Some of the information received may not yet be fully verified or evaluated by the Region Staff.

UPDATE: JAMES A. FITZPATRICK NUCLEAR POWER PLANT NOTIFICATION OF UNUSUAL EVENT DUE TO FIRE IN MAIN TRANSFORMER

This preliminary notification supplements information in PNO-1-12-008, which documented the declaration of an Unusual Event at the Fitzpatrick plant on November 11, 2012, following a fire in the T1-A main transformer. On-site fire brigade personnel, with assistance from a local fire department, extinguished the transformer fire.

Operators declared an Unusual Event (UE) at 5:45 a.m. EST and exited the UE at 8:01 a.m. the same day. The plant includes two main transformers T1-A and T1-B. These transformers are located on-site, outside and adjacent to the turbine building.

On November 12, 2012, while the FitzPatrick plant was shut down, Entergy staff began maintenance activities that included inspection of the T1-A and T1-B main transformers.

Entergy's investigation determined that the problem initiated in the "B" Phase winding of the T1-A main transformer that subsequently resulted in the fire. Entergy personnel replaced the T1-A

main transformer and determined the T1-B was not damaged. FitzPatrick operators re-started the plant after completing replacement and testing of the replacement T1-A main transformer, and testing of other similar transformers. The plant reached full power on November 27, 2012.

The NRC staff continues to closely follow Entergy' s causal evaluations of the transformer failure and corrective actions. NRC oversight is being conducted through the NRC's baseline inspection program.

Entergy issued two news releases on November 11, 2012, related to the Unusual Event declaration, transformer fire, and termination. The information presented herein has been discussed with Entergy and is current as of November 28, 2010, at 10:00 a.m. Region I Public Affairs has responded to media inquiries.
