l Sequoyah Unit 1-A Notice 'of Violation was issued to the licensee on' May 21,1981 based

.on alleged. violations' associated with the improper operation of the l

containment' spray system at the plant on February 11, 1981'. The

violations included:
1) inadequate.on-the-job, training of an auxiliary -

operator,:2) failure to implement appropriate procedures, and

3) inadequate administrative procedures.

Meeting with Congresswoman Marge Roukema On May 18,1981, NRC staff participated in a meeting in Congresswoman Roukema's office to discuss.the thorium contamination recently discovered at two private. residences in Maywood, New Jersey. 00E agreed to undertake a radiological assessment of the two residences to determine the extent _of ground contami' nation and what remedial actions are needed to reduce radiation exposures to acceptable levels.

EPA was asked to determine how funds might be made available from the Superfund Act to pay the cost of the required remedial actions.

t Radiation Policy Council On May 21, 1981, the Working Group of the Radiation Policy Council met to conti6ue'.its reexamination of the Federal guidance functicn for radiation protection which is now exercised by EPA. The Council is expected to go out of business at the end of the fiscal year.

In addition to lacking funding for FY '82, Carl Gerber, the Council's Director, met with reoresentatives of OMB who informed him that they are fundamentally opposed to interagency working grouos. An EPA l

member reported that it would take until December 1981 for EPA to analyze the-coments on their " Federal Radiation Protection Guidance fer Occupational Exposures" and to develoo a new strategy based on that analysis. There have been significant negative comments on most j

of the important features of the proposed guidance.

Final Report of Tsuruga Incident l

Tokyo newspapers are reporting that the final MITI report on the recent "Tsuruga Incident" was completed and delivered to the Nuclear Safety Commission on May 18. A copy is expected in the office of the MITI representative in Washington within the next week.

The. Federal Communications Commission in a Memorandun Opinion and Order

_(FCC-81-217,29301, Docket No. 20840, released May 18,.1981), requires the ibeep tone or the prior consent of all parties for recording telephone con-

.versations. Three exceptions to the re;ised tariff provision have been included in the Order; the first of which pertains to the NRC Operations Center:

"We have decided to allow three exceptions to the revised tariff requirement which will. supersede.the five current exceptions.1The first exception is for incoming-calls made to telephone numbers publicized for emergencies and

- outgoing calls made in immediate response.

In many types of emergency situ-ations, such as 'those involving fire, health care, and police, it is infeasible to obtain consent, and.use of the beep tone'could confuse callars or obliterate important portions of the message. Therefore we will not require the beep tone or prior consent for the recording of calls reporting or made in immediate response to these emergencies.. Recordings made at the Department of Defense Command Centers and the Operat'ons Center of the Nuclear Regulatory Commission will be included under this exception. See 59 FCC 2d 538 (1976) and Mimeo No.

06482 (January 29,1981).

" The secund exception is for the recording of. calls made for patently un-lawful. purposes, such as bomb threats, kidnap ransom requests, and obscene telephone calls. -Outgoing calls made in immediate response to such a call will also be excepted. Under thir exception the U. S. Secret Service will still be allowed to record calls-referred to it.which threaten the safety and security of the President, his immediate family, and the White House.

' The third exception is for recordings made pursuant to an explicit and lawful order of a. court issued pursuant to 18 U.S.C. 52516. We adopt this exception so that our requirement will not hinder law enforcement efforts or conflict with Title III of the Omnibus Act."

During the period May 15, 1981. through May 21, 1981, the effluents contained no detectable radioactivity at the discharge point although individual affluent sources which originated within Unit 2 contained usinute amounts of radioactivity.

Calculations indicate that less than 1 millionth (0.000001) of a curie of Cs-137 was discharged. This represents Icss than 0.00001% of the permissabic total liquid activity as specified in Technical Specifications for operatlanal conmercial reactors.

