NARRATIVE

REPORT Georgia power Company Plant E. 1. Hatch Baxley, Georgia 31513 Reportabic Occurrence Report No. 50-366/1980-062.

On April 19,1980, while performing the primary Containsent Isolation System Operability Test, the Drywell was vented thru the 2T48-F323 Torus Vacuum Breaker Valves.

Vacuum Breaker Valve, 2T48-F323E, failed to open when the

control switch was actuated.

Tech Spec requirements uere not applicable as' the reactor was in cold shutdown. The failure of a Torus Yacuum Breaker was repetitive, reference Reportable Occurrence No. 50-366/1980-018.

.During this outgge, the air cylinders on the vacuum breater valves, 2T48-F323A thru II, were disassembled and inspected.

They were reastembled using new seals and gaskets. New limit switches were also installed.

Following maintenance on the vacuum breaker valves, a leak rate test was performed on each valve.

prior to performing the leak rate test, personnel disconnected the air supply lines to prevent actuation with the test switch.

When personnel reconnected the 2T48-F323E valve, the Svage-Lock ferrul was not reinstalled. This allowed the air line to come loose and caused the O

valve to fail when acutated from the control room.

The fitting was replaced and the valve was tested satisfactorily.

Public health and safety acre not affected as the reactor was in cold shut-down and the valve would have actuated had a differential existed between the Suppression Chamber and Drywell.

No inherent problems were discovered.during the generic review.
