Dear Mr. Mueller:
SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT NO. 50-295/96008(DRP);
50-304/96008(DRP))
This will acknowledge receipt of your letter dated October 18, 1996, in

response to our letter dated September 16, 1996, transmitting a Notice of Violation associated with Inspection Report No. 50-295/96008(DRP);
50-304/96008(DRP). The corresponding-inspection had been completed on July 26, 1996.
Your response addressed inattention to detail which had contributed to an unacceptable sequence of errors and operational events at Zion, as well as your staff's inability to consistently identify problems in a timely manner.
These concerns were also discussed during our recent management meeting on October 7.
In addition, during the most recent resident inspection exit meeting on October 11, we expressed our concerns with the continuing trend in human-performance errors and operational events. The issue with operators making fundamental errors during the performance of required duties which demonstrated insufficient attention to detail and the issue with untimely or ineffective corrective actions to prevent recurrence were the subject of escalated enforcement action in August.
In your response, you described the work stoppage you instituted from October 8 through 14, and you committed to take additional management actions until you are successful in operating the Zion Station without errors or plant events. While we recognize that adequately addressing the fundamental root
causes of human performance problems requires long term corrective actions,

your interim corrective actions have not been effective to date.
We expect you to implement sufficient interim corrective actions to prevent the recurrence of problems. We will continue to evaluate the effectiveness of both your short term and longer term corrective actions in addressing these problem areas.