Airborne effluents from the TMI site released to the environment, af ter processing, were made within the regulatory limits and in accordance with NRC requirenents During the reporting period January 1.1981 - March 31,1981, the licenses reported the following gaseous releases:

The instantaneous airborne effluents are measured on a continuous basis, however for accurate calculation of total curies released effluent samples are analyzed in the laboratory and the data evalv. ted.

On the basis of the data from these samples, a draft effluent r, port is' issued monthly by the licensee. Airborne effluent infomat', will be provided in the weekly status report on a monthly basis.

Qcensee Redioactive Material and Radwaste shipments On Monday. May 18,1981, a 40 ml Unit 2 reactor coolant sample was sent to Babcock and Wilcox (BlW) 1.ynchburg, Virginia.

On Tuesday, May 19, 1981, the U.S. Departmon: of Energy (DOE) shipped an EPICOR-11 resin liner (PF-16) from Unit 2 to Battelle Columbus Laboratories in Columbus, Ohio.

(Theliner and resin will undergo examination and testing at the laboratory.)

May 20,1981, one 4' x 4' EPICOR-Il dewatered On Wednesday (liner DF-12) from Unit 2 was shipped to U.S. Ecology.

Major Activities 1.

. submerged Demineralizer System (50s).

Preparation of the Safety Evaluation RepoFt (SER) by the TML Program Office 4 in progress although some necessary information has not yet been received. On May 13,1981, the licensee submitted a revised schedule for providing the needed infunnation. The revised schedule indicates the infonnation will be provided by May 29, 1981.

Iunction41 tests are complete with the exception of a few items which are undergoing engineering evoluation and further testing.

Operator training on components not involved with the outstanding functional testing has been initiated.

The operator training does not involve processing of contaminated water.

Shipment of EPICOR-II Prefilter (PF-16) to Battelle Columbus Laboratories (BCL). As part of a DOE sponsured resin characterization study, PF-Ib was shipped to the DCL in West Jefferson, Ohio, on Hay 19,1981.

This research effort is to supplement the development of technology for safely processing contaminated resins and to evaluate liner material compatability.

PF-16 was part of the 1st stage EPICOR-II treatment process for the accident generated water collected in the Auxiliary Building.

Prior to shipment, special preparations were made to ensure that PF-16 did r;nt contain significant quantities of nonradioactive gases that could be generated by organic resin degradation (e.g.,

11. hydrocarbon gases, etc.) While gas generation was indicated 2(a result of the approximately 16 month on-site storage period),

5 4-the Pf-16 liner pressure was reduced to atmospheric.

No increase in. airborne activity levels was detected during this procedure.

.The liner was then placed in t. type "B" shipping cask which is designed to withstand a transportation accident.

The shipping cask weighs 32 tons and is constructed of steel and lead.

The PF-16 liner is constructed of steel with thickness ranging from 1/4 inch to 5/S inches and contains approximately 1,150 curies of radioactive cesium and strontium.

The shipment arrived safely at BC1. without incident after the one day trip.

The research effort will start inadiately and is expected to last over a 21/2 year period.

Reactor Building (RB) Entry an_d Purne. The eleventh entry into the Unit 2 RB 1s scheduled for Thursday, )iay 28, 1981 Tentative plans call for the completion of six tasks:

e Installation of safety equipment on the polar crane.

Installation of a portable, shielded gama spectrometer system.

Troubicshoot and repair of the Gai-Trunics (an installed comunications system within the Containment Building).

l Survey of the "D Ring" area, preventive maintenance of certain i

valves, and photography of the PORV (pressure operated relief valve).

Complete the hose connections to the SOS surface suction pump.

Troubleshoot and repair an electrical panel (lighting) on the 347' elevation.

If, all tasks are attempted,18 men will enter the RB.

Complete plans for all proposed tasks w'lll be reviewed by the TMI Program 3

Office prior to the entry.

