ATTACHMENTTOPLA-414SFILER41-2PAGE1OF6REPIYTOANOTICEOFVIOI.ATIONA.Violation(EA94-022/01013)(387/93-80-01)TechnicalSpecification6.8.1.arequiresthatwrittenproceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesreferencedinRegulatoryGuide1.33.RegulatoryGuide1.33requiresthatrefuelingprocedures,andproceduresformaintenanceandsurveillances,beimplemented.Inaddition,TechnicalSpecification6.8.1.brequiresthatwrittenproceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesreferencedinrefuelingoperations.ViolationA.1Precaution6.2.1ofSusquehannaNuclearDepartmentProcedureOP-AD-107,Rev.1,"Removalof1orMoreControlRodDriveMechanismforMaintenanceDuringOperationalCondition5",providesthatwithanycontrolbladeinthecorefullywithdrawnformaintenance,fuelmovementisrestrictedtoofHoadonly,andonlyfuelmovementsfromthecoretofuelpoolarepermitted.Contrarytotheabove,procedureswerenotadheredtoduringrefuelingoperations,asevidencedbythefollowing:OnOctober6,1993,fuelbundle31-56wasincorrectlyremovedfromthecoreandsubsequentlyreplacedinitscorelocation.Thus,thismovementwascontrarytoOP-AD-107,Rev.1,becauseitwasnotamovementfromthecoretothefuelpool.~Resense1.ReasonfortheViolationFuelbundle31-56wasincorrectlyremovedfromthecoreandsubsequentlyreplacedinitscorelocationonOctober6,1993,forthefollowingreasons:1.Thereactoroperatorwhoperformedthefuelbundlemanipulationreliedsolelyonthebridgeencoderandtheseniorreactoroperatordidnotproperlyperformbundleverificationpriortoitsremovalfromthecore.Sincefuelbundle31-56isaperipheralbundle,visualobservationofitcanbeobscured,and ATTACHMENTTOPLA-4148FILER41-2PAGE2OF62.Proceduralcontrolsandcommunicationswerelessthanoptimum.Thisresultedintheoperatorsreturningbundle31-56toitscorelocationinsteadofthefuelpool.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieveda.Immediatetrainingwasgiventorefuelingbridgeoperatorsidentifyingtheimportanceofassuringthatprocedurallyidentifiedfuelmovementsareperformed.b.Theoperatingprocedurewasrevisedtorequirethattwoindividualsconfirmthegrapplealignmentpriortobundlemovement,andtorequirethattherefuelingbridgeseniorreactoroperatorandreactoroperatorconcuronthelocationandorientationofthegrapplepriortomovement.c.Anoff-normalprocedurehasbeenissuedtorequirethatifanyfuelbundlehasbeenliftedclearofthevesseltopguide,itmustbemovedtoasafelocationinthespentfuelpool.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolationsNofurtheractionsarerequired.4.DateofFullComlianceBasedon(2)above,PP&Lisinfullcompliance.ViolntionA.2Section6.13ofSusquehann'aNuclearDepartmentProcedureNDAP-QA-0754,Rev.0,"NonconformingReports-ControlandProcessing,"providesapprovedmeanstopreventinadvertentuseofnonconformingitems.Contrarytotheabove,procedureswerenotadheredtoduringrefuelingoperations,asevidencedbythefollowing:OnOctober5,1993,theUnit1grapplewasreplacedwithasparegrapplethatwasprocuredas"non-Q"andhadnotbeendedicatedpriortouse.NonconformanceReportNCR93-112hadbeenpreviouslywrittentodocumentthiscondition.HoweverthisgrapplewasplacedintousewithoutdispositioningofNCR93-112whichwascontrarytoSection6.13ofNDAP-QA-075 ~ResenseATTACHMENTTOPLA-4I4SFILER41-2PAGE3OF61.ReasonfortheViolationThereasonthe"non-Q"grapplewasusedontherefuelingflooronOctober5,1993,priortotheNCRbeingdispositioned,wasduetothelossofsegregationcontrol.Topreventthepossibilityofforeignmaterialenteringthereactorcavityorotheropenfuelpoolsontherefuelingfloor,loosematerialtakentotherefuelflooriskepttoaminimum.ThispolicyresultedintheNCRtagbeingremovedfromthe"non-Q"grapple.Whenthetagwasremoved,alossofsegregationcontroloccurred,andasaconsequence,thegrapplewasinstalledontherefuelingplatform.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieveda.AnEngineeringevaluationwasperformedtodetermineacceptabilityofthegrapple.Theevaluationconcludedthatthegrapplewasacceptableforuse,asreceivedandasinstalled.b.TheNCRadministrativeprocedure,NDAP-QA-0754,wasenhancedtorequirealternatemethodsofsegregationcontrol(suchasroped-ofFareawithplacard)whentaggingisnotappropriate.c.Areviewofhistoricalcontrolof"Q"componentsatSusquehannawasconducted.ThisreviewconcludedthatSusquehannamaintainsgoodcontrolof"Q"componentsandthisincidentwasnotindicativeofaprogrammaticprobleminthisarea.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolationsPP&Lbelievescompletionofcorrectivestep(2.b)abovewillpreventrecurrenceofthistypeevent.4.DateofFullComlianceBasedon(2)above,PP&Lisinfullcomplianc "ATTACHMENTTOPLA-4148FILER41-2PAGE4OF6B.Violation(EA94-022/01023)(387/93-80-02)10CFRPart50,AppendixB,CriterionXVI,"CorrectiveActions,"requires,inpart,thatincasesofsignificantconditionsadversetoquality,measuresshallassurethatthecauseoftheconditionisdeterminedandcorrectiveactiontakentoprecluderepetition.ViolationB.IContrarytotheabove,onSeptember20,1986;September27,1987;April15,1988;April13,1989;September19,1989;April17,1991;andMarch20,1992,fuelbundlesorbladeguideswereeitherimproperlyremovedorfoundtobeinanincorrectlocation,whichweresignificantconditionsadversetoquality.However,thecorrectiveactionsfortheseevents,primarilytrainingandcounseling,didnotprecluderepetition,inthatonOctober6,1993,fuelbundle31-56wasremovedincorrectlyfromthecore.