Introduction:
The enclosed Steam Generator Tube Inspection Report for Turkey Point Unit 4 is submitted for the inspection of the SGs during refueling outage 33 (hereafter referred to as the TP4-33 inspection or outage), as required by Technical Specification section 6.9.1.8. Per the Turkey Point Unit 4 Technical Specification section 6.8.4.j.d.2.c, the 5th ISi period is 72 EFPM. The inspection in TP4-33 was performed in accordance with Technical Specification 6.8.4.j and was the 1st inspection of the SGs in the 5th ISi period. The prior outage (TP4-32) was a SG inspection "skip". At unit shutdown for the TP4-33 inspection, the SGs had operated for approximately 31.27 EFPY since installation, including 7.35 EFPM in the 5th ISi period. Since the previous SG inspection (TP4-31), the SGs operated for approximately 1.43 EFPY and 1.30 EFPY during fuel cycles 31 and 32, respectively, leading up to the TP4-33 inspection. Initial entry into Mode 4 following completion of the TP4-33 inspection was made on April 7, 2022. References to past SG Tube Inspection Reports for Turkey Point Unit 4 are provided in Appendix A along with a list of acronyms used in this report.
Turkey Point Unit 4 is a Westinghouse 3-loop PWR with Model 44F steam generators. The SGs are U-tube heat exchangers with tube bundles fabricated using thermally treated Alloy 600 tubing. Each SG contains 3,214 tubes arranged in 45 rows and 92 columns, in a square-pitch configuration. Nominal tube OD is 0.875" with a 0.050" nominal wall thickness. Each SG tube bundle is supported by one drilled-hole flow distribution baffle (FOB) and 6 quatrefoil, broached-hole tube support plates (TSPs) all fabricated from stainless steel. Two (2) sets of anti-vibration bars (AVBs) in the U-bends also provide tube bundle support (AV1/4, AV2/3). Rows 1-8 of tubing in each SG were stress relieved in the U-bend region after bending.
A. Scope of Inspections Performed on each SG The TP4-33 inspection scope was selected to meet the requirements of plant Technical Specifications, NEI 97-06 Rev 3 and its referenced EPRI SGMP Guidelines. In addition, since Turkey Point plant has submitted a License Amendment Request 1 to adopt "Revised Frequencies for Steam Generator Tube Inspections" (TSTF-577 Rev 1), the SG inspection in TP4-33 is considered as the implementation outage to align the scope of the SG Program with TSTF-577 Rev 1 requirements. Unless otherwise noted, the TP4-33 base inspection scope in each SG was:
Primary-side:
- 100% full-length enhanced probe 2 exams of all active tubes.
- U-bends of rows 1 & 2 were inspected with the +Point' probe between the uppermost TSP on the HL and CL.
- All other active tubes were inspected full-length with the X-probe (array/bobbin coils).
- Examination of peripheral tubes at the TTS, BLG/OXP indications within the H* depth of the tubesheet, and all tubes within the tubesheet program are included in utilizing full-length enhanced probe exams.

- All "high residual stress" tubes were inspected with the +Point' probe:
- in the HL tubesheet section (TSH+3" to TEH), and
- at all HL tube support structures, and at the uppermost CL TSP.
- 100% inspection of dings/dents >5V in the HL, U-bend and uppermost CL TSP using the
+Point' probe.
- All dings/dents -5,.5v were screened with the bobbin coil.
- Various diagnostic/special interest exams using the +Point' probe including all I-codes from the X-probe exams (bobbin/array coils), tubes in the vicinity of newly identified or prior PLPs and tubes adjacent to the locations of known foreign objects.
- Visual inspection of all mechanical and welded plugs.
- Visual inspection of all channel head bowl components and bowl scan everywhere within the channel head bowl with remote visual camera in both the HL and CL per Westinghouse NSAL 12-1 Rev 1, "Steam Generator Channel Head Degradation," October 2017.
Secondary-side:
- Visual/cleanliness inspection at the TTS (annulus, no-tube lane) after upper bundle flush and sludge lancing at the TTS.
- Foreign object search and recovery (FOSAR) at the TTS including in-bundle exams.
- Upper-bundle visual inspection from the uppermost TSPs.
- Visual inspection of components of the Upper Internals including the feedwater ring & supports, J-tubes, and moisture separator components. UT thickness measurements were also obtained at select locations on the feedwater ring.
B. Degradation Mechanisms Found The following degradation mechanisms were identified during the TP4-33 inspection:
- A single axially-oriented, crack-like ODSCC indication (SAi) was identified in the U-bend of the tube located at R9C80 in SG-C. This was the first occurrence of a crack-like indication in the Turkey Point Unit 4 SGs. No other indications were reported for the TP4-33 inspection that were indicative of corrosion degradation within the defined pressure boundary.
- Wear at AVB contact points.
- Wear at horizontal tube support structures (FDBs, TSPs).
- Wear due to foreign object(s) at the TTS.
Inspection Expansion:
Since the base scope exams included a 100% full-length inspection of all tubes in each SG with an enhanced probe, expansion of the inspection program due to the ODSCC indication found was not required. Additional details on the crack-like ODSCC indication are provided later in the report.

