PCB 24B Failures z 8/14/2004 - Hurricane Charley exiting service area z Transmission Line Insulator Failed z PCB 24B cycled open as designed, but fault current was erroneously still detected which resulted in B 230KV bus lockout; LOOP to U1.

z PCB 24 B was repaired and returned to service.

2nd Failure; Reason for Fault z October 2004 - PCB 24B failed again.

z Based on Visual Inspection and Vendor Input

- B Phase Bus Side Bushing Experienced Internal Fault z Bushing Sent to ABB for Analysis z Determined that standard pressure test had not been performed & would have identified assembly error z Newly Installed & Recent Shipment (3) Rebuilt Bushings were pressure tested within spec.
