., n.I On 12/26/84, investigation was initiated to verify Containment integrity, it was suspected integrity might have been lost as containment internal pres-sure was at 10.2 psia and rising. As a result of this investigation, [RS-113]

the "A" Outside Recirculation Spray Pump casing drain valve was found open.

On 1/7/85, further investigation determined that the valve had been open from before 12/23/84 the date the plant changed from operating mode 5 to cper-8 ating mode 4.

This violated Technical Specification 3.6.1.1, which requires Containment integrity to be met prior to entry into mode 4.

The applicable action statement had been met, however, the reporting requirement of 10 CFR 50.72.b.ii was not met.

It was detennined that the cause of the valve being open was a breakduwn in communication regarding the use of the "Off-Log-Only" procedure for removing equipment from clearance for testing purposes.

This procedure is being re-eval-uated to establish tighter control of system configuration. The 10 CFR 50.72 reporting requirements were not met due to lack of recognition of the violation by station personnel. A training package for these personnel is being establish-ed to prevent a similar reporting failure.

Additional actions are being taken to further verify Contairvaent integrity af ter an extended outage or any unusual system configuration.

cr u.,nn On 12/23/84, Containment Vacuurn was drawn in preparation for entering Mode 4.

During the subsequent heatup, operators were given direction to check all penetra-ticns for inleakage due to the rising pressure in Containment. The investigation continued and on 12/24/84, it was noted that Containment internal pressure was at 10.2 psia and rising. An investigation was initiated to verify Containment integrity.

As a result of this investigation, [RS-113] the "A" Outside Recirculation Spray Pump casing drain valve was found open. The valve was immediately shut and the drain line capped. This was done to meet the action statement of T.S. 3.6.1.1.

At this time, a log entry was made as to the problem and the NRC Resident was notified infomally out of courtesy. At that time, there was no 10 CFR 50.72 report cade to the Nuclear Regulatory Commission, as would be discovered later. A station investigation identified the circumstances surrounding the valve position. This in-vestigation was essentially complete on 1/7/85.

At this time, it was also detemined by the NRC Resident Inspector that the event should have been reported under 10 CFR 50.72.

It was detemined that the valve was open due to a breakdown in communica-tion regarding the use of the "Off-Log-Only" procedure for removing equipnent from clearance for testing purposes. Before a clearance can be complete, the portion of the system under clearance must be restored to Nomal System Arrangement (NSA).

However, the "Off-Log-Only" procedure allows equipnent to be energized for testing purposes after maintenance without restoring the system to NSA if the testing does not require it. Af ter the testing is complete, the system must be returned to NSA and the clearance completed before the equipment can be returned to service.

In this case it was detemined not necessary to return [RS-113] to NSA when, on 12/13/84, while the plant was in mode 5 the "A" Outside Recirculation Spray Pump was signed "Off-Log-Only" for testing af ter replacing a seal package.

This decision was based on a review of the Outside Recirculation Spray Pump startup procedure which verifies the position of [RS-113] before starting the pump.

However, on 12/14/84, when the pump was tested, the entire procedure was not required to be p;rfomed to selectively test the repaired seal. Therefore, the section of the procedure which would have closed [RS-113], was not perfonned.

The valve would have been returned to NSA prior to removing the clearance, but, this was not completed prior to entering mode 4.

The plant startup check-list is bein escalation. g revised to require an equipnent clearance review prior to each mode Two other administrative controls that should have prevented this event also failed to identify the valve as open.

The Containment Integrity Veri-fication Test (OST 1.47.2) did not refert.nce [RS-113] and, has been revised to include this and any other valves which could cause a similar event. The valve checklist used to verify that the system was in NSA prior to startup was completed before the "A" Outside Recirculation Spray Pump clearance was hung. The "Of f-Log-Only" procedure is being re-evaluated to establish tighter control of system con-l figuration.

General training is being conducted for each shift crew that encompasses the causes and corrective actions pertaining to this event. Additional personal meet-ings are being conducted with the personnel directly involved in this incident, g,,
