How It HapDened?

The activity to crosstie the Unit 1 B03 and B04 busses was originally scheduled to be performed during A train RHR work during the core-flooded window. There was no schedule logic tie for bus work completion to the RHR train restoration.

Potential for unqualified workers due to revision of training documents With the recent incorporation of revision three (3) of FP-T-SAT-41, a training program administrator identified that the procedure specifically addresses who is responsible for granting qualification: the Training Program Owner (TPO). This guidance has changed from previous revisions.

Revision 5 of the OJT/TPE template (QF-1030-12) clearly requires the TPO to sign for the completion of the guide prior to returning to Training for incorporation into the respective matrix and into the Registrar database. However, previous revisions (1, 2 or 3) of the templates state that a "manager/supervisor" can sign for this.

This constitutes an error likely situation. The potential exists for confusion between the recently revised procedure and the older OJT/TPE guides. This creates the potential for unqualified workers. The best interim actions that can be taken are two fold:

1) Ensure that all managers and supervisors are aware that they cannot sign as TPO unless they are designated as the TPO.
2) Using the guidance in AM 4-1, create a minimum number of designated individuals that can act as the TPO to qualify workers to perform work. This should be one or two individuals would have this authority during periods where the TPO is off site.

The long-term action is to revise the OJT/TPE guides to remove this discrepancy. This will be done slowly as the need arises to revise them. Until then the situation needs to be known and clarifications put in place that the procedure is the guiding document.

Speaking Points - CAP Extensions Due to Missed Training

  • In the event a CAP action item (RFT, CA, OTH, etc.) cannot be completed by the due date, a due date extension needs to be requested per NP 5.3.1, Attachment 4, which states 'When the individual responsible for an activity determines that the assigned due date is inappropriate or cannot be met, they shall return the activity request, with the appropriate justification, to the CARB or Site VP, as appropriate".
  • For example, if an RFT that requires training for a specific population cannot be closed out by the due date due to some outstanding makeup training, an extension will need to be requested. It is not the training department policy to close out the original RFT and create a new RFT with a new due date to track makeup training. To do this is, in effect, bypassing the requirements of NP 5.3.1.
  • This policy applies to CAP action items, but the practice of closing partially completed items and creating new items to track the remainder of the action is highly discouraged for non-CAP items.
  • T-track due dates should be established - 2 months after training is projected to be completed to allow for proper closeout.

5/28/2004 Security Drill The purpose of this drill is to test the procedural and communications interfaces between the Security and Emergency Plans.

OBJECTIVES

SUMMARY

  • Integration of Security threat analysis with existing EPIPs (ClassificationlNotification)
  • Response of Security Personnel when implementing both plans Communications interfaces
  • Demonstrate the ability to DETECT and properly ASSESS plant conditions and parameter trends which warrant an emergency classification, and to PERFORM core damage assessments and Demonstrate the ability to assess Emergency Action Level parameters and correctly classify the emergency.
  • WARN all personnel within the Owner-Controlled Area of emergency classification(s) and advise on expected actions. (Management of onsite personnel)

The drill is not expected to mimic or replace force on force requirements. Security participation is limited to communications, decision-making and expected ERO support.

The drill commences a situation that requires Security to notify the simulator control room and Operations enters the security Abnormal Operating Procedure, AOP-29. This procedure directs Operations to make the announcement for all personnel onsite that a security event is in progress and to take cover until further notice. It is expected that all site personnel not involved with Ul critical path work will participate by performing "duck and cover" actions.

Immediately following the announcement that the immediate security threat has been terminated, a controller will call the CAS (Security Office) as the NRC and inform the CAS Operator of a credible threat to the site.

Participation includes the following:

Background:

PBNP is committed to the requirements of the ASME OM Code (95E/96A) for our Inservice Testing Program. Paragraph ISTC 4.2.2.g states in part that valve exercising during cold shutdown shall commence within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> of achieving cold shutdown and continue until all testing is completed or the plant is ready to return to power. This means that if a unit enters a forced cold shutdown PBNP must start to perform cold shutdown frequency IST valve testing within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />.

Implementation: The following is the recommended list of IST Cold Shutdown Frequency test procedures that the IST Engineer recommends should be performed if PBNP finds itself in a forced cold shutdown condition. This list should only be considered a recommended starting point and discussions between Operations and IST Engineering are recommended to determine if any specific tests would be advantageous in determining the condition of any component that may have contributed to the forced cold shutdown. This IST Engineer should also assure that a variety of Cold Shutdown Procedures be preformed on subsequent forced cold shutdowns. Note that this list does not apply to refueling outages where all cold shutdown frequency tests are required to be performed prior to returning to power.
