Dear Sir:

This letter provides a follow up report describing a reportable occurrence involving the Refueling Water Storage Tank. Submittal is in accordance with the reporting requirements stipulated in Section 6.9.2(a) of Appendix A to the Provisional Operating License DPR-13. This occurrence was originally reported by our letter dated November 10, 1981.

On November 9,1981, at 1508, at routine boron analysis of the contents of the Refueling Water Storage Tank was out of specification at 3731 ppm. Appendix A Technical Specification 3.3.3 requires a minimum boron concentration of not less than 3750 ppm. Within the one hour allowed by Technical Specification 3.0, the refueling water storage tank boron concentration was increased above the minimum permissible.

Our investigation into this event determined that non-representative boron concentration samples were taken the prior week. Sufficient time for adequate mixing must pass during Refueling Water Storage Tank recirculation before a representative sample can be taken.

Plant procedures were reviewed and two improvements have been made:

1.

A precaution has been added to warn operators that adequate time for tank mixing must be allowed prior to obtaining boron concentration samples.

2.

Following a refueling water storage tank boration or dilution, boron concentration samples must agree with predicted concentration within specified limits.

If agreement is not reached, the tank will continue to be recirculated and sampled until this condition is satisfied or consistent sample results are obtained.
