HIGHLIGHTS OF WEEKLY INFORMATION REPORT .

WEEK ENDING FEBRUARY 6, 1987 FERMI-2 During calibration testing of feedwater (FW) flow on February 4,1987, the licensee discovered that the reactor was operating above the 200 percent power level at which plant operation is presently limited. The licensee has determined that this power excursion, which lasted several minutes, and peaked at 22.5 percent, occurred when a simulated FW flow signal irt the plant process computer isolated a flow ifmiter causing an increase in redirculation

, flow. Upon discovery, the simulated FW flow signal was removed and the power immediately dropped below 20 percent (to about 19.7 percent). A Deviation Event Report has been written by the licensee, and the requisite FW surveil-lance testing procedures are being reviewed.

River Bend Station _ p.

On Februaryy 3, 1987, the Texas Public Utility Commission granted a $39.9 million emergency rate relief to Gulf States Utilities (GSU).

In October, 1986 GSU informed the Securities and Exchange Commission that it would be unable to meet $65 million in interest and other payments by March 1987 and "may have to seek protection from its creditors under the backruptcy code" unless it received rate increases or external funding.

GSU's financial problems are tied to recovery of its investment in River Bend Station. GSU's service area includes central Louisiana and Eastern Texas. Prior to this rate action, neither Texas nor Louisiana rate making i-bodies had approved rate relief for River Bend. GSU has appealed to the Louisiana courts on emergency rate relief for River Bend. .

4 Access Authorization Policy Statement and Guidelines At the direction of the EDO, NMSS has sent a letter to the Chairman of the NUMARC Steering Committee requesting additional information concerning the use of temporary workers. The information is intended to assist NRC management in

resolving questions concerning screening requirements for unescorted access for temporary workers in situations other than cold shutdown.

Sequoyah Fuels Corporation, Gore, Oklahoma On February 3, 1987, representatives from Sequoyah Fuels Corporation (SFC) and NMSS met to discuss issues related to the Sequoyah Facility license. During the meeting, a, presentation was made on the status of the facility since the withdrawal of the Confirmation of Action Letter which allowed SFC to resume production operations. The licensee also discussed environmental monitoring, license format, and the UF 6to UF amendment 4 request.

Conference of Radiation Control Program Directors Frank Young, OSP, attended the winter Executive Board meeting of the Conference of Radiation Control Program Directors (CRCPD), held in Baton Rouge, Louisiana, February 1-5, 1987. At this meeting, the Board reaffirmed its desire to have the Atomic Energy Act amended to provide I for regulation of Naturally Occurring Radioactive Material (NARM) similar to that of byproduct material. The Board established a strategy committee that will work with NRC and the U. S. Environmental Protection Agency and expect to work through the National Governors' Association to seek such an amendment.

6 OFFICE OF' NUCLEAR REACTOR REGULATION 4 ~ ITEMS OF INTEREST Week Ending January 30, 1987-Diablo Canyon Nuclear Power Plant. Units 1 and 2 On January 28.-1987 the ASLB issued a Memorandum and Order on the subject:

3. Motion by Intervenors for summary disposition was denied.

Mothers for Peace and C.0.D.E.S. have withdrawn from the hearing; their conten-tions have bee'n dismissed-by the Board. Staff is continuing preparation of testimony on admitted Sierra Club contentions. Staff is also preparing SER Supplement on wet reracking aspects and supplement for environmental assess-ment on issue of generic environmental assessment as had been identified by Ninth Circuit Court.

Palo Verde Unit 1 The unit has been down since January 19, 1987 to investigate the source of 0.3 gpm leakage from the primary system to the secondary system. In a conference call with the licensee on January 30, 1987, the' staff was provided with the

following status of this ongoing investigation.

Visualexaminationofthesuspectedsteamgenerator(No.1)revealedthatan 1

outer row tube was leaking at the level of the lowest (first) "eggcrate". Eddy 4

current testing of about 1000 tubes has revealed four other tubes with indications

(wall thinning) also in the' outer rows and at the level of the first or second eggerate. Approximately 3000 tubes will be eddy current tested in this steam generator. Eddy current testing is also underway in the other steam generator (No. 2). To date (1/30/87), four tubes with indications have been found out of i several hundred tubes tested. These are also in the outer rows at the first and second eggerate but rotated about 180 degrees from the other steam generator.

