Title | Failure to Test Eight Valves in Accordance with the IST Program |
Description | The inspectors identified a finding of very low safety significance (Green) and associated Non-Cited Violation of Technical Specification 5.5.6, Inservice Testing Program, for the failure to perform the required testing in accordance with the American Society of Mechanical Engineers Code for eight valves that had active safety functions. Specifically, these valves were required to operate in Mode 3 to return the residual heat removal system from the shutdown cooling mode to the low pressure coolant injection mode of operation. The licensee entered this finding into their corrective action program and verified that the valves were operable based on recent exercising of the valves during the last refueling outage. The finding was determined to be more than minor because it was associated with the Mitigating Systems Cornerstone attribute of Equipment Performance, and affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the licensee would be unable to trend the performance of the valves due to inadequate testing, which could result in not identifying degraded valve performance. The finding screened as very low safety significance (Green) because the finding was not a design or qualification deficiency, did not represent a loss of system safety function, and did not screen as potentially risk significant due to a seismic, flooding, or severe weather initiating event. The finding had a cross-cutting aspect in the area of problem identification and resolution because the licensee failed to identify a condition adverse to quality. Specifically, when the licensee identified the concern with additional valves during an extent of condition review, the licensee failed to initiate a new action request to ensure the condition adverse to quality was adequately evaluated. |
