AUG29 O ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 The following violations were identified during. an inspection conducted on July 27 - August 16, 1985.

The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

1.

Technical Specification 3.0.4 prohibits entry into an operational mode unless the conditions for the Limiting Condition for Operation (LCO) are met.

Technical -Specification 3.8.1.2.b. an LCO, requires that at least one of the two emergency diesel-generators (EDGs) be operable in operational modes 5 and 6.

This specification further requires in the Action statement, that if the EDG is not operable, core alterations must be suspended.

Technical Specification 4.8.1.2 requires, in part, that the automatic load sequence timers be operable.

This Technical Specification provides the basis for operability of the EDG.

Contrary to the above, during the period of June 27, 1985, through July 25, 1985:

The automatic load sequence timers were not tested for operability

-

thereby making the B-EDG inoperable;-

The A-EDG was inoperable for maintenance;

-

A core alteration was conducted on July 10; and,

-

An operational mode change from Mode 6 to Mode 5 was made on

-

July 18.

(Testing of the automatic load sequence timers was completed on

July 25, at which time they were found to be operable.)

This is a Severity Level IV violation-(Supplement I).

2..

Technical Specification 6.8.1.a requires that the procedures implemented as a result of the listing in Appendix A of Regulatory Guide 1.33, November 1972, be adequate.

The Regulatory Guide 1.33-listing includes procedures that direct equipment control activities.

Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 the discretion of the operator and permit verification by hands on operation of the valve, visual observance of valve stem position, or the fact that the system is operating properly.

Contrary to the above, the OSIM Section VI, paragraph B.1.1 was found to be inadequate when a valve position checklist for operating procedure OP-605 was completed and verified by signature that valve EFV-7 was open when in actuality the valve was shut. This verification was made by only observing the valve stem position of EFV-7.

This is a Severity Level IV Violation (Supplement I).

Technical Specification 3.3.3.1 requires, in part, that the condenser vacuum pump exhaust monitor (RM-A12) be operable while in Modes 1 through 4.

If this monitor is not operable, the Technical Specification requires that grab samples be collected and analyzed for gross activity at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Contrary to the above, on August 7, 1985, monitor RM-A12 was made inoperable for maintenance and grab samples were not collected and analyzed until approximately 10:30 a.m. on August 9,1985, a period of approximately 40 hours4.62963e-4 days <br />0.0111 hours <br />6.613757e-5 weeks <br />1.522e-5 months <br />.

This is Severity Level IV Violation (Supplement I).

Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including:

(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d)

or 10 CFR 73.21.
