Nine Mile Point Unit 1 Docket No. 50-220 Facility Operating License No. DPR-63 Submittal of Inservice Inspection Owners Activity Reports Gentlemen: Enclosures 1 through 7 contain inservice inspection owners activity reports for Nine Mile Point Unit 1 (NMP1). Some explanation relating to these reports is provided below. In accordance with the requirements of the American Society of Mechanical Engineers (ASME), Boiler and Pressure Vessel Code, Section XI (1989 Edition with no Addenda, 1992 Edition with 1992 Addenda, and 1998 Edition with no Addenda), Nine Mile Point Nuclear Station, LLC (NMPNS) conducted nondestructive examination of selected components on Nine Mile Point Unit 1 (NMP1) during the first period of the third 10-year inservice inspection (ISI) interval, which began on December 26, 1999 and was originally scheduled to end on December 25, 2002. Due to excessively high radiation levels experienced in the drywell during refueling outage sixteen (RFO16), the end date of the first inspection period was extended from December 25, 2002 to April 23, 2003, approximately four (4) months, in order for NMPNS to complete the first inspection period examinations. The extension of inspection periods is allowed by ASME Code, Section Xl, and will not alter the NMPI third ten-year ISI interval starting and ending dates of December 26, 1999, and December 25, 2009, respectively, for inservice examinations and containment inspections. The first inservice inspection period originally consisted only of RFO16, which occurred in Spring 2001. Due to the extension of the first inspection period, refueling outage seventeen (RF017), which occurred in Spring 2003, has also been included in the first inspection period. In the case of the first containment inspection period, which also originally consisted only of RFO16, refueling outage fifteen (RF015), which occurred in Spring 1999, has also been included in the first inspection period. bU-
Page 2 NMP1L 1748 By letter dated August 17, 2000, the NRC approved Relief Request RR-IWE/IWL-1 for NMP1 allowing containment inspections to be performed per the 1998 ASME Code Edition with no Addenda in lieu of the 1992 ASME Code Edition with 1992 Addenda. Therefore, while the 1992 Code Edition with 1992 Addenda was used for the RFO15 containment inspections, the 1998 Code Edition with no Addenda was used for the RFO16 and RFO17 containment inspections. By letter dated October 5, 2000, the NRC approved relief request RR-ISI-8 to utilize ASME Code Case N-532 as an alternative to the requirements of IWA-6210(c) and (d), IWA-6220, IWA-6230(b), (c), and (d), and IWA-6240(b) for preparation and submittal of inservice inspection summary reports. In accordance with requirements of ASME Code Case N-532, an Owners Activity Report Form (OAR-1) and applicable Tables were prepared and certified upon completion of RFO16. For those examination items that were re-scheduled to RFO17 as part of the first inspection period extension, additional OAR-1 forms were prepared and certified upon completion of the outage. Revision 13 to Regulatory Guide (RG) 1.147 is due to become effective on August 7, 2003. This revision approves Code Case N-532-1 with a condition that the Owners Activity Reports be submitted to the NRC within 90 days of the completion of the refueling outage. As a condition to the NRC approval of relief request RR-ISI-8, NMPNS should follow all provisions of Code Case N-532-1 with limitations (if any) specified by 10CFR50.55a. As a result of Revision 13 to RG 1.147, NMPNS is also including OAR-1 forms for RFO17, second inspection period reports (see Enclosures 6 and 7). With the exception of the examinations allowed to be deferred until the end of the inspection interval by ASME Code, Section XI, or those examinations deferred based on NRC approved relief requests, the percentage requirements for the NMP1 first inspection period have been satisfied, and this constitutes a close-out of the first inservice inspection period. As an alternate to the requirements of the American Society of Mechanical Engineers (ASME), Boiler and Pressure vessel Code (B&PVC), Section Xl, Article IWA-6000, NMPC submitted Request for Relief ISI-8 on October 30, 1999, under Letter NMP1 L 1480, to implement ASME Code Case N-532, Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000, Section Xl, Division'1 ". USNRC Safety Evaluation dated October 5, 2000, under TAC No. MA7129 authorized use of this alternative. An Owner's Activity Report Form OAR-1, was prepared and certified upon completion of the 2001 refueling outage (RFO-1 6) Examination activity. Note: Pursuant to 10 CFR 50.55a(a)(3)(i) ASME Code Case N-582 is authorized for the Third Ten-Year Inservice Inspection Interval, or until ASME Code Case N-532 is approved for general use by reference in 10 CFR 50.55a (previously Regulatory Guide 1.147). After that time, NMPC must follow the conditions, if any, specified in the regulation. In addition to the Owner's Activity Report, Form OAR-1, the following Tables are also included:
- a. Abstract of Examinations and Tests, Table 1, provides such information as ASME Code Examination Category, Total Examinations Required for the Inspection Interval, Total Examinations Required for this Period, Total Examinations Credited (%) for this Period, Total Examinations Credited (%) to Date for the Interval, and any remarks that may be applicable.
- b. Items-with Flaws or Relevant Conditions that Require Evaluation for Continued Service, Table 2, provides a listing of item(s) with flaws or relevant conditions that required evaluation to determine acceptability for continued service, whether or not the flaw or relevant condition was discovered during a scheduled examination or test, and provides such information as Examination Category, Examination Item Number, Item Description, Flaw Characterization (as required by IWA-3300), and whether the flaw or relevant condition was found during a scheduled Section Xl examination or test.
- c. Abstract of Repairs, Replacements or Corrective Measures Required for Continued Service, Table 3, identifies those items containing flaws or relevant conditions that exceeded the ASME Section Xl acceptance criteria (IWX-3000) or in the case of supports IWF-3000 acceptance criteria; even though the discovery of the flaw or relevant condition that necessitated the repair, replacement or corrective measure, may not have resulted from anexamination or test required by Section Xl, and provides such information as Code Class, whether the item is a repair, replacement or corrective action, item description, description of work, if the flaw or relevant condition was found during a scheduled Section XI Examination or test, date completed and the repair replacement plan number.

