EVENT DESCRIPTION

On 12/1/82 the suppression pool narrow range level'sightglass isolation valves 1CM039-45 were found in the locked open position by a shift engineer.

These valves are required to be maintained in the locked closed position per LAP 240-1.

The sightglasses had been valved in on 11/24/82 in order to check a questionable control room indication, and the equipment attendant performing the icvel reading left the valves in the locked open position according to his interpretation of instructions given by the unit NSO.

V.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

Since there was no significant amount of airborne activity in the primary containment during the time that the sightglasses were valved in and the unit was at power operation and since the sightglasses have demonstrated the ability to withstand the containment's peak accident pressure during the Unit 1 ILRT, the health and safety of the public was not affected as a result of this event.

VI.

CAUSE

The cause of this event is an apparent communication failure between the unit NSO and the equipment attendant performing the valving. The EA claims to have been instructed by the unit NSO to leave the sight-glass isolation valves open, while the NSO does not recall giving those instructions.

There exists no documentation to verify either case.

VII.

CORRECTIVE ACTION

Valves 1CM039 through ICM045 were immediately returned to the locked closed position on 12/1/82. Results of a pro investigation conducted by station management is being reviewed by shift personnel in tailgate training sessions (AIR 1-82-592).

In addition, an operating procedure is being drafted to provide a means of verification and documentation of final position of locked valves following temporary position changes (AIR 1-82-593).
