOC exercise controller who indicated that the response provided by the law enforcement representative was incorrect. Both the evaluator and the controller were under the assumption that the ingestion of KI with a known allergy to shellfish or a known pregnancy was contraindicated. Their assumption was based on training that they had previously received in their careers.

FEMA staff reviewed the Consumer Package Insert from the manufacturer of iOSAT TM. The review determined that it is not contraindicated toingest KI if one has a known allergy to shellfish or if one is pregnant.

Upon further investigation, the following information was obtained from the Food and Drug Administration (FDA), Center for Disease Control (CDC) and the three FDA approved manufacturers of KI (iOSAT TM, ThyroSafe TM, ThyroShield TM).

Who needs to take potassium iodide?

  • Infants (no. repeat dosing) and children
  • Pregnant & nursing females (no repeat dosing)
  • All ages 18-40
  • Over 40 ONLY if predicted exposure is high enough to destroy thyroid & induce lifelong hypothyroidism (this group also has greater change of having allergic reaction) 39

LESSON LEARNED Contrary to the assumption made by the evaluator and controller, pregnant and nursing females may take one dose of KI. Additionally, a shellfish allergy does not necessarily indicate a known sensitivity or allergy to iodine.

Individuals that should not take KI include those with, persons with certain skin disorders, and persons with thyroid disease.

RECOMMENDATION To ensure compliance with the manufacturer recommendation for the ingestion of KI:

1. Review and update all training materials.
2. If applicable, supply updated information .to all affected personnel such as, but not limited to:
  • Instructors
  • Emergency Responders
  • General Public
  • Exercise scenario developers, controllers, and evaluators.

After Action Report/Improvement Plan Clinton Power Station APPENDIX F: EXERCISE PLAN This appendix lists the exercise criteria, which were scheduled for demonstration in the Clinton Power Station Radiological Emergency Preparedness Partial Participation Plume Exposure Pathway Exercise on November 18, 2009.and the offsite extent-of-play agreement accepted by DHS/FEMA Region V on October 21, 2009. The exercise criteria, contained in the FEMA "Radiological Emergency Preparedness Exercise Evaluation Methodology; Notice," as published in the Federal Register Notice/Vol. 67, dated April 25, 2002, represent'a functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA-REP-1, Rev], "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980. Because the exercise criteria are intended for use at all nuclear power plant sites, and because of variations among offsite plans and procedures, an extent-of-play agreement is prepared by the State and accepted by DHS/FEMA to provide evaluators with guidance on expected actual demonstration of the criteria.

Exercise Criteria Listed on the following pages are the specific radiological emergency preparedness criteria that were scheduled for demonstration during this exercise.

Criteria that may be re-demonstrated, as approved on a case-by-case basis by the Chairperson of the Regional Assistance Committee, include the following: 2.a. l, 2.b. l, 2.b.2, 5.a.1 and 5.b. 1.

EVALUATION AREA 1 - EMERGENCY OPERATIONS MANAGEMENT Criterion L.a.l: OROs use effective procedures to alert, notify and mobilize emergency personnel and activate facilities in a timely manner.

State of Illinois: The State Emergency Operations Center (SEOC), which houses the IEMA Communications Center (Initial Warning Point), the State of Illinois Response Center (SIRC) and the Radiological Emergency Assessment Center (REAC), will be activated. The IEMA Emergency Response Telecommunicator will implement procedures for notification of IEMA staff as defined in IEMA SOPs. The SIRC will be partially activated with IEMA personnel.

REAC will be fully activated. Rosters will be provided to demonstrate 24-hour staffing of the SIRC and REAC. The evaluator will not be allowed to remove the rosters.

The Unified Area Command (UAC) will be activated with State district and regional personnel at the Central Illinois Regional Airport in Bloomington. Radiological Assessment Field Team (RAFT) will be activated for training purposes. IEMA personnel will be prepositioned in the local'area and dispatched to the County EOCs, UAC, EOF, Technical Support Center (TSC) and the JIC. FEMA will not evaluate the TSC.

An actual phone call will be made to the FEMA duty officer advising them of the situation at Clinton Station. Follow up calls to FEMA will be simulated.

The prepositioned State staff will begin exercise play no earlier than the Alert.

Dewitt County: DeWitt County intends to fully activate the Emergency Operations Center (EOC) in Clinton, Illinois. The County will implement notification procedures as defined in IPRA-Clinton. Representatives from the EPZ municipalities, IEMA and Exelon will staff the County EOC but will not be pre-positioned in the EOC. A roster will be provided to demonstrate 24-hour staffing of the EOC. Evaluators will not be allowed to remove the roster from the EOC.

An evaluator will be positioned at the County Sheriff s- Communications Center (initial warning point) at the start of the exercise..