An RB purge will co.mence one day prior to entry.

It is expected that less than three curies of Kr-85 will be released during the purge.

Solid Waste 5tga ing Fac-111ty Sump Contamination.

The licensee continues to colleneckly sampliis of water from the solid wLie staging

~

facility sump. These stmples are analyzed for radioactive isotope contents as a method for early indication of liner degradation.

To date the results of the analysis has been stab 1c and do not i

indicate liner degradation.

The "A" staging module, in service since January 1980, now has E5 spent-resin linars; the B module, in service since December 1980, now has 18 spent-resin liners.

B&W Navy-Lynchburg 10 SRD/ Evaluation As part of the contract effort, NRC-02-81-028 " Evaluation of Material Accounting Performance at Nuclear Fuel Cycle Facilities", Nusac, Inc., and NRC personnel visited the B&W Navy fuel manufacturing facility at Lynchburg, Virginia,- during the week of May 11, 1981. Data was collected and discussions held with facility personnel in order to evaluate B&W's material accounting experience.

The AMASS model will be applied to independently compute the LEID and any differences with B&W results will be reconciled. The ID variance contribution due to non-measurement sources will be assessed by statistically examining B&W's historic'ID performance over the past several years.

Preliminary results of the evaluation should be available by the end of June.

Meeting of Developers of High-Level Waste Forms for Defense Waste i

.The Savannah River Laboratory (SRL) sponsored a meeting of developers of high-level

. waste forms for defense waste i.. Atlanta on May 18 and 19. The purpose of the meeting was to allow respective developers to present proce:..ing and performance information on waste forms they have be2n developing under SRL/ DOE contracts to a Peer Review Panel. The Panel's job is to assess the information and select two l

. most promising waste forms. The waste forms reviewed by the panel were SYNROC, I

FLECHT SEASET Westinghouse published "PWR FLECHT.SEASET Unblocked Bundle, Forced and Gravity Reflood Task Data Report," NUREG/CR-1532. This two' volume document reports the result of the 17 x.17 unblocked bundle experiment.

.A meeting was held with NRR staff to discuss the drafted RIL T67 17 x 17 Reflood Heat Transfer. Comments _ from NRR are being incorporated into the final RIL.

2. - Advanced Two-Phase Flow Instrumentation for Power Plant Applications

- ORNL published the Quarterly Progress Report for October-December 1980, NUREG/CR-2007. It reports the THTF test results of the CE heated

' thermocouple -(HTC) -liquid level probe, the multi-element HTC probe

'and the ultrasonic probe.

3. ' BWR CCFL Refill /Reflood Program Lynn 30' SSTF separate effects tests in a BWR/6 configuration are about 2/3 complete. So far, the instrument line insulation problem seems to have been corrected. The PMG approved a somewhat reduced test matrix in an attempt to recover from delays caused by the insulation problem and overall funding constraints. The PMG also approved the' first-three BWR/6 transient tests so that a costly delay will_ not result while the PMG reviews the total test matrix.

These tests should start in early June.

Discussions with NRR, GE and EPRI about the technical merits of the 0

360 ' facility are continuing.

Semiscale Quick Look reports for tests S-UT-6 and S-UT-7 have been completed (5 percent cold leg break with and without UHI).

The tests confirmed previous PWR analyses that a 5 percent break was a worst case for the conditions tested, and that UHI provided greater inventory.

The discussion was pt.rt of a continuing series of meetings of OSP staff with economic reguhtory agencies to follow up on recomendations made in the Rogovin report with respect to possible-financial disincentives to safety.

During the meeting with the Governor and the Texas State Liaison Officer, Edward Vetter, issues discussed were the construction of nuclear power plants at South Texas and Commanche Peak, high and low level waste disposal, and the mission of the NRC Region IV office. Governor Clements emphasized the need for a permanent low-level waste burial site in Texas for Texas waste. He urged the NRC to publish l'
