Inadequate Procedure for Turbine Startup Caused a Reactor Trip

Description

A self-revealing Green NCV of TS 5.4.1, Procedures, was identified because Entergy did not provide adequate guidance in 2-SOP-26.4, Turbine Generator Startup, Synchronization, Voltage Control, and Shutdown. Specifically, Entergy did not provide adequate procedural direction to ensure the main turbine control oil stop valve Z was in the correct position. As a result, the steam generator water level exceeded the trip setpoint for the main boiler feed pumps which led the operators to insert a manual reactor trip.
