10 CFR 50 Appendix B, Criterion XVI, Corrective Action, requires, in part, measures shall be established to assure that
conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected. Contrary to the above, between July 2000 and Sept 14, 2015, the licensee failed to implement corrective actions which would have identified a
condition adverse to quality on the output switch on the Spare Vital Inverter 0-II. This issue was entered into the licensees corrective action program as CR 1081482. The finding was screened using
Inspection Manual Chapter 0609, Appendix A, The
Significance Determination Process for Findings At-Power, and was determined to be of very low safety significance (Green).
