Dear Sir or Madam:

By letter dated August 28, 2015 (Reference 1), the USN RC issued a revised NRC Form 311 Quality Assurance Program Approval for Radioactive Material Packages to Entergy Nuclear Northeast, Entergy Nuclear Operations, Inc. - Vermont Yankee. By letter dated October 11, 2018 (Reference 2), the USN RC provided consent to the direct and indirect transfer of the Vermont Yankee (VY) license from Entergy Nuclear Operations, Inc. (ENOI) and Entergy Nuclear Vermont Yankee, to Northstar Nuclear Decommissioning Company, LLC (NOC). The transfer of the VY license is expected to be completed on January 11, 2019, assuming that all closing conditions and all of the conditions contained in the regulatory approval orders have been satisfied.

BVY 19-002 / Page 2 of 3 On May 1, 2018, (Reference 3), the USNRC approved the VY Quality Assurance Program Manual (QAPM), Revision 7. This change to the QAPM reflects the transition of the decommissioning status of VY following the completion of the transfer of all spent.nuclear fuel from the spent fuel pool to dry fuel storage in the ISFSI, and represented a reduction in commitments from the previously approved VY QAPM. The VY QAPM satisfies the requirements of 10 CFR 50 Appendix B, 10 CFR 71 Subpart H, and 10 CFR 72 Subp~rt G.

Pursuant to 10 CFR 101 (f), this letter notifies the NRC of VY's intent to apply our previously approved QAPM (including any subsequent changes that did not constitute a reduction in commitment) to transportation activities.

In accordance with 10 CFR 101 (f), ENOI, on behalf of Northstar NOC, herby requests the following changes to the subject Form 311 Quality Assurance Program Approval for Radioactive Material Packages:

1) Revise Block 2 "Name" to:

Northstar Vermont Yankee, LLC and Northstar Nuclear Decommissioning Company, LLC - Vermont Yankee

2) Revise Block 2 "Street Address" to:

320 Governor Hunt Road

3) Revise Block 2 "City" and "Zip Code" to:

Vernon and 05354 A marked up copy of the NRC Form 311 has been provided as an attachment to this letter.

VY requests that this change be approved as soon as practical and will be effective upon issuance coupled with the completed transfer of the VY License.

This letter contains no new regulatory commitments.

5 CONDITIONS

1. Activities conducted with regard to transportation packagings under applicable criteria of Appendix B to 10 CFR Part 50 are authorized by this approval : procurement, maintenance, repair, and use. All other activities (i. e., design , fabrication , assembly, testing , and modification) shall be satisfied by obtaining certifications from packaging suppliers that these activities were conducted in accordance with an NRG-approved Quality Assurance Program . It shall remain the responsibility of the Quality Assurance Program holder that all transportatio n activities meet the requirements of 10 CFR 71.101 .
2. Records shall be maintained in accordance with the provisions of 10 CFR Part 71 . Specifically:
a. Records of each shipment of licensed material shall be maintained for 3 years after that shipment (10 CFR 71 .91 (a)].
b. Records providing evidence of packaging quality shall be maintained for 3 years after the life of the packaging (10 CFR 71 .91 (d)J.
c. Records describing activities affecting packaging q ality shall pe maintained for 3 years after this Quality Assurance Program Approval is terminated [10 CFR 71 .135).
3. Planned and periodic audits of all aspects of the Quality Assurance Program shall be conducted in accordance with written procedures or checklists, by appropriately trained personnel not having direct responsibility in the areas being audited , in accordance with 10 CFR 71 .137.