Approximately 3000 tubes will be tested in this steam generator.

Cause of the tube leakage and wall thinning is not yet known, but flow induced vibration is suspected. Palo Verde Unit 1 is still in its first cycle of operation. The licensee will keep us informed as the investigation progresses.

Byron Station Unit 2

' On-January 30, 1987 the Commission voted 5-0 to give Byron 2 a full power license. - The license was issued later that day. On the next day, February 1, 1987, Byron 2 exceed 5% power.

Vennont Yankee' Nuclear Power Station The New England Coalition on Nuclear Polution and the States of Vermont and Massachusetts mailed petitions to intervene on the proposed Vermont Yankee spent fuel pool expansion.

ST. LUCIE PLANT UNIT NO. 1 The St. Lucie Plant, Unit No.1 is scheduled to be removed from service on February 7,1987 for refueling in preparation for Cycle 8 operation. The following major projects are scheduled to be accomplished during the outage.

Millstone Unit 2 On February 4,1987, Millstone Unit 2 identified the leak in Steam Generator No.1 that had caused a voluntary plant shutdown on January 29.

The defect, located in the " sludge pile region" of a tube, had caused a primary-to-secondary .2 to .5 gpm leak. The tube in question had been

! inspected during the Fall-Winter 1986 refueling outage. The licensee l is inspecting the ECT data for the tube in question in order to deter-l mine how the defect was missed.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR February 2 - 8, 1987

1. DEFUELING

- Defueling continued using the Airlift System through Thursda3 On Friday, defueling workers began probing the upper-core surface. The current top of the core contains a number of rock like structures of varying size from a few inches to approximately 2 feet. The GPU defueling crews are currently in a two-week outage for data acquisition.

2. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regJlatory limits, NRC requirements, and the City of Lancaster Agreement.

- TMI water effluents are sampled from the station (Units 1 and 2) discharge and analyzed by EPA. Gamma spectrum analyses of the seven

, daily composited samples for January 17 - 23, 1987 indicated no TMI-2 related radioactivity.

- EPA's ga'mma spectrum analysis of the NRC's TMI outdoor air sample for the period January 29 - February 5,1987 showed no reactor related radioactivity.

- The Lancaster water works intake composited seven daily samples January 18 - 24, 1987. The gamma spectrum analyzed by EPA showed no reactor related radioactivity.

3. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES

- Desludging of the Auxiliary Building Sump started during the week.

- Scabbling of the diesel generator basement was completed during the week.

- Scabbling of the Seal Injection Valve Room was terminated during the week. Modifications are being made to facilitate waste pickup.

- Flushing of the pressurizer spray line was completed.

- Scarifying of the Reactor Building basement walls began using a modified Remote Reconnaissance Vehicle (RRV).

5. MEETING The next meeting of the Advisory Panel for the Decontamination of Three Mile Island Unit 2 will be held on February 26,1987, at the Lancaster Council Chambers, Public Safety Building, 201 North Duke Street, Lancaster, PA. The NRC and GPU Nuclear staffs will respond to the Panel's questions on the licensee's proposed disposition of the accident-generated water and on the NRC staff's Draft Environmental Impact Statement on the disposal of accident-generated water. There will be an opportunity for public comment.

The meeting is presently scheduled to begin at 3:30 PM and will continue into the evening.

0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending February 6. 1987 FBI Criminal History Rule CRGR review of the FBI Criminal History Rule was conducted on February 5, 1987. Subsequent steps to publication include EDO approval, Commission approval, Rules and Records edit, and Federal Register publication. Delays at

any of these points would impact meeting the Federally mandated publication date of February 27, 1987.

University Research Reactor Workshop

! On February 2, 1987, Carl Withee of NMSS delivered the Safeguards keynote speech to the University Research Reactor Workshop at Lawrence Berkeley  ;

Laboratory. The purpose of the workshop was to help the National Research l Council obtain information for their report to DOE on recommended future i government policy with respect to university operated research reactors.

