Title | Inadequate Effectiveness Reviews for Corrective Actions to Prevent Recurrence |
Description | The team identified a Green finding for the licensees failure to ensure that effectiveness reviews to assess the adequacy of corrective actions as required by procedure EN-LI-118-ANO-RC, Cause Evaluation Process, were appropriate. Specifically, the team identified numerous examples in which effectiveness reviews for corrective actions to prevent recurrence failed to assess whether corrective actions achieved the intended results. The licensees corrective actions included revising the effectiveness reviews to ensure that the corrective actions achieve the desired effect, and documenting the issue in the corrective action program as condition reports CR-ANO-C-2016-00482 and CR-ANO-C-2016-01013. The failure to establish adequate effectiveness review success criteria to verify the intended results for corrective actions to prevent recurrence were achieved was a performance deficiency. The performance deficiency was determined to be more than minor because, it impacted the human performance attribute of the Initiating Events cornerstone and adversely affected the cornerstone objective to limit the likelihood of events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. Specifically, the failure to complete actions related to identifying and correcting the extent for a significant condition adverse to quality could potentially lead to an initiating event. The finding was evaluated using Inspection Manual Chapter 0609, Significance Determination Process, Attachment 0609.04, Initial Characterization of Findings, and Appendix A, The Significance Determination Process (SDP) for Findings At-Power, Exhibit 1 Initiating Events Screening Questions, dated June 19, 2012. The team determined the finding was of very low safety significance (Green) because it did not cause a reactor trip or the loss of mitigation equipment relied upon to transition the plant from the onset of the trip to a stable shutdown condition. This finding had a problem identification and resolution cross-cutting aspect of Self-Assessment because the licensee did not ensure that the organization routinely conducted self-critical and objective assessments of its programs and practices. Specifically, the Corrective Action Review Board tasked with validating the effectiveness of the corrective action plans did not ensure that the effectiveness review plans assessed whether the implemented corrective actions were effective [P.6]. |
