Description | The inspectors identified a non-cited violation of 10 CFR 50, Appendix B, Criterion V, Instructions, Procedures and Drawings, because Exelon did not specify, in writing, the exact inspection scope and criteria for boric acid inspections of the reactor coolant pump bolted flanges during plant refueling outages. Lack of specific procedural guidance contributed to the failure to detect reactor coolant system leakage from the thermal barrier flange of the B reactor coolant pump (RC-P-1B) prior to November 2011. Exelons failure to ensure that both the upper and lower RCP thermal barrier flanges were visually inspected for the complete 360 degrees for all RCPs is a performance deficiency within Exelons ability to foresee and prevent. Exelon completed a boric acid evaluation which showed there was reasonable assurance that the flange could safely operate until the next refueling outage. Additionally, Exelon prepared an adverse condition monitoring plan and is performing periodic remote monitoring of the affected flange for changes in leakage from the degraded gasket. Exelon entered this issue into the corrective action program as IR 01344561. The finding is more than minor because it is associated with the Equipment Performance attribute (a degraded RCP flange gasket) of the Initiating Events cornerstone and affected the cornerstone objective to limit the likelihood of those events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. Also, this finding is similar to the more than minor example 4.a in Inspection Manual Chapter (IMC) 0612, Power Reactor Inspection Reports, Appendix E. The inspectors completed IMC 0609.04, Phase 1- Initial Screening and Characterization of Findings, and screened the finding as very low safety significance (Green). This finding has a cross-cutting aspect in the area of Human Performance, Work Control, because Exelon did not ensure supervisory and management oversight of work activities, including contractors, such that nuclear safety is supported |
