| Description | TS 6.8.1.f requires in part that written procedures shall be implemented covering the activities for the fire protection program. Contrary to the above, on September 25, 2012, the licensee failed to implement fire protection program procedure, FPP-025, Fire Containment, because required compensatory actions were not established for approximately 138.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> for an inoperable door, DRIB/409, with fire protection and steam propagation barrier requirements. This PD is more than minor and therefore a finding because it impacted the mitigating systems cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences and the attribute of human performance performance because the licensee failed to follow FPP-025 to ensure the appropriate compensatory measures were established for the inoperable door. The inspectors used IMC 0609, Attachment 4 and Appendix A and F, and determined the finding was of very low safety significance or Green, because (1) the configuration of the door and associated corridor presented a low challenge from a turbine building fire and fire detection instrumentation was operable on both sides of the affected door, and (2) the affected door closure mechanism was functional and maintained the door in a closed position which would allow steam pressure from a steam line break in the turbine building to force the door tightly against the seals. This finding has been entered into the licensees CAP as CR- 12-04139. |
