IDENTIFICATION OF OCCURRENCE:

Diesel Camshaft Failure CONDITION PRIOR TO OCCURRENCE:

Mode 5 Cold Shutdown DESCRIPTION OF OCCURRENCE:

While performing routine surveillance on No. 2B Diesel Generator IAW Surveillance Requirement 4.8.1.1.2b, it was discovered that the coupling that connects two sections of the camshaft ha' all eight of its attaching bolts sheared.

Since this equipment failure was detected after the plant was shutdown, this failure must be reported as if the plant had been operating in a degraded mode in accordance with Technical Specification 6.9.1.9b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Inspection of the coupling revealed that one of the nuts was not attached to its bolt.

This would indicate that the joint may not have been properly tightened during initial assembly.

No other damage to the other journals was noticeable.

ANALYSIS OF OCURRENCE:

With this Diesel Generator out of service, the remaining two.conprise the minimum number required to meet the limiting condition for operation in Mode 5 IAW Technical Specification 3.8.1.2.

CORRECTIVE ACTION

Two new camshaft sections with their attaching nuts and bolts have been ordered.

Repairs are expected to be completed on January 26, 1981.
