| Title | Failure to Identify Inoperable Feedwater Isolation Valve Exceeds Technical Specification Allowed Outage Time (Section 4OA3) |
| Description | A self-revealing non-cited violation of Technical Specification 3.7.3.c was identified for the failure of operations personnel to perform the actions required for an inoperable main feedwater isolation valve. Specifically, on July 17, 2006, operations personnel failed to perform actions to place the unit in Mode 3 within 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
and Mode 5 within 36 hours4.166667e-4 days
0.01 hours
5.952381e-5 weeks
1.3698e-5 months
, as required by Technical Specification 3.7.3.c, for an inoperable main feedwater isolation valve that had not been closed or isolated in 72 hours8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
, as required by Technical Specification 3.7.3.a. This resulted in main feedwater isolation Valve 2JSGAUV0174 to steam Generator A exceeding the Technical Specification 3.7.3 allowed outage time. This issue was entered into the licensee\\\'s corrective action program as Condition Report/Disposition Request 2915450. This finding is greater than minor because it is associated with the equipment performance attribute of the mitigating systems cornerstone and affects the cornerstone objective to ensure the availability and reliability of systems that respond to initiating events to prevent undesirable consequences. A Phase 2 analysis was required because the Manual Chapter 0609, \\\"Significance Determination Process,\\\" Phase 1 Worksheets, determined that there was a loss of main feedwater isolation of a single train to steam Generator A for greater than the technical specification allowed outage time. Using the Phase 2 Worksheets associated with a steam generator tube rupture without steam generator isolation, the finding is determined to have very low safety significance since all remaining mitigation capability was available or recoverable.
