Technical Specification 3.6.5 1 requires secondary containment integrity to be maintained at all times in condition 1. On November 18, 1982 both' i doors into the Unit i Reactor Building on elevation 761' opened at the i same time while the Unit was in Condition 1.
V. PROBABLE CONSEQUENCES OF THE OCCURRENCE: ,
The purpose of secondary containment is to ensure that no uncontrolled
' radiocctive releases occur. When door 418 was opened the differential pressure caused door 285 to open allowing an inflow of air to the reactor building. Although the definition of secondary containment was violated, negative pressure was maintained in the Reactor Building and
- the air flow was from the Turbine building into the Reactor Building. In addition primary containment integrity was in effect at all times. Safe.
j operation of the plant was maintained. , The health and safety of the public was not affected.
i VI. CAUSE: l I The cause of this occurrence was that door 285 electric strike was out- ,
i of-adjustment. Therefore the latch bolt could not work properly. The lock was manufactured by Yale model 8048.
j Vll. CORRECTIVE ACTION:
! The electric strike was adjusted the same day so that the. latch bolt worked properly. Procedure LO3-CS-MI, Secondary Containment Integrity
' for Conditions 1, 2, 3 and
- will be revised to check the' operability
! of the Reactor Building interlock doors. AIR l-82-577 has been generated to track this procedure change.
