0 This Condition Report is generated based on Inspection of the Incore Instrumentation Guide Tubes R

performed under Inspection Plan IP-M-030. The requirements of NG-EN-00324 need to be considered in the evaluation of this CR.

G The below observations were made from the Inspections of the 52 incore Instrumentation guide N

tubes performed using the magnetic wheeled crawlor.

A

1. Remnants from what appears to be tape were found on tubes 22, 33, 38, 49, and 51. A tape-like l

residue was found on several other tubes and on the adjacent reactor vessel surface.

2. Tubes 1. 12, 13. 14, 42, 43, 44, and 45 are in the path of the rust/corroslon stain down the side of N

the reactor vessel between approximately 270 to 305 degrees (CR's 02-01690 and 02-02498). On several of these tubes, the penetration annulus at the vessel was completely covered by this stain.

3. A small trails of boric acid residue were found on the bottom of the reactor vessel. Tubes 1. 3, 27,
28. and 32 are In the path of this trail and have boric acid residue around the penetration annulus and tube surfaoe.
4. Light corrosion was noticed around numerous tube penetrations. This corrosion was similar to that noticed on other areas on the surface of the reactor vessel with chipped or peeled paint.

There were no 'popcorn boron crystals, that were indicative of CRDM nozzle leakage, found around any of the incore nozzles. Details of the inspection results for each tube will be documented in the inspection report data sheets.

Following review of the initial Inspection results and discussions with Framatome, the inspection plan was revised to direct obtaining samples from select areas. Samples were taken from the rust/corrosion stain and a boric acid residue trail on the side of the vessel and from 12 select tubes.

The tube sampling included removing all material around the tube penetration annulus down to bare metal. Following this sampling, the penetration annulus around 9 tubes was visually Inspected. No degradation of the surface metal was Identified around any of the tubes Inspected.
