Descriotion of Occurrence: At 2030 hours0.0235 days <br />0.564 hours <br />0.00336 weeks <br />7.72415e-4 months <br /> on January 3,197S, unidentified RCS leakage was approximately 1.6 GP:1. This placed the unit in the Action Statement of Technical Specification 3.4.6.2 which requires unidentified leakage to be i

less than 1 GPM in Modes 1, 2, 3 and 4.

The unit was reduced to 40" power ac'2400 hours0.0278 days <br />0.667 hours <br />0.00397 weeks <br />9.132e-4 months <br /> on January 3,1978 and the unidentified leakage uns determined to be a pach-ing leak on DR.2735, the Pressurizer Auxiliary Spray Containment Isolation Valve.

The identificacian of the leakage removed the unit from the Action Statement of l

Technical Specification 3.i.6.2.

Dacinnation of Aonaren't Cause of Occurrence: The packing was too loose which allowed leakage out of the packing on DH 2735. The packing had been adjusted

.(loosened) por Maintenance teork order 77-2588 on December 3, 1977, for packing had previously been tightened to a point which caused valve to torque out when it was being operated. The packing gland was loose on LT-RC-14-3 Source Valve which allowed leakage out of the packing. The cause of the loose packing gland is unknown.