Macon County: Macon County intends to activate the Emergency Operations Center (EOC) in Decatur, Illinois, to the appropriate level. The County will implement notification procedures as defined in IPRA-Clinton. The EOC will be staffed with representatives from the County Board, EMA, and the Sheriffs Department. If the scenario does not drive proteýive actions in Macon County, the County may decide to down staff the EOC to minimum staffing. A roster will be 49

If the scenario does not drive protective actions in Piatt County, the County may decide to down staff the EOC to minimum staffing. A roster will be provided to demonstrate 24-hour County staffing of the EOC. Evaluators will not be allowed to remove the roster from the EOC.

Criterion 1.b.l: Facilities are sufficient to support the emergency response.

This criterion will not be demonstrated during this exercise.

Criterion 1.c.1: Key personnel. with leadership roles for the ORO provide direction and control to that part of the overall response effort for which they are responsible.

State of Illinois: The ability to direct and control emergency response activities will be demonstrated at the SIRC, REAC, the UAC and the JIC. Coordination will be demonstrated between the SIRC, the REAC, the UAC, the JIC, the EOF and the county EOCs.

Dewitt County: Decision making will be demonstrated at the DeWitt County EOC.

State of Illinois: IEMA will use a variety of communication systems [Nuclear Accident Reporting System (NARS), telefax, commercial telephone, cellular phone, radio] to communicate with other organizations and locations.

Dewitt County: DeWitt County will use NARS, commercialtelephone, radio and telefax to communicate with departments and agencies at other locations.

Macon County: Macon County will use commercial telephone, radio and telefax to communicate with departments and agencies at other locations.

McLean County: McLean County will use commercial telephone, radio and telefax to communicate with departments and agencies at other locations.

Piatt County: Piatt County will use commercial telephone, radio and telefax to communicate with departments and agencies at other locations.

Criterion L.e.l: Equipment, maps, displays, dosimetry, potassium iodide (KI) and other supplies are sufficient to support emergency operations.

State of Illinois:

The State of Illinois will demonstrate the use of equipment, maps and displays to support emergency operations. Dosimetry Control Officer (DCO) logs will be available in each County EOC.

A DCO briefing/Traffic and Access Control (TAC) interview will be conducted by the Illinois State Police on November 17, 11 a.m. (confirmed) at the ISP District 6 Headquarters in Pontiac.

No more than 10% of the ISP District 6 dosimetry kits will be inspected during the interview to confirm serial numbers.

Dewitt County: DeWitt County will demonstrate the ability to support operations through the use of maps, status boards and other displays as appropriate. DCO logs will be available from the IEMA Radiological Advisor in the DeWitt County EOC. The availability of dosimetry and KI will be demonstrated during the traffic and access control demonstration and the school interview. The DCO briefing for the deputy demonstrating traffic and access control will be conducted at the DeWitt County EOC. The DCO briefing for the Clinton Police officer demonstrating traffic and access control will be conducted-at the Clinton Police Department.

Macon County: Macon County will adequately demonstrate the ability to support operations through the use of maps, status boards and other displays as appropriate. The availability of dosimetry and KI will be demonstrated during the traffic and access control demonstration. The DCO briefing for the deputy demonstrating traffic and access control will be conducted at the County EOC.

McLean County: McLean County will adequately demonstrate the ability to support operations through the use of maps, status boards and other displays as appropriate. The availability of dosimetry and KI will be demonstrated during the traffic and access control demonstration. The DCO briefing for the deputy demonstrating traffic and access control will be conducted at the County EOC.

Piatt County: Piatt County will adequately demonstrate the ability to support operations through the use of maps, status boards and other displays as appropriate. The availability of dosimetry and KI will be demonstrated during the traffic and access control demonstration. The DCO briefing for the deputy, demonstrating traffic and access control will be conducted at the County EOC.

EVALUATION AREA 2 - PROTECTIVE ACTION DECISION MAKING Criterion 2.a.l: OROs use a decision-making process, considering relevant factors and appropriate coordination, to ensure that an exposure control system, including the use of K1, is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides.

State of Illinois: The ability to make the decision to recommend the use of KI to emergency workers will be demonstrated in REAC.

A message will be injected to the IEMA Radiological Advisor in the DeWitt County EOC to drive the decision to authorize exposure in excess of administrative limits.

Criterion 2.b.l: Appropriate protective action recommendations are based on available information on plant conditions, field monitoring data and licensee and ORO dose projections, as well as knowledge of on-site and off-site environmental conditions.

State of Illinois: The REAC will evaluate the licensee information and complete independent dose projections based on that information and simulated field monitoring data. REAC will make an evaluation of the data and make a protective action recommendation to the SIRC.

This criterion does not apply to the counties.