2 Weld Cracks in Type B Packaging Circumferential cracks were detected at the weld joint of the outer shell and base plate of the Model No. 1600 cask at Big Rock Point. The cask is a steel encased lead shielded packaging used primarily for shipment of irradiated waste materials. The cask in which the cracks were found, and one other, are owned by Chem-Neclear Systems, Inc. Chem-Nuclear Systems, Inc. has removed their casks from service until the matter is resolved. The General Electric Company owns four Model No. 1600 casks and has taken their casks out of service until all casks are inspected and no cracks are detected in any cask.

Electric Utilities Transportation Meeting The Electric Utilities Companies' Nuclear Transportation Group will hold a meeting in Washington, DC on February 5, 1987. NRC representatives have been invited to brief the group on matters relating to the transportation of spent fuel and low-level waste. Staff from NMSS, RES, and IE are scheduled to participate in the meeting.

Sequoyah Fuels Corporation, Gore, Oklahoma On February 3, 1987, representatives from Sequoyah Fuels Corporation (SFC) and NMSS met to discuss issues related to the Sequoyah Facility license. During the meeting, a presentation was made on the status of the facility since the withdrawal of the Confirmation of Action Letter which allowed SFC to resume production operations. The licensee also discussed environmental monitoring, license format, and the UF6to UF amendment 4

request.

Combustion Engineering, Windsor, Connecticut Representatives from Combustion Engineering (CE) and NRC met on January 28, 1987, to discuss two license amendment requests. The first request pertains to organizational changes, minimum qualifications of personnel, t .d the removal of Building 03 from the license. The second request is for modifications to the nuclear safety analysis for the storage and processing of SNM under controlled interspersed moderation.

UNC, Inc., Montville, Connecticut On February 4-5, 1987, UNC representatives met with NRC Region I and NMSS personnel to discuss issues on the license renewal application which need resolution. UNC will consider the issues and submit revised pages for the application.

Participants included UNC site and corporate personnel, NMSS, NRC Region I personnel, and NMSS consultant from ORAU. Soil and water contamination are still issues needing resolution. UNC will consider the NMSS suggestions to meet with state and local government officials to discuss the decommissioning status. UNC is awaiting DOE guidance on disposal of about 2500 drums of contaminated soil.

ORAU will attempt to provide all the survey data in the near future. UNC cor-porate officials will be involved in resolution of issues leading to ifcense termination.

Plant Risk Status Information Management (PRISIM) System i

l On January 28, 1987, a demonstration of the Plant Risk Status Infomation Management (PRISIM) system was given at the Region V resident inspectors 1 meeting in Walnut Creek, California. ThePRISIMsystems(developedorbeing  ;

developed for the ANO-1, Peach Bottom 2 and Surry nuclear plants) have '
capabilities to identify and prioritize inspection activities based on their l risk significance. The PRISIM program objectives, background and status were discussed, the preprocessed PRISIM infomation base was displayed, and use of the interactive PRISIM plant risk model was demonstrated. 2 NRC Employee Honored by Health Physics Society Robert E. Alexander, Chief of the NRC Safety Research Applications Branch in the Office of Nuclear Regulatory Research, has been elected President of the Health Physics Society, a professional society of more than 6,000 scientific personnel engaged in radiation protection and health effects activities throughout the US and abroad. His SRA Branch responsibilities include research and standards development in radiation protection and health effects. The HPS, founded in 1957, provides a comprehensive program for keeping its membership up to date, including formal courses, two technical meetings each year, monthly publication of a technical journal and a newsletter, and support for a certification program for health physicists conducted by American Board of Health Physics. The society is also active in the consensus standards development field; several ANSI standards developed by the HPS are used in the NRC regulatory program. A procram for HPS accreditation of NBS secondary calibration laboratories (for health physics instruments) is just starting.

Thus, the HPS is highly influential in all radiation protection and health effects aspects of NRC licensed activities and regulatory programs.

Mr. Alexander, who joined the AEC Regulatory staff in 1972, is the first HPS President to come from the AEC Regulatory or NRC staff.

10 CFR Part 50, Appendix J, " Leakage Rate Testing of Containments of Light-Water-Cooled Nuclear Power Plants" On October 29, 1986, the NRC announced in the Federal Register that it is proposing to revise 10 CFR Part 50, Appendix J " Leakage Rate Testing of Containments of Light-Water-Cooled Nuclear Power Plants." An invitation to coment was presented in the Federal Register with a public coment period to expire on January 26, 1987.

