TOLEDO EDISON CG'!PA'iY DAVIS-BESSE NUCLEAR PO'!ER ETAT 105 UNIT ONE SUPPLEMENTAL INFOPJL\\ TION For LER SP-33-79-S3 DATE OF EVENT:

July 8, 1979 F ACILITY:

Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE:

Containment Spray Ptrap 1-2 outboard bearing oil reser-voir leak Conditions Prior to Occurrence:

The unit was in Mode 3, with Power (MWT) = 0, and Load (Cross MWE) = 0.

Description of Occurrence: On July 8,1979, while making routine rounds, op era t ion s personnel observed that there was no visible oil level in Containment Spray Punp 1-2 outboard bearing oil reservo ir sigh t glass, and oil was found en the floor. Mainten-ance personnel were contacted to investigate the oil leak and refill the il reservoir.

The pump was declared inoperable at 0130 hours0.0015 days
0.0361 hours
2.149471e-4 weeks
4.9465e-5 months
on July 8,1979, placing t.ie unit in the action statement of Technical Specification 3.6.2.1 which requires the operability of tw> independent containment spray syst.ns in Modes 1, 2, 3, and 4.

_c c.,..,,,, r occurrenc e: The apparent cause is componenr ttiinre.

pg_q

.;y Investigation conducted under Maintenance Work Order 79-2462 found oil leaking it a gasketed joint at the bottom of the sight glass and at a threaded plug in the center of the hearing end cover. No maintenance had been done on this pump since.Tanuary, 1979.

The leaks may have developed from vibration loosening the nut en top ot the sight glass and the plug in the center of the bearing end cover.

Analysis of Occurrence: There was no danger to the health and saf ety of the public or to station personnel. Containment Spray Pump 1-1 was operable and capabic of per-forming the intended safety function if required.

Corrective Action

Under Maintenant.e Work Order 79-2462, oil was added to the out-board bearing reservoir and the fittings were ti;;h t ene d.

The pump was declared opera-ble at 0400 hours0.00463 days
0.111 hours
6.613757e-4 weeks
1.522e-4 months
on July 8, 1979, removing the unit from the action statement of Technical Specification 3.6. 2.1.

"o further leaks have been observed. Maintenance Work Order 79-2529 was issued to lock the above mentioned fittings on both pumps with Loctite to prevent future loosening fron vibration.

Failure Data:

No pr_.vious oil leaks have occurred on Containment Spray Pumps.
