Based on its review of your letter dated June 2,1998, and your correspondence dated June 5, -

1998, the staff has concluded that all licensable radioactive material has been removed from j your facility at 4831 Old Seward Highway, Suite 107, Anchorage, Alaska, and that residual l radioactive material attributable to licensed activities does not exceed current NRC criteria. 1 I
Bas'ed on these conclusions, no further remediation is required, and the 4831 Old Seward Highway facility has been removed from your license as a field station.
NRC expects licensees to conduct their programs with meticulous attention to detail and a high standard of compliance. Because of the serious consequences to em'ployees and the public i which can result from failure to comply with NRC requirements, you must conduct your program .l involving radioactive materials in accordance with the conditions of your NRC liceni.a, ]

We would appreciate it greatly if the matter of the omission of the sealed

source and exposure device combination, specifically AMSHM Model A42429i 140 curies, AMSHM Model 660B, from License Number 50-29015-01 could be urgently expedited. We have a total of six (6) exposure devices with this combination. that are required in the field to service existing contrscis. :
We are currently waiting for a response from Sentinel concerning the results i of the wipe tests that were performed during the decommissioning of the vault I located at 4831 Old Seward. As of 1 p.m., this date. St,ntinel had not received the tests in the mall.
All other required information is on hand and will be faxed and mailed to you i
with appropriate fees attached when th7 test results are received.

Dear Beth,
We would appreciate it greatly if the matter of the ombsion of the sealed source and exposure device combination,- specifically AMSHM Model A424-9l 140 curies, AMSHM Model 6608, from License Number 50-29015-01 could ,
be urgently expedited. We have a total of six (6) exposure devices with this combination that are required in the field to service existing contracts.
All sources cnd exposure devices that were located at 4831 Old Seward were
accounted for and moved with the existing storage vault to the new Field i Station location. i i
l We are currently waiting for a response from Sentinel concerning the results 1 of the wipe tests that were performed during the decommissioning of the vault located at 4831 Old Seward. As of 1 p.m., June 4*, Sentinel had not received the tests in the mail.
Enclosed is a drawing of the new Field Station located at 7930 King Street in Anchorage. This will be the primary storage vault and no gamma radiography will be performed at this location.
The office of the RSO is located at 301 Arctic Slope Avenue, Suite 100 in l
Anchorago. All necessary documentation is also located at this address.
