| Description | Duane Arnold Operating License Condition 2.C.(2) requires, in part, that the licensee shall operate the facility in accordance with the Technical Specifications. Contrary to this requirement, the licensee did not operate the facility in accordance with TS 3.3.1.1, Reactor Protection System Instrumentation, and 3.3.4.1, End of Cycle Recirculation Pump Trip Instrumentation on January 4, 2010. Ultimately, TS Required Actions to reduce thermal power to <26% rated thermal power were not taken within the associated Completion Times for TS 3.3.1.1, Condition E, and TS 3.3.4.1, Condition C. The facility was in violation of TS 3.3.1.1 and 3.3.4.1 for approximately 1.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> and 0.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />, respectively. The licensee entered the issues into their corrective action program as CAP 072527. Based on consultation with the NRC Region III Senior Risk Analyst, who determined that the delta core damage frequency for this finding was much less than 1E-6, it was considered to be of very low safety significance (Green). |
