Tonlon, Section Chief, RCES Branch Date Signed SUM 1ARY Inspected on November 5-8, 1979 Areas Inspected This routine, unannounced inspection involved 30 inspector-hours onsite in the areas of structural concrete quality records, Level B and C storage, soils and concrete laboratory, and construction status.

Results of the four areas inspected, no apparent items of noncompliance or deviations were identified in three areas; one apparent item of noncompliance was found in one area (Improper curing and testing of concrete cylinder - Details para-graph 5).

Exit Interview The inspection scope and findings were summarized on November 8, 1979 with those persons indicated in Paragraph 1 above. The licensee acknowledged the noncompliance discussed in Paragraph 5.

In addition, the requirements of reporting of construction deficiencies was discussed briefly with the licensee.

The inspector noted that the licensee is presently meeting these requirements.

Curing and unconfined compression testing of concrete cylinders.

Examination of the curing and testing of concrete cylinders in the soils and concrete laboratory disclosed the following noncompliance:

Procedure QCI-C-208 specifies that concrete cylinders be stored in accor-dance with the requirements of ASTM C-31.

ASTM C-31 requires cylinders to be moist cured until time of testing. ASTM C-31 defines moist curing as a condition in which the concrete cylinders have free water maintanined on their entire surface at all times. Procedure QCl-C-209 specifies that concrete cylinders be tested in accordance with requirements of ASTM C-39.

ASTM C-39 requires that the rate of loading during testing of the concrete cylinder be applied within the range of 20 to 50 psi per second. On November 7, 1979, during inspection of the curing room, the inspector observed that the surfaces of cylinders number 1308F, 1371E, 1371F, P ^I, 1378F, and 1434F were dry. On the same date, during observation of unconfined compression testing of concrete cylinders, the inspector noted that the rate of loading was applied in the range of 60 to 70 psi per second during testing of cylinder numbers 1174E, 1174F, 1175E, 1175F, 1176E, 1176F, and 1170E. The above examples of failure to follow procedure were identified to the licensee as deficiency items 553/79-16-01 and 554/79-15-01. The inspector reviewed the concrete cylinder log and noted that concrete cylinder strength test results were approximately 50 percent higher than the strengths required by design.

Containment (Structural Concrete I) - Review of Quality Records - Units 1 and 2 The inspector examined quality records for pour numbers R1-AM01-1A, R1-AM01-1B, R1-AM01-IC, ID, IE, R2-AM01-1A, and R2-AM01-1B.

Pour number R1-AM01-1A was placed on December 19-20; pour number R1-AM01-1B on February 6-7, 1979; pour number R1-AM01-16, ID, 1E on September 24-26,

1979; pour number R2-AM01-1A on June 13-14, 1979; and pour number R2-AM01-1B on August 24, 1979. Records examined were as follows:
