Areas Inspected: A routine, unannounced inspection was conducted of licensee action on previous inspection findings, licensee response to IE Bulletin 79-03A, and Preservice Inspection (PSI) activities including review of program, NDE implementing procedures, PSI data and observations of work in progress.

Licensee Response to Previous Inspection Findings I
(Closed) Unresolved item (86-17-01):
Evaluation of UT indications.
l Originally, 32 shop welds from Southwest Fabrication contained ultrasonic
'
indications requiring evaluation and disposition by the licensee.
Nineteen of the welds were evaluated and dispositioned by the licensee.
- At the time of inspection 86-17 the licensee committed to evaluate each of the remaining 13 welds against the criteria stated in paragraphs IWB 3514.2 and IWB 3414.6 of the ASME Code Section X1, 1980 ndition including winter 1981 addenda.
The licensee has completed the evaluations and one of the welds, DLA-208-1-6x-sw2, will be repaired.
The remaining 12 welds were found to be acceptable per the criteria of IWB3514.2 and IWB 3514.6.
The inspector reviewed documentation regarding evaluation and disposition of the 12 welds, and based on the review this item is close ___

Licensee Response to IE Bulletins (Closed) IE Bulletin 79-03A - Longitudinal weld defects in ASME SA-312 type 304 stainless steel pipe.
The inspector reviewed the licensee's documentation in response to the Bulletin requirements and found that Bechtel had performed calculations per item 1 of the bulletin which concluded that the subject pipe in safety related systems is subject to stresses less than 85% of the code allowable stresses. A search of internal NRC documentation indicates that item 2 and 3 of the bullecin do not apply if the criteria of item 1 are met.
Based on the above this bulletin is closed.

Preservice Inspection (PSI) Program Review The inspector reviewed the PSI Program to ascertain compliance with ASME code and regulatory requirements.

Specification 8031-P-504, Revision A "Preservice Inspection Program" The reviewed documents identify the applicable ASME Code Edition and Addenda, areas of responsibility of PSI contractors, and examination and testing requirements, including regulatory and licensing commitments.
Additionally, the inspector found that the computerized system for tracking program status regarding completed examinations is updated at a frequency which allows the licensee to know the daily status of the program.
Examination data were found to be filed with cross references

so that they can be retrieved expeditiously with little or no delay.
No violations were identified.

Preservice Inspection NDE Procedure Review The inspector reviewed selected implementing NDE Procedures to ascertain compilance with ASME code and regulatory requirements and for technical adequacy.

G. E. procedure UT - Austenitic - M, Revision 0, " Manual Ultrasonic Examination of Similar and Dissimilar Metal Welds in Piping Systems" The inspector found that the procedures reviewed complied with applicable ASME code and regulatory requirements.
The procedures were written to meet the ASME Code Section XI, 1980 Edition through Winter 1981 Addenda.
The procedure for automatic examinations defines the method, equipment and requirements for automatic contact ultrasonic examination of austenttic and dissimilar metal welds and base material and requires the use of
system sensitivity which is sufficient for the detection of intergranual l
stress corrosion cracking. This procedure makes use of the Ultra Image data acquisition equipment used by the General Electric Company.
The remaining procedures specify equipment, indication recording level,
J calibration techniques and personnel qualification / certification j
requirements.
No violations were identified.

6.
Observations of Work in Progress The inspector observed the ultrasonic examination of the following welds:
VRR - 2 RD - 2A - WA9, Recirculation system Tee to Cross connection weld
l ER8 - 206 - 1 - FW2, Main steam system Pipe to Elbow weld
The inspector's observations were made to ascertain that the examinations were performed in accordance with applicable procedures by qualified and I
certified examination personnel.
The recirculation system weld was examined by a General Electric Company i
technician, and the main steam weld examination was performed by Bechtel I
personnel.
Each examination was performed manually.
In addition to the examinations the inspector witnessed calibration of the examination system and subsequent calibration checks.
The inspector found that the examinations were done in accordance with
the appitcable procedure, and a review of personnel qualification / certification records confirmed that the examination personnel were properly qualified and certified.
The intpector confirmed also that the examination equipment was used within its current I
calibration period, i

7.
Review of PSI Data The inspector reviewed selected data to ascertain its completeness, accuracy and compliance with ASME Code and regulatory requirements. Data associated with the following welds were included in the inspector's review:

The completed data packages are maintained by Bechtel, and the inspector found that the system provides for rapid retrieval of the data.
The inspector's review confirmed that ASME Code,Section XI requirements j
for volumetric and surface examinations with ultrasonic and liquid penetrant methods were met for each weld reviewed.
Ultrasonic indication locations were plotted to aid in the evaluation process, and all data were reviewed by licensee Level III personnel.

l The inspector verified that the plots were accurately drawn and his discussions with the Level III reviewers indicated that they had a thorough understanding of their responsibility and were qualified to i
perform their assigned duties.
Additionally, the inspector reviewed the preliminary data associated with the examinations listed in paragraph 6 of this report, and found that the data documented the conditions under which the examinations were performed, and that the results were accurately reported.

8.
Exit Meeting The inspector met with licensee representatives, denoted in paragraph 1, at the conclusion of the inspection on March 13, 1987.
The inspector summarized the purpose and scope of the inspection and the findings. At i
no time during this inspection was written material provided by the j
inspector to the licensee.
