NARRATIVE

REPORT FOR LER 2-79-26 On February 28,1979, at 1000 CST while testing safety related hanger anchor bolts per ' Inspection and Testing Procedure for Concrete Expansion Anchors' procedure HNP-2-11005, it was noted that a 9.84 percent failure was found when testing had been completed on 183 anchor bolts.

Although this dev-iation from normal performance of plant safety related structures requires reporting per Tech Specs section 6.9.1.8.i, the plant was already shutdown and thus this occurrence had no significant affect on plant safety.

This was a non-repetitive occurrence.

The cause of the failure of the self-drilling Phillipsy' Red Head' type an-chors resulted from improper installation.

The method'to determine if a proper installation was made is to test the self-drilling and wedge type anchors per HNP-2-11005.

All safety related system hangers will have its anchor bolts tested and repaired by replacement as required per HNP-2-11005 before startup is allowed.

All failed anchor bolts will be replaced with wedge type anchors in the future.