Criterion 2.b.2: A decision-making process involving consideration of appropriate factors and necessary coordination is used to make protective action decisions (PADs) for the general public (including the recommendation for the use of KI, if ORO policy).

State of Illinois: The coordination to determine protective action recommendations (PARs) based upon available shelters, evacuation time estimates and other relevant factors will be demonstrated between the SIRC and REAC. The State PAR will be transmitted to DeWitt County via NARS.

The capability to determine which centers should be activated for monitoring, decontamination, registration reception and congregate care will be demonstrated at UAC, with input from the SIRC and REAC.

Dewitt County: The State of Illinois will provide protective action recommendations to DeWitt County officials in the County EOC. The DeWitt County officials will make the final decision regarding what protective actions will be recommended to the public.

This function is responsibility of DeWitt County and this criterion does not apply to Macon, McLean or Piatt counties.

Criterion 2.c.l: Protective action decisions are made, as appropriate, for special population groups.

State of Illinois: The coordination to determine protective action recommendations (PARs) based upon available shelters, evacuation time estimates and other relevant factors will be demonstrated between the SIRC and REAC. The State PAR will be transmitted to DeWitt County via NARS.

This function is responsibility of DeWitt County and this criterion does not apply to Macon, McLean or Piatt counties.

Criterion 2.d.l: Radiological consequences for the ingestion pathway are assessed and appropriate protective action decisions are made based on the ORO's planning criteria.

This criterion will not be demonstrated during this exercise.

Criterion 2.e.l: Timely re-location, re-entry and return decisions are made and coordinated as appropriate, based on assessments of the radiological conditions and criteria in the OROs plan and/or procedures.

This criterion will not be demonstrated during this exercise.

State of Illinois: This will be demonstrated during the ISP DCO briefing/TAC interview will be conducted on November 17, 11 a.m. (confirmed), at the ISP District 6 Headquarters in Pontiac.

Dewitt County: DeWitt County will demonstrate this criterion by close coordination with the IEMA Radiological Advisor and timely dissemination and exchange of information within the County Dosimetry Control Officer network. Exercise messages will be injected which will ensure the opportunity to demonstrate this criterion in the County EOC.

This criterion will also be demonstrated during the traffic and access control demonstrations and the school interview. The DCO briefing for the deputy demonstrating traffic and access control will be conducted at the DeWitt County EOC. The DCO briefing for the Clinton -Police officer demonstrating traffic and access control will be conducted at the Clinton Police Department.

Macon County: Macon County will demonstrate this criterion by close coordination with IEMA and timely dissemination and exchange of information within the County Dosimetry Control Officer network.

Dewitt County: When notified by the State of Illinois to distribute and administer KI, DeWitt County officials will discuss the issuance of KI to emergency workers and immobile populations, as a voluntary measure, and will notify the municipalities of the State's recommendation.

This criterion will also be demonstrated during the traffic control demonstrations and the school interview. The DCO briefing for the deputy demonstrating traffic and access control will be conducted at the DeWitt County EOC. The DCO briefing for the Clinton Police officer demonstrating traffic and access control will be conducted at the Clinton Police Department.

State of Illinois: This criterion does not apply to the State of Illinois.

Dewitt County: This will be demonstrated by DeWitt County through the simulated implementation of the appropriate procedures. The appropriate officials will coordinate transportation and receiving facilities for special populations. One of each type of special facility will be contacted during the exercise. One of each type of transportation provider (ambulance/bus) will be contacted during the exercise. The public will not be involved in the demonstration.

The Mobility Impaired List will be available for inspection in the County EOC during the exercise. The evaluator will not be allowed to remove the list from the EOC.

This function is responsibility of DeWitt County and this criterion does not apply to Macon, McLean or Piatt counties.

Criterion 3.c.2: OROs/School officials decide upon and implement protective actions for schools.

State of Illinois: This criterion does not apply to the State of Illinois.

Dewitt County: DeWitt County officials will demonstrate this capability through the simulated implementation of the appropriate procedures in the county EOC. The EPZ schools and transportation providers will be contacted during the exercise.

A school interview will be conducted with the Deland-Weldon School District on November 17, 9 a.m. (confirmed), in the District Office. The District Superintendent, a Principal, a Teacher, the transportation coordinator (DCO), the maintenance supervisor and a bus driver will be present at the interview.

This function is responsibility of DeWitt County and this criterion does not apply to Macon, McLean or Piatt counties.

Criterion 3.d.]: Appropriate traffic and access control is established. Accurate instructions are provided to traffic and access control personnel.

State of Illinois: This will be demonstrated by the State through coordination between the UAC and the County EOCs, in accordance with the Clinton Station EPZ Traffic and Access Control procedures. The staffing of the State posts will be simulated during the exercise.

