| Description | Title 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures and Drawings, requires, in part that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Licensee procedure PPM 1.3.57, Barrier Impairment, Revision 0-32 is a safety related procedure used to establish compensatory measures to maintain equipment operable following a high energy line break when certain hazard barriers are removed. Contrary to the above, prior to April 15, 2015, the licensee failed to prescribe procedures of a type appropriate to the circumstances for activities affecting quality. Specifically, licensee procedure PPM 1.3.57, Barrier Impairment, Revision 0-32 failed to establish appropriate measures to maintain electrical instrument racks operable following a postulated high energy line break because the licensee incorrectly concluded that high energy line break events are slow developing events where there would be sufficient time to implement compensatory measure to restore disabled barriers. The finding was of very low safety significance because the finding did not represent an actual loss of function of one or more non-technical specification trains of equipment designated as high safety-significant in accordance with the licensees maintenance rule program for greater than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. This issue was entered into the licensees corrective action program as AR 325768. |