Location, orientation (if linear), and measured sizes (if available) of service induced indications Axial ODSCC: A single axial crack-like indication (SAi) was reported on a tube (R9C80) in the U-bend section of SG-C. The indication was located close to the apex of the tube, on the flank, and approx.
3.5 inches from the nearest anti-vibration bar support (AV1 +3.49). The indication was identified during full-length ECT exams using the X-probe and was detected by both the bobbin and array coils (reported as NQI with bobbin, SAi with array). The flaw was confirmed with the +Point' (and mag-biased +Point') probes to be OD-initiated and was characterized as ODSCC. The ECT response did not reveal the presence of a ding or geometric anomaly in proximity to the indication, and the tube does not belong to the population of tubes with high residual stresses. The indication had a voltage amplitude of 0.43 volts using the +Point' probe. Flaw profile analysis (line-by-line sizing) determined an axial length of 0.16 inches and a maximum depth of 62% TW using the voltage amplitude sizing of ETSS 1-28432.1. The flaw profile is shown in the Chart below. (It should be noted that the maximum depth was calculated at 71 % TW when a phase angle sizing approach was used. This more conservative value is also benchmarked for condition monitoring purposes (see section G of the report.).

Wear: Attachment B provides the listing of other service-induced indications (mechanical wear) reported during the TP4-33 inspection, including locations and measured sizes. For AVB wear, the indication depths as sized by the bobbin probe ETSS are provided. For all other locations, the indication depths as sized by the +Point' probe ETSS are provided.

Number of tubes plugged during the inspection outage for each degradation mechanism Seven (7) tubes were plugged based on the TP4-33 inspection; the tubes plugged for each degradation mechanism are summarized below in Table 2:

F. Number and percentage of tubes plugged to date, and the effective plugging percentage in each steam generator The number and percentage of SG tubes plugged to-date, and the effective plugging percentage in each SG are summarized in Table 3.