~Resense1.ReasonfortheViolationThecauseofeachpreviouslyidentifiedeventhaduniquefeatures.Inadditiontotrainingandcounseling,correctiveactionssuchasproceduralrevisions,addressingofhumanfactorsconcerns,andimplementationofminormodificationsweretakentoaddresstheseevents.Althoughthecorrectiveactionstoeachspecificeventwereadequate,wedidnotfullyrecognizethesignificanceandcommonalityofthesetypesofrecurringevents.Therefore,thepreviouscorrectiveactionsdidnotpreventtheoccurrenceofthefuelbundle31-56movementeventonOctober6,1993,sincetheyonlyaddressedthespecificcauseofeachevent.SeetheresponsetoviolationA.lforthereasonforthefuelbundle(31-56)movementeventonOctober6,1993.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchievedSeetheresponsetoviolationA.lforthecorrectivestepswhichhavebeentakenforthefuelbundle(31-56)movementeventonOctober6,1993.Also,revisionstotheSignificantOperatingOccurrenceReport(SOOR)programhavebeenmadethatstrengthenstheabilitytorecognizeandappropriatelyaddressrecurringevents.Additionally,NuclearDepartmentpersonnelhavebeensensitizedtothesignificanceofanyeventthatoccursatthestatio ATTACHMENTTOPLA4148FILER41-2PAGE5OF63.CorrectiveStesWhichWi)lBeTakenToAvoidFurtherViolationsPP&LhasconvenedaContinuousPerformanceImprovementTeam,.headedbyafunctionalunitmanager,toevaluateandrecommendupgradestothevariouscorrectiveactionprogramsthatexistintheNuclearDepartment.Thisteamisscheduledtocompleteit'sevaluationandrecommendationsbyDecember31,1994.4.DateofFullComlianceBasedontheactionstakeninviolationresponseA.1section(2),PP8cLisinfullcompliance.Viola(ionE2Contrarytotheabove,betweenMarch1984andOctober1992,onatleastthirteenoccasions,therefuelingmastwasbentordamagedbyimpactoroperation(asignificantconditionadversetoquality),duetooperatorperformance.Correctiveactionstakenbythelicensee,primarilytrainingandcounseling,wereinsufficienttopreventrecurrenceonOctober26,1993,whenthemastagainwasdamagedonimpactwiththereactorvesselflangeduetooperatorperformance.~Resonse1.ReasonfortheViolationPPE.L'sevaluationoftherefuelingmastimpactdamagefortheOctober26,1993event,thatwasconductedfollowingrecognitionofthesignificanceofthistypeevent,concludedthat:1.Therootcauseinvestigationsforpreviousoccurrencesattributedtohumanerrordidnotprobeintothecausesforhumanerror.2.Themastoperatingpractices,whencoupledwiththemastdesignlimitationsweremarginallyadequate.3.Failuretoproceduralizecontrolsthatrecognizedmastdesignlimitationsaffectedmastoperations.2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieveda.Operatingprocedureshavebeenrevisedandtrainingcompletedtoprohibitsimultaneoushorizontalandverticalmovement.Therevisedproceduresonlyallowfullspeedmovementinthehorizontaldirectionwiththemastinthe"fullup"position,andrequirethemasttobeinthe"normalup"positionformovementthroughthetransfercana ATTACHMENTTOPLAI148FILER41-2PAGE6OF6b.Operatingprocedureshavebeenrevisedtodictatewhenmaintenancetestingontherefuelplatformmastisrequired(i.e.,priortocoreoff-load,priortocorereload,onceevery24hoursduringofF-loadandreloadoperations,andfollowinganyeventpotentiallycausingmastdamage).Maintenanceprocedureshavebeenrevisedtospecifywhattestingisrequiredineachinstance.c.Proceduresforperformingrootcauseevaluationswereenhancedtoprobefurtherintowhyhumanerrorsoccurred(e.g.,verbalandwrittencommunications,environmentalconditions,workschedule,managementchanges).~Id.Theprocedureforperformingrootcauseanalysisforstationeventswasenhancedtorequireareviewoftheeffectivenessofcorrectiveactionstopreventsimilarevents,andwhytheydidnotpreventrecurrence.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolationsAmajorprojecthasbeencreatedtoevaluateconcernsassociatedwiththerefuelingplatform,fuelhandlingequipmentandcomponenthandlingequipment.Theobjectivesoftheprojectistoreducethesourceorimpactsofhumanerrorbyenhancingtheman-machineinterface,minimizingunplannedfuelorcomponenthandlingequipmentdowntime,improvingefficiency,andtoreducethelikelihoodofevents.Shorttermactionsidentifiedbytheteamhavebeencompleted.LongtermactionsidentifiedbytherefuelplatformprojectwillbecompletedpriortotheUnit1ninthrefuelingandinspectionoutagecurrentlyscheduledtobegininSeptember1996.4.DateofFullComlianceBasedon(2)above,PPALisinfullcompliance.
