Dear Ms. Riedlinger:

This letter is in response to your July 1, 1988 letter sent to our laboratory regarding renewal of our byproduct material license. The following information addresses the questions raised in your letter:

1. The actual site the licensed materials will be stored is at our laboratory facility located in the Colonial Park Professional Building at Mile 6.5 Palmer-Wasilla Highway near Kosilla, Alaska. The gange will bt used at various field i locationns where construction activitiles are occurring within the State of Alaska and are specified as " temporary j ob sit es."

2. Licensed material will be used by, or under the supervision and in the physical presence of, Bob Gilfilian, Steve R.

Rowland, J. Steve Rebillard, Bill Queitzsch, C. Peter Curtis, i or any other individuals who have completed the !

manuf acturer's training course in the use of the devices and have been designated by the Radiation Protection Officer, Bob Gilfilian. Attached are copies of these individual's certificate of training.

3. Our laborat ory has one (1) Model 3400 Series Moisture-Density Gauge manufactured by Troxler.

4. The supplier of our personnel monnitoring devices is R.S. ,

Landauer, J r. & C o. , and these devices consist of film badger that are exchange monthly during the use of our sealed source device.

5. We intend to continue using our sealed source.

'This is in reference to your request dated February 19, 1988, and received on June 17, 1988, for renewal of your byproduct material license. Because the license expired on May 31, 1988, we will consider your request as an application for a new license. This was discussed with you in.a telephone discussion on June 9,1988 with Frances Roselinsky of this office. In order to complete our review, we need the following additional information:

1. Please submit information on the actual site where the licensed materials will be'used. . Permanent facilities such as field office storage areas for the gauge should be identified by' street address, city, anti state.

In addition, field locations of use should be specified as " temporary job sites."

2. Please provide the name and qualifications of each person who will use the licensed material. An authorized individual must be present and.

directly supervise use at any temporary job site. Use or qualification should include, as a minimum, the completion of the device manufacturer's training course or program.

3. Your application does not give the name of the manufacturer and model number of each sealed source and device. This information is necessary to ensure that all sealed sources, and devices containing radioactive materials, used by your organization have been registered by the manufacturer with either the NRC or an Agreement State.

4. With regard to personnel monitoring devices, you should specify:

a. Name of supplier b. Type of devices used (e.g., film badges, TLD)

c. . Frequency of changing monitoring devices (Note: Film badges should be exchanged at least monthly, and TLD's should be exchanged at leastquarterly)

In the event the sealed sources will no longer be needed, you should specify.your means of disposal. Sealed sources containing byproduct material may be returned to the manufacturer,- transferred to another licensee authorized to possess the specific quantity and form being transferred, or transferred to a licensed waste disposal firm.

We.will continue the review of your request for a byproduct material license-upon receipt of this information. If we do not receive a reply from you within do not 30 calendar wish.to days pursue from your the date of this. letter, we shall assume that you application. Please reply in duplicate, and refer to Mail-Control No. 70786
