SUBJECT: SEQUOYAH NUCLEAR PLANT - SUPPLEMENTAL INSPECTION REPORT 05000327/2013408 AND 05000328/2013408, AND CLOSED REGULATORY PERFORMANCE MEETING
Dear Mr. Shea:
On January 10, 2013, your staff failed to meet a regulatory requirement, in that, they failed to control vehicle access into the protected area (PA) and initiate an alarm for a potential threat. On March 8, 2013, the U. S. Nuclear Regulatory Commission (NRC) completed an inspection at your Sequoyah Units 1 and 2. Based on the results of this inspection, documented in NRC Inspection Report 50000327, 328/2013404 issued on March 18, 2013 and the final significance determination documented in NRC Inspection Report 50000327, 328/2013407 dated June 12, 2013, the NRC assigned a finding Action Matrix input to the security cornerstone in the first quarter of 2013. In response to this Action Matrix input, the NRC informed you that a supplemental inspection would be required.
On July 24, 2013, you informed the NRC that the station was ready for the supplemental inspection. On September 27, 2013, the NRC completed the supplemental inspection and the inspector discussed the results of this inspection, and the implementation of your corrective actions, with Mr. J. Carlin and other members of your staff. Immediately following the exit meeting, a regulatory performance meeting was conducted to discuss the results of the mid-cycle assessment letter dated September 3, 2013. The exit meeting and the regulatory performance meeting were not open to the public. Immediately following the exit meeting a regulatory assessment meeting was conducted to discuss the results of the mid cycle assessment letter dated September 3, 2013. The exit meeting and the regulatory performance meeting were not open to the public. Further, on May 22, 2013, the NRC had discussed the implementation of your immediate corrective actions with Mr. J. Carlin and other members of your staff. The inspector documented the results of this inspection in the enclosed inspection report. The NRC performed this supplemental inspection to determine if: (1) the root and contributing causes for the significant issues were understood, (2) the extent of condition and extent of cause for the identified issues were understood, and (3) your completed or planned corrective actions were sufficient to address and prevent repetition of the root and contributing causes.The NRC determined that the corrective actions to address the violation were determined to be adequate. The inspection objectives of inspection procedure (IP) 95001 have been satisfied.
The Greater than Green finding is considered closed. Therefore, the security performance issue will not be considered as an Action Matrix input after the end of the fourth Quarter 2013.
However, due to the two White findings in the mitigating systems cornerstone, plant performance will remain in the Degraded Cornerstone of the NRC's Action Matrix.
In accordance with Title 10 of the Code of Federal Regulations (10 CFR) 2.390, "Public inspections, exemptions, requests for withholding," of the NRC's "Rules of Practice," a copy of this letter will be available electronically for public inspection in the NRC Public Document Room, or from the Publicly Available Records (PARS) component of NRC's Agencywide Documents Access and Management System (ADAMS), which is accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html (the Public Electronic Reading Room).
However, the material enclosed herein contains Security-Related Information in accordance with 10 CFR 2.390(d)(1), and its disclosure to unauthorized individuals could present a security vulnerability.
