On 10/20/25, during Refueling Outage 27, 2 doors (Door 31011 and Door 33012) that are part of the control building envelope (CBE) boundary were discovered to be left open during core reload. Door 31011 was tied open without an active breach permit, and Door 33012 was left open in a manner inconsistent with the approved breach plan. Upon discovery, both doors were closed.
Both doors were open at the same time for approximately 13 minutes, from 0906 to 0919. The 2 doors being left open caused the CBE boundary to be inoperable. With the CBE boundary being inoperable while moving irradiated fuel, Technical Specification (TS) LCO 3.7.10 Required Action E.1 and E.2 require that core alterations and movement of irradiated fuel assemblies be suspended immediately. Since the CBE boundary was inoperable for 13 minutes while continuing to move irradiated fuel, this event is being reported under 10 CFR 50.73(a)(2)(i)(B) as an operation or condition prohibited by TS. SAFETY SIGNIFICANCE
There were no actual safety consequences. Although the CBE boundary was inoperable for a short duration, the CREVS remained functional, and no unfiltered in-leakage occurred. The event did not result in actual exposure or compromise of safety systems. The safety function to protect the control room inhabitants was maintained. During a licensing basis fuel handling accident, the open doors would affect the calculated control room dose; however, the impact is negligible and would not challenge the 5 TEDE control room dose limit.
OPERATING EXPERIENCE/PREVIOUS EVENTS A review of WCGS LERs submitted in the last three years identified no similar LERs.
