- FEB 121987 UPDATE A0 86-3, SEQUOYAH FUELS CORPORATION RUPTURE OF UF 6 CYLINDER AND RELEASE OF GASES On November 14, 1986, subsequent to withdrawal of the Confirmation of Action Letter by Region IV, Sequoyah Fuels Corporation (SFC) began production opera-j tions at its UF Conversion Facility in Gore, Oklahoma. On December 15, 1986, 6
the first UF cylinder (14 ton) began filling and was completed on December 17.
6 The remodeled steam chest was successfully used on December 22, to heat an out-of-specification (high chromium level) UF6 cylinder. By late December, SFC was operating at a producticn rate of 300 MTV per month. The independent oversight organization and NRC oversight team maintained 24-hour surveillance throughout the month. The independent oversight organization released two reports to SFC staff describing its activities, findings, and recommendations. No significant safety concerns were noted during December, and SFC continued to operate without incident.
