| Description | Technical Specification 3.6.4.1, Secondary Containment, requires, in part, that the secondary containment shall be operable in Modes 1, 2, and 3 and during operations with potential to drain the reactor vessel (OPDRVs). Technical Specification 3.6.4.1, Required Action C.1, requires that when secondary containment is inoperable during OPDRVs, action is immediately taken to suspend OPDRVs. Contrary to the above, June 3, 2013, secondary containment was inoperable during OPDRVs and action was not taken to immediately suspend OPDRVs. This finding was identified by the licensee and entered in the licensees corrective action program as AR 286676286676 This finding was determined to be of very low safety significance because the finding did not increase the likelihood of a loss of reactor coolant system inventory, the finding did not degrade the licensees ability to terminate a leak path or add reactor coolant system inventory when needed and the finding did not degrade the licensees ability to recover decay heat removal systems if lost |
