The inspection involved thirty-five (35) inspection-hours on site by one (1) hTC inspector.

Licensee Action on Previoas Inspection Findings (Closed) Open Item (50-267/79-01):

Corrective action for CARS (Corrective Action Request) not completed within 60 days.

The licensee has significantly reduced the number of outstanding CARS with corrective action not completed within 60 days.

(Closed) Item of Noncomf ormance (50-267/79-03):

Failure to follow procedures.

The inspector examined the licensee's corrective action relative to failure to follow crocedures.

Department supervisors and the personnel involved have been re-instructed regarding the importance of completing the requirements statei in procedures.

Surveillance Activities The inspector witnessed surveillance tests performed on Class I piping system Hydraulic Snubbers MSS-257, BFS-423E and CRS-C3-5.

The tests were performed utilizing surveillance test 5.3-8d-R, " Hydraulic Snubber Functional Test," Revision 1, and the Bergen-Paterson Model 25000 Hydenulic Test Stand.

Each of the snubbers observed failed to pass the acceptance cr'.teria specified in SP-5.3.8d-R.

MSS-257 failed to achieve lock-up within the specified poppet closure rate during extension of 1 to 30 inches per minute and also failed to meet the specified release rate of 0.5 to 10 inches per minute for both extension and retraction.

BFS-423E also failed in this same manner.

to have a seal leak and to have a broken pin bearing.

Four other snubber tested prior to the inspection observation had also failed to pass the ecceptance criteria for various reasons:

leaking, no lockup, no bleed rate and low bleed rate.

However, two of these had been repaired and were tested successfully.

Because all seven snubbsrs tested had failed the licensee is required by the

,

technical specification to test additional snubbers.

As a consequence of the snubber failures during testing the licensee had scheduled a meeting with a representative of the snubber fabricator,

ITT Gtinell, to discuss the failures, the testing procedure and the availability of spare parts. Additionally, at the conclusion of the

inspection the licensee was considering reconditioning all class 1

.

piping system hydraulic snubbers prior to testing on the hydraulit test

-

stand.

This matter will remain as an open item pending successful

testing of the hydraulic snubbers.

3450A Install battery charger, batteries, power distrubution panels, supply transformers, and install cabling and terminate.

3451 Install cables for ACM primary power distribution and ACM Diesel Control.

3452A Frovide Electrical interconnection of ACM Diesel-Generator Control panels with ACM 5 KV switchgear.

3453B Provide power feed to ACM load center from turbine plant HVAC MCC.

3454C Install and Connect ACM R.A.T. Breaker to HVAC-4160 Switchgear; install and connect required circuitry.

3455B Install ACM power feeders and transfer switches for lighting circuit 81 and for service water return pumps 1A and IB.

Provide necessary lighting at the ACM power transfer switch area.

3459 Document Changer only - no associated work authorization.

3460A Disconnect IACM loads from IACM MCC (N-4801) and reconnect to ACM L.C. switchgear (N-4868) and ACM MCC (N-4870).

3462B Add 40 KVA isolating transformer to provide a ground neutral for the ACM 277 V. lighting distribution panel.

3463B Identify conduits, boxes aad equipment as required, revise documents.

3719 Modify ACM Switchgear.

To complete the ACM installation change notices 524, 703 and 718 remain to be implemented as well as functional test 4358.

CN 524 requires the pulling of 12 cables, CN 703 requires addition of a back feed (to accommodate hard testing), and CN 718 requires resolution of voltage requirements. FT4350 will verify that the ACM Diesel Generator functions properly on starting and includes engine safety features.

SOP 48-01 has been completed for operation of the alternate cooling method.

Completion of the ACM installation and checkout remains as an open item.

No items of noncompliance or deviations were identified.

(b) Stage 3 Fire Protection System Upgrading and flow testing of the fixed water spray system, change notice (CN) 543, for the Reactor Building and the Turbine Building has been completed.

Relocation of some smoke detectors, and conversion of the halon system to an automatic system, CN 554, has also been completed. Work to accomplish CN 517 and CN 619 is in progress.

CN 517 rectires sealing of the control room floor c

(penetrations), sealing the 480 and in the auxiliary electrical equipment room.

CN 619 requires the installation of spray shields in the auxiliary electrical cquipment room and portions of the reactor building for protection of electrical equipment.

This matter remains open pending completion of work required by CN 5.17 and 619 and a review of required procedures.

No items of noncompliance or deviations were identified.

(c) Firewater Booster Pumps Two fire water booster pumps P-2109 and P-2110, were added by FCN 3651A to supply fire water at an increased pressure through the emergency condensate or emergency feedwater headers to thc :irculator pelton wheels. During observation of the installed system it was determined that pump P-2109 had been removed for maintenance and that FCN 3651B was in the process of being issued to require moving a portion of the associated piping to eliminated interference with plant piping already installed. This matter will remain open pending completion of the installation, functional testing of the equipment, and a review of procedural revisions.

5.

Startup Report The ninth startup report for Fort St. Vrain for the period August 22, 1978 through November 22, 1978, reviewed by the inspector, contained no relevant information on the initial approach to power tests since none were performed during this period of time due to the restriction for plant operation in excess of 70 percent power.

6.

Exit Interview An exit interview was conducted with representatives of the licensee on April 20, 1979, at the conclusion of the inspection.

At the interview, the inspector discussed the findings indicated in the previous paragraphs.

The licensee acknowledged these findings.