G. Results of Condition Monitoring, including the results of tube pulls and in-situ testing All indications found in TP4-33 met the condition monitoring (CM) requirements of NEI 97-06 Rev 3 and Turkey Point Technical Specifications for structural and leakage integrity. No indication exceeded the structural limits. The tubes identified for plugging were screened against the in-situ test selection criteria contained in the EPRI SGMP Steam Generator In-Situ Pressure Test Guidelines, Rev 5, and provided for Turkey Point Unit 4 conditions. None of the tubes identified for plugging met the requirements for in-situ pressure testing, and no tube pulls were performed.
Appendix B has a listing of all mechanical wear indications.
- 1. The axial ODSCC indication on the tube in the U-bend ( described in Section D of this report) was assessed for CM and for in-situ testing. Comparison with the CM screening chart indicates the ODSCC indication met both burst and leakage integrity using both sizing methods discussed in section D of the report. For both methods, the 95/50 burst pressure (as
Turkey Point Unit 4 L-2022-149 Docket No. 50-251 Page6of10 Enclosure found) was greater than 6,000 psi, which exceeded the 4,542 psi (3xNOPD) margin requirement. The ODSCC indication fell below the screening limits for burst and leakage; therefore, structural integrity requirements were met.
A review of potential causal factors was performed for the unprecedented crack-like indication in a SG on this unit. A lookback review of prior ECT inspection results showed no precursor signals in the bobbin data. (This section of tubing had not previously been examined with an enhanced probe.). It should be noted that the U-bends in row 9 are likely to have higher residual stresses (due to U-bend forming) than the U-bends of rows 10 and higher. This may be a contributing factor. The tube was also confirmed to be supported in the U-bend by AV1/AV4 based on the plant-specific AVB positioning study previously completed. Review of raw ECT data also revealed there was no AVB interaction with the tube at the location of the indication. The operating experience and results of this indication was reported to the industry SG Management Program (SGMP) for further review.
- 2. The maximum observed AVB wear indication (31%TW) in TP4-33 was less than the CM limit lines for both burst and leakage requirements. The maximum wear indication depths at TSPs and at TSP edges were 17%TW and 20% TW, respectively. These indications fell well below their respective CM limit curves. The maximum wear indication at FOB locations was 12%TW; all indications fell below the CM limit curve. Leakage potential for these shallow TSP and FOB wear indications is negligible. Therefore, TSP and FOB wear indications met the structural performance criteria for burst and leakage.
- 3. Three (3) tubes had newly reported FO wear during the full-length array probe exam in the CL at the TTS region of SG-A. The indications were sized using the +Point probe; the maximum depth was 39% TW (2 tubes). The tubes were located several columns in from the periphery and are relatively close to each other. It is not known whether the indications were attributed to the same foreign object or multiple objects. No FOs were identified during visual exams on the secondary side in that region. A lookback review of the historical bobbin data for these indications shows there was no discernable ECT signals at the prior inspection (TP4-31). It is likely these indications were not reported at a prior inspection since the locations were not previously examined with an enhanced probe. To add margin to the OA, these tubes were preventively plugged and stabilized. Historical FO wear indications that have been detected at TSPs were reported again in TP4-33 but showed no significant change. No PLP signals were detected at TTS and TSP locations having FO wear. All FO wear indications at TSPs and at the TTS fell below the CM limit curve.
- 4. No degradation was detected during the +Point inspection of "high residual stress" tubes, low row U-bends and the DNG/DNT inspection scope. During the array probe inspection of tubes within the tubesheet region, no indication was detected at locations where BLGs/OXPs were located.
- 5. Channelhead Components Visual Inspection:
- Tube Plug Inspection: During the inspection of tube plugs in TP4-33, all installed plugs were confirmed to be in their correct location. In addition, all plugs were found to be dry; no dripping plugs were identified. No degradation or visible signs of leakage were noted on the plugs during the visual inspection.
- Other Channelhead Inspections: Visual inspection of various channel head components were performed to identify degradation per guidance in Westinghouse NSAL 12-1 Rev 1, "Steam Generator Channel Head Degradation". Areas inspected include the divider plate-to-channelhead weld, the weld at the top of the channelhead bowl drain tube, the channelhead-to-tubesheet girth weld seam region, the divider plate, and all cladded
Turkey Point Unit 4 L-2022-149 Docket No. 50-251 Page 7 of 10 Enclosure surfaces of the channel head bowl and tubesheet. No degradation was found; the observed condition during the TP4-33 inspection is consistent with the manufacturing process.
- 6. Secondary-side Inspections and Maintenance:
- A total of 211 lbs. of sludge was removed from the three SGs based on upper bundle flush and sludge lancing in TP4-33 (76, 65 and 70 lbs. in SGs A, B and C, respectively). The TTS locations of all legacy FOs were visually inspected. One newly identified FO was found; it was removed during TTS FOSAR. The object was in the HL of SG-B and appears to be a piece of machine curl. An evaluation is in place for all irretrievable (legacy) FOs that have the potential to cause tube damage. Upper tube-bundle visual exams in all 3 SGs in the upper bundle reported the broached openings on the TSPs to be open and free of deposits.
- Upper Internals Inspection: Visual inspections performed in all 3 SGs (for erosion/corrosion, mechanical damage, foreign material, and unusual conditions) included the primary/secondary moisture separators, drains, swirl vanes, feedring and J-nozzles. No fouling was observed in the flow holes of the moisture separators, and no erosion or corrosion was apparent on the separators or swirl vanes. A minimal amount of scouring was observed inside some of the J-tubes at the interface with the feedring; these have been noted previously and do not appear to have changed. The welds of all steam drum components were noted to be intact. Repairs made in TP4-31 to address wall-thinning on the outside of the SG-B feedring 3 were noted to be intact, and no abnormalities were reported based on UT thickness measurements taken on all 3 feedrings in TP4-33.
H. Primary-to-secondary leakage rate observed in each SG during the previous cycle No primary-to-secondary leakage was observed during the previous 2 cycles (Cycles 31, 32).
I. Calculated accident induced leakage rate As described in the H* alternate repair criteria (ARC) for the Turkey Point Plant4, the accident induced leakage rate from the portion of the tubes below 18.11 inches from the TTS is calculated from any observed normal operating leakage that cannot be attributed to a source other than the tubesheet expansion region. For Turkey Point Unit 4, the maximum operational primary-to-secondary leakage rate from the portion of the tubes below 18.11 inches from the top of the tubesheet is determined by multiplying any normal operating leakage by a factor of 1.82 to determine the accident induced leakage rate. Since no operational primary-to-secondary leakage has been observed (per section H of this report), the calculated accident induced leakage rate is zero.
J. Results of monitoring for tube axial displacement (slippage)
A condition for licensing H* was to monitor for tube slippage within the tubesheet region. Monitoring for tube slippage was completed during the TP4-33 inspections. No tube slippage was reported.