- 16) which started on March 18,2001 and ended on May 18,2001. The Nine Mile Point Unit 1 (NMP1) First Inservice Inspection Period which started on December 26, 1999 and was scheduled to end on December 25, 2002 is being extended to April 23, 2003, approximately three (3) months and twenty-three (23) days, in order to complete the examination category percentages required by IWB-2412-1.
In accordance with ASME Section Xl, IWB-2412 (a), with the-exception of the examinations that may be deferred until the end of the inspection interval as specified in Table IWB-2500-1, the required examinations in each examination category shall be completed during each successive inspection interval in accordance with Table IWB-2412-1, and per IWB-2412 (b) the inspection period specified in (a) above may be decreased or extended by as much as 1 year to enable an inspection to coincide with a plant outage, within the limitations of iWA-2430(d). The following examination categories are being extended primarily due to a crud burst at the start of the refueling outage that resulted in an excessively high radiation level in the drywell. The ASME Section Xl Examination Categories being re-scheduled to refueling outage seventeen (RFO-1 7), the next scheduled refueling outage are as follows:
- 1. Examination Category B-D;
- 2. Examination Category B-K;
- 3. Examination Category B-N-1;
- 4. Examination Categories B-F, B-J, C-F-1 and C-F-2 are not being rescheduled because the USNRC authorized NMPC Request for Relief IS-13, which replaces these examinations with a Risk-informed ISI Program.
- 5. Examination Category C-G states that deferral of examinations are authorized by NMPC Request for Relief ISI-7, therefore, Inspection Program EBB percentages are not required.
Note: The above rescheduled items will be separated from the examinations required for the Second Inservice Examination Period, starting on December 26, 2002 in refueling outage seventeen (RFO-
- 17) in order to avoid dual credit.
Although specifically not addressed in the ASME Code Case, NMPC approximate submittal date will be ninety (90) days following the end of each specific inspection period. This Owner's Activity Report will be submitted as defined in the Table below following completion of RFO-17. ~onstelllafioh ~ THIRD INSERVICE INSPECTION INTERVAL Rev. 00 Energy Group 2003 OWNERS' ACTIVITY REPORT Nine IMle Point FOR Date: April 23, 2003 Nuclear Station INSERVICE EXAMINATIONS Page i-4 of 1-10 ABSTRACT This Owner's Activity Report summarizes the results of the Nine Mile Point Nuclear Station (NMPNS), Unit 1, ASME Section Xl Inservice Examinations that were re-scheduled to refueling outage seventeen (RFO-17) as a result of a crud burst that resulted in excessively high radiation levels in the drywell. The First Inservice Inspection Period started on 26 December 1999 and was originally scheduled to end on 25 December 2002. Due to the crud burst, the First Inspection Period was extended to April 23, 2003.
As an alternate to the requirements of the American Society of Mechanical Engineers (ASME), Boiler and Pressure vessel Code (B&PVC), Section Xl, Article IWA-6000, NMPNS submitted Request for Relief ISI-8 on October 30, 1999, under Letter NMP1 L 1480, to implement ASME Code Case N-532, "Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000, Section Xl, Division 1". USNRC Safety Evaluation dated October 5, 2000, under TAC No. MA7129 authorized use of this alternative. An Owner's Activity Report Form OAR-1, was prepared and certified upon completion of the 2001 refueling outage (RFO-16) Examination activity. The OAR is being supplemented to indude the results of the re-scheduled inservice examinations in order to close-out the first inservice inspection period percentage requirements. Note: Pursuant to 10 CFR 50.55a(a)(3)(i) ASME Code Case N-532 is authorized for the Third Ten-Year Inservice Inspection Interval, or until ASME Code Case N-532 is approved for general use by reference in 10 CFR 50.55a (previously Regulatory Guide 1.147). After that time, NMPNS must follow the conditions, if any, specified in the regulation. In addition to the Owner's Activity Report, Form OAR-1, the following Tables are also included:
- a. Abstract of Examinations and Tests, Table 1, provides such information as ASME Code Examination Category, Total Examinations Required for the Inspection Interval, Total Examinations Required for this Period, Total Examinations Credited (%) for this Period, Total Examinations Credited (%) to Date for the Interval, and any remarks that may be applicable.
- b. Items with Flaws or Relevant Conditions that Require Evaluation for Continued Service, Table 2, provides a listing of item(s) with flaws or relevant conditions that required evaluation to determine acceptability for continued service, whether or not the flaw or relevant condition was discovered during a scheduled examination or test, and provides such information as Examination Category, Examination Item Number, Item Description, Flaw Characterization (as required by IWA-3300), and whether the flaw or relevant condition was found during a scheduled Section Xl examination or test.
- c. Abstract of Repairs, Replacements or Corrective Measures Required for Continued Service, Table 3, identifies those items containing flaws or relevant conditions that exceeded the ASME Section Xi acceptance criteria (IWX-3000) or in the case of supports IWF-3000 acceptance criteria; even though the discovery of the flaw or relevant condition that necessitated the repair, replacement or corrective measure, may not have resulted from an examination or test required by Section Xl, and provides such information as Code Class, whether the item is a repair, replacement or corrective action, item description, description of work, if the flaw or relevant condition was found during a File: OAR-1lSI16R1.WPD

Form OAR-1 and applicable Tables were prepared following the completion of the rescheduled examinations performed during Refueling Outage (RFO-17), which started on March 14, 2003 and ended on April 23,2003. With the completion of this OAR-1 and the associated Tables, the First Inservice Inspection Period for Nine Mile Point Nuclear Station, Unit 1 is closed-out. In accordance with ASME Section Xl, IWB-2412 (a), with the exception of the examinations that may be deferred until the end of the inspection interval as specified in Table IWB-2500-1, the required examinations in each examination category shall be completed during each successive inspection interval in accordance with Table IWB-2412-1, and per IWB-2412 (b) the inspection period specified in (a) above may be decreased or extended by as much as 1 year to enable an inspection to coincide with a plant outage, within the limitations of IWA-2430(d). USNRC Safety Evaluation dated October 5, 2000, under TAC No. MA7129 authorized use of this alternative. IWE Containment Inservice examinations were initially performed during refueling outage (RFO-1 5), which were conducted during the period of April 6,1999 through June 16, 1999. The IWE examinations consisted of examining the general condition of the Torus internal pressure retaining components at or below the waterline. The results of these inspections were documented in the RFO-15 Torus Inspection Plan, Document NER-1 S-025, Revision 1, and was provided in Appendix D of the 1999 Summary Report Submittal. In order to meet the intent of ASME Code Case N-532, an Owners' Activity Report Form OAR-1 was prepared to document the 1999 (RFO-15) Examination activity. Note: Pursuant to 10 CFR 50.55a(a)(3)(i) ASME Code Case N-532 is authorized for the Third Ten-Year Inservice Inspection Interval, or until ASME Code Case N-532 is approved for general use by reference in 10 CFR 50.55a (previously Regulatory Guide 1.147). After that time, NMPC must follow the conditions, if any, specified in the regulation. In addition to the Owner's Activity Report, Form OAR-1, the following Tables are also included:

- 4. Description of the type and estimated extent of degradation, and the conditions that led to the degradation; Photo documentation from previous Torus entries indicate that the waterline region may potentially exhibit generalized, non specific corrosion.
Supplemental examinations will be performed from the outside surface of the Torus by taking ultrasonic thickness measurements of the Torus shell plate at the waterline region. These examinations were to determine the general Torus shell thickness and if minimum wall thickness requirements have been violated by the corrosion.
- 5. An evaluation of each area, and the results of the evaluation; Engineering had determined that eight (8) locations/areas will be ultrasonically measured for thickness.
Of the three hundred and sixty (360) readings taken, the lowest recorded point (o.0438 inches) was in Bay 3-I at location B6. Since the allowable minimum wall was 0.397 inches at this location, the recorded readings were found to be acceptable.