A Sheriffs deputy will be available during the exercise to demonstrate the staffing of one post. The post for the demonstration will be selected by DeWitt County during the exercise based on exercise play. The deputy will drive to the selected post location for the demonstration. The staffing of the remaining posts will be simulated.

A Clinton Police officer will be available to demonstrate the staffing of one post. The post for the demonstration will be selected by the Clinton Police Department during the exercise based on exercise play if the scenario causes Clinton to staff posts. If it does not, the post will be selected by the evaluator.

Macon County: Coordination between Macon County and DeWitt County will be demonstrated. A Sheriffs deputy will be available during the exercise to demonstrate the staffing of one post. The post for the demonstration will be selected by Macon County during the exercise based on exercise play if the scenario causes Macon County to staff posts. If it does not, the post will be selected by the evaluator. The deputy will drive to the selected post location for the demonstration. The staffing of the remaining posts will be simulated.

The initial instructional message to the public must include as a minimum the elements required by current DHS REP guidance.

State of Illinois: The State's role in demonstrating this criterion is. limited to transmitting the State's protective action recommendation (PAR) to the counties and monitoring the instructional messages provided to the public by the counties. This monitoring will be accomplished, by IEMA representatives in the DeWitt County EOC.

Dewitt County: DeWitt County will demonstrate the capability to provide both an alert signal and an initial instructional message to populated areas throughout the 10-mile plume pathway EPZ .in a timely manner after their decision to activate the alert and notification system to implement protective action recommendations (PAR).

This capability will be demonstrated through the simulated activation of the Clinton Station EPZ Siren Warning System as defined in IPRA-Clinton. The Siren Warning System (outdoor warning sirens) will not be activated during the exercise.

The ability to provide instructions to the public will be demonstrated through the simulated activation of the local radio station (WBNQ/WBWN/WJBC). The local radio station will be contacted during the first, and possibly the second, alert and notification sequence. An evaluator and a controller will be positioned at the radio station during the exercise to monitor for a message containing a PAR. If the initial message does not contain a PAR, the evaluator and controller will stay at the radio station for the second message.

This function is responsibility of DeWitt County and this criterion does not apply to Macon, McLean or Piatt counties.

After Action Report/Improvement Plan Clinton Power Station APPENDIX G: SCENARIO DETAILS This appendix contains a summary of the Exercise Scenario, a simulated sequence of events, that was used as the basis for invoking emergency response actions by Offsite Response Organizations (OROs) in the Clinton Power Station REP Partial Participation Plume Exposure Pathway exercise on November 18, 2009.

Results of a technical review of the scenario, submitted by the State of Illinois and Exelon Nuclear on September 17, 2009, indicated that the scenario was adequate to support demonstration of Department of Homeland Security's requirements, as well as criteria selected by the Offsite Response Organizations provided in the State's July 18, 2009, extent-of-play submission.

This exercise scenario was accepted by DHS/FEMA Region V on October 21, 2009.

During the exercise, in addition to information and data provided through the CPS onsite scenario, controllers from the State of Illinois gave "inject messages" containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events. The inject messages were the method used for invoking additional specific response actions by OROs.

DHS/FEMA concurred with the offsite scenario and extent-of-play submission for the State of Illinois, DeWitt, Macon, McLean and Piatt Counties for the November 18, 2009 Biennial Radiological Emergency Preparedness Exercise for the Clinton Station.

Clinton Power Station ALERT ScenarioSummary 0810-0934 Anticipated Trip Without Scram (ATWS)

The Clinton Power Station (CPS) is operating at 100% power when a Turbine Trip results in an automatic scram (shutdown) initiation. An Anticipated Trip Without Scram (ATWS) occurs as automatic Reactor Protection System (RPS) actions fail to result in a reactor scram. The Control Room Operating Crew responds to the scram signal by recognizing that all control rods did not insert into the reactor core. Actions are initiated to address the ATWS and the appropriate emergency procedures are entered to address the situation. Manual intervention results in a reactor scram and the reactor is shutdown. The Emergency Director declares an Alert based on MA3 (Automatic scram signal failing to initiate a reactor scram). CPS notifies the state through the Nuclear Accident Reporting System (NARS).

At 0850, Air in-leakage results in low condenser vacuum. Additionally, an inadvertent Group 1 Isolation occurs and the Main Steam Isolation Valves close. The Operationscrew uses Safety Relief Valves (SRVs) to remove decay heat from the reactor. By 0910, Drywell Radiation monitor readings increase to greater than 260 R/hr and Containment Radiation monitor readings increase to greater than 45 R/hr indicating developing fuel failure.

The wind direction (from 260 degrees) induces the plant to recommend via the NARS form the evacuation of Sub-Areas 1, 3 and 4.
