On July 20, 1978, MP6L informed IE:II that Bechtel had informed i

MP&L that eight non-safety related field welds within the reactor water cleanup system (filter demineralizer system) had been made without the required fit-up visual inspection being performed.

This matter is documented in Bechtel Nonconformance Report No. 2776, dated July 19, 1978.

Rpt Nos. 50-416/78-13 and 50-417/78-13 I-3 During welder interviews, four ASME III, Class 3 welds, located in the component cooling water system (two welds) and the suppression pool cleanup system (two welds) were identified where the cleanliness and fitup inspection points were bypassed. These welds are documented on Nonconformance Report Nos. 2798 and 2799.

During this inspection the inspectors reviewed the applicable.

nonconformance reports, minutes of the investigative committee meetings, held discussions with responsible FT&L and Bechtel personnel and interviewed twenty Bechtel welding personnel which included general foremen, foremen, welders and pipefitters. The

'l questionable activities involved those activities of the Bechtel

QC inspector and not the activities of the contractor, Peabody l

Testing Company, that performs nondestructive examination (radiography, liquid penetrant and nagnetic particle).

It appears that this natter is receiving adequate investigative attention to determine extent of problem and corrective actions.

This matter is identified as an inspector followup item I

Reactor Vessel (Unit 2)

Receipt of the reactor vessel and head is anticipated during the week of July 31, 1978.

The contract for off-loading, transporting and erection of the vessel was awarded to Reliance Truck Company by contract number 9645-FSC-18 on July 25, 1975.

Tb contract by reference incorporates Revision 5 of Bechtel Technical Specification 9645-C-1940 which includes as Appendix C, " General F :quirements for Subcontractor Quality Assurance Programs." Supplen.nt E of the vessel contract lists the drawings applicable to tae scope of work authorized.

A subsequent contract amenCment dated January 17, 1978, was used 'o invoke the requirements of 10 CFR 21.

The inspectors held discussions with responsible FT&L and Bechtel personnel and reviewed the contract controlling specification,

'eliance QA Manual and its site oriented supplement, procedures, nd drawings. Additionally, the inspectors performed an inspection of the heavy-haul roadway, dock area, and storage location.

Proofloading of the heavy-haul roadway las been completed.

The QA Manual addresses the requirements of 10 CFR 59, Appendix B and defines the company organizational structure under which quality programs are planned and implemented.

The supplement addresses i

organization and authority, control of purchased items, special

processes, inspections, control of nonconforming items, corrective actions, records, and audits.