As a alternate to the requirements of the American Society of Mechanical Engineers (ASME), Boiler and Pressure Vessel Code (B&PVC), Section Xl, Article IWA-6000, NMPC submitted Request for Relief ISI-8 on October 30, 1999, under Letter NMP1 L 1480, to implement ASME Code Case N-532, "Altemative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000, Section Xl, Division 1". For each inaccessible area identified, NMPC shall provide the following:

Description of the type and estimated extent of degradation, and the conditions that led to the degradation; During scheduled inservice examination of the containment moisture barrier between the dry well liner and the concrete floor at elevation 225-ft was identified as portions of the sealant was either missing and those portions of the existing sealant was degraded.
The moisture barrier was degraded over the entire 360 degree circumference, with approximately 60% of the moisture barrier being missing.
- 5. An evaluation of each area, and the results of the evaluation; Detailed Visual examination found no unacceptable degradation in the visible areas of the dry well liner adjacent to the moisture barrier.
Evaluation of the inaccessible areas of the Containment Liner condition below the concrete is not possible without breaking and removal of the concrete. Engineering evaluation of this condition is that a significant portion of the existing barrier is still in good condition and requires corrective actions to be fully effective to function as a moisture barrier.
- 6. Description of necessary corrective actions; Restore containment moisture barrier to original intended function by repairing degraded portions and replacing missing portions of moisture barrier.

USNRC Safety Evaluation dated October 5, 2000, under TAC No. MA7129 authorized use of this alternative. An Owners Activity Report Form OAR-1, was prepared and certified upon completion of the 2003 refueling outage (RFO-1 7) Examination activity. Note: Pursuant to 10 CFR 50.55a(a)(3)(i) ASME Code Case N-532 is authorized for the Third Ten-Year Inservice Inspection Interval, or until ASME Code Case N-532 is approved for general use by reference in 10 CFR 50.55a (previously Regulatory Guide 1.147). The second inservice inspection period consists of refueling outages seventeen (RFO-17) and eighteen (RFO-1 8). As a alternate to the requirements of the American Society of Mechanical Engineers (ASME), Boiler and Pressure Vessel Code (B&PVC), Section Xl, Article IWA-6000, NMPNS submitted Request for Relief ISI-8 on October 30, 1999. under Letter NMP1 L 1480, to implement ASME Code Case N-532, "Alternative Requirements to.-Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000, Section Xl, Division 1w.USNRC Safety Evaluation dated October 5, 2000, under TAC No. MA7129 authorized use of this alternative. This Owner's Activity Report Form OAR-1 and associated Tables were prepared and certified upon completion of the 2003 third refueling outage (RFO-1 7) Examination activity. Note: Pursuant to 10 CFR 50.55a(a)(3)(i) ASME Code Case N-532 is authorized for use during the Third Ten-Year Inservice Inspection Interval, or until ASME Code Case N-532 is approved for general use by reference in 10 CFR 50.55a (previously Regulatory Guide 1.147). After that time, NMPNS must follow the conditions, if any, specified in the regulation. In addition to the Owner's Activity Report, Form OAR-1, the following Tables are also included:

Applicable ASME Section Xl Code 1998 Edition, and NO Addenda For Class MC applications, NMPNS shall evaluate the acceptability of inaccessible areas when conditions exist in accessible areas that could indicate the presence of or result in degradation to such inaccessible areas. For each inaccessible area identified, NMPNS shall provide the following:

Description of the type and estimated extent of degradation, and the conditions that led to the degradation; During scheduled General (G-VT) visual inservice examination (VT-3) of the drywell interior surface of the liner plate, at the 225 foot elevation, major corrosion (rust) was identified near and underneath the area coolers.
The corrosion was observed over the entire 360 degree circumference of the liner plate, with varying degrees of oxidation (light, medium and course rust).
- 5. An evaluation of each area, and the results of the evaluation; Detailed (D-VT) Visual examination (VT-1), was performed and found no unacceptable degradation in the visible areas of the dry well liner.
Evaluation of the inaccessible areas (backside) of the drywell interior surface of the liner plate was performed using ultrasonic thickness measurements to determine any areas of degradation. All thickness measurements were determined to be in excess of material design thickness.
- 6. Description of necessary corrective actions; No immediate corrective action was required, other than the steps taken above.
