Title | Licensee-Identified Violation |
Description | The following violation of very low safety significance (Green) was identified by the licensee and is a violation of NRC requirements which meets the criteria of the NRC Enforcement Policy for being dispositioned as a NCV. A violation of Title 10 CFR 50, Appendix B, Criterion III, Design Control, which requires, in part, that design control measures shall provide for verifying or checking the adequacy of design, such as by the performance of design reviews, by the use of alternate or simplified calculational methods, or by the performance of a suitable testing program. Contrary to the above, from initial plant operation until November 2012, the licensee failed to verify the adequacy of the design of the EDG and TDAFWP speed circuits. Specifically, the licensee failed to verify the adequacy of the noise shielding design of the EDG and TDAFWP control circuits speed sensor signal line in accordance with the speed switchs vendor requirements. The design had the speed switch signal wire shield terminated to a case screw. However, the drawing provided by the switch manufacturer, Synchro-Start, required the wire shield to be terminated to terminal 14 in the control circuit. The violation is more than minor because it was associated with the design control attribute of the Mitigating Systems Cornerstone and affected the associated cornerstone objective to ensure availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609, Appendix A, The Significance Determination Process For Findings At-Power, the inspector determined that the violation is of very low safety significance (Green) because the finding was a deficiency affecting the design or qualification of a mitigating structure, system, or component, and did result in a loss of operability of the B EDG in 2006 and 2009. The licensee planned to rewire the EDG control circuits and replace the TDAFWP control cabinet during the current outage. Since the finding is of very low safety significance, was identified by the licensee, and has been entered into the licensees CAP as CR 65324, this violation is being treated as a NCV, consistent with Section 2.3.2 of the NRC Enforcement Policy. |
