Safety monitor for both units have transitioned to a Yellow Risk Status based on the Probabilistic safety assessment group identifying a new failure mode and therefore creating a factor which allows for operator action to prevent pump failure in the event of not feeding forward. The factor takes credit for the postings near the flow indicators as an operator aid to ensure that the pumps are secured if feed is below pump specific values.

The PSA group will be reviewing other actions that are taken by the PBNP organization to determine their impact on the failure mode and any changes necessary to the safety monitor factor. [Holzmann, Mike - STA
