This report is being submitted in accordance with 10 CFR 50.73 (a)(2)(i)(B), an operation prohibited by Technical Specifications. This event is considered to be of no significance with respect to the health and safety of the public. There are no regulatory commitments contained in this LER.

Abstract

The two sub-trains of the starting air system for each diesel generator had been cross-tied during maintenance activities without complying with Technical Specification (TS) Required Actions.

This event was determined reportable per 10 CFR 50.73(a)(2)(i)(B) -

Operations Prohibited By Technical Specifications.

The root cause was an inadequate 10 CFR 50.59 Evaluation associated with a Nuclear Station Modification (NSM) in 1994.

The contributing causes were deviation from Westinghouse Standard TS (NUREG-1431),

misapplication of TS Condition, vague Safety Evaluation wording, and inadequate Updated Final Safety Analysis Report (UFSAR).

This event is considered to have ho significance with respect to the health and safety of the public.

Significant corrective actions include evaluating additional NSMs and associated 10 CFR 50.59 Evaluations, revising the affected UFSAR, and submitting a License Amendment Request to revise the affected Technical Specification.

After further evaluation, no similar event was found; therefore, this event is not considered recurring.