The Northeast Utilities requested a 2-month extension of the coment period, and the BWR Owners' Group and Boston Edison requested a 3-month extension.
These requests and similar expressions of desire were considered by the
staff, and it was agreed to extend the coment period to April 24, 1987.

The announcement to this effect appeared in the Federal Register on January 22, 1987.

ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING FEBRUARY 6, 1987 OSART Activity IP was advised on February 6 that Robert Burns, Vice President for Nuclear Operations for the FitzPatrick and Indian Point-3 Nuclear Power Stations, PASNY, can serve as the U.S. member on the OSART to the Caorso nuclear power plant (un 875 MW BWR) in Italy, now scheduled March 16-April 3,1987. The IAEA had requested a U.S. industry expert to review the area of p' int management, organization, and administration. Both INP0 and NRC participated in the search for the U.S. candidate.

Withdrawal of Assignment Application The National Research Council of the National Academy of Sciences advised IP in late January that the Korean government had withdrawn the application of Mr. Won Young Kim of the Korea Institute of Energy and Resources for a six-month assignment to NRC, DOE, or BMI to study how to perform site selection and investigat1ons for low- and intermediate-level waste repositories.

0FFICE OF STATE PROGRAMS ITEMS OF INTEREST WEEK ENDING FEBRUARY 6, 1987 Conference of Radiation Control Program Directors Frank Young, OSP, attended the winter Executive Board meeting of the Conference of Radiation Control Program Directors (CRCPD), held in Baton Rouge, Louisiana, February 1-5, 1987. At this meeting, the Board reaffirmed its desire to have the Atomic Energy Act amended to provide for regulation of Naturally Occurring Radioactive Material (NARM) similar to that of byproduct material. The Board established a strategy committee that will work with NRC and the U. S. Environmental Protection Agency and expect to work through the National Governors' Association to seek such an amendment.

II. SECY-86-366 - Final Rule on Licenses and Radiation Safety Requirements for Well Loquing (New Part 39)

The Commission, by a 5-0** vote, approved a final order on specific radiation safety requirements related to the use of licensed materials in well logging, subject to the following:

1. Because of the very important role of training for worker safety in well logging, priority should be given to the development of the training manual for well loggers which is scheduled for publication mid-1987. The manual should be prepared using clear text and illustrations to serve as a ready reference on radiation safety issues for well loggers similar to the training manual prepared for industrial radiographers (NUREG/BR-0024).
  • Section 201 of the Energy Reorganization Act, 42 U.S.C. $5841,  :

provides that action of the Commission shall be determined by a i

" majority vote of the members present." Commissioner Asselstine was not present when this item was affirmed. Accordingly, the formal vote of the Commission was 4-0 in favor of the decision.

Commissioner Asselstine, however, had previously indicated that he would approve this paper and had he been present he would have affirmed his prior vote.

n 2. Requirements for calibration of radiation survey instruments to an accuracy of +20 percent in 39.33 are inconsistent with accuracy requTrements in proposed Regulatory Guide OP-032-5 and other Division 8 Regulatory Guides. Although, the commission has no objection to the proposed requirement l

as it applies to well logging, it believes that calibration requirements for radiation survey instrumentation should be consistent in NRC regulations and guidance unless exceptions 3

4 are defined for specific applications.

3. In response to a public comment, the staff notes that NRC i regulations do not contain criteria for acceptable levels of residual contamination in soils. An OGC memo of October 29, 1986, on the subject rulemaking observes that this involves a broader agency problem. The Commission is aware that the staff is participating in an interagency task force headed by EPA to define limits of release for unre-stricted use. It is also aware of other NRC initiatives to define de minimus and levels below regulatory concern. As a broader issue the Commission asks that the EDO advise it as to how these activities are related and will result in consistent release limits for all NRC licensed activities.

it involves a broader agency issue.

! 4. The regulatory guide should specify qualifications of

individuals performing training and testing. They should  ;

be experienced logging supervisors.

5. The changes on the attached mark-up should be incorporated.
