ABNORMAL OCCURRENCE REPORT TO CONGRESS FOR THIRD QUARTER CY 1986 NMSS has reviewed the draft abnormal occurrence report forwarded by your '

memorandum of January 14, 1987, and has the following comments.

Page 14, A0 86-18. The writeup should explain that the seeds used in this ,

incident at the University of Cincinnati were of higher activity (approximately (

i- 40 mci / seed) than those normally implanted. In most cases, licensees permanently implant lower activity seeds. In this case, the University of Cincinnati used higher activity seeds as temporary sources, and intended to recover and reuse them because of the high cost of these high activity seeds.

Page 15, line 1. Rad is a measure of absorbed dose, not exposure.

Page 37, A0 86-3, next-to-last paragraph. Amend to read as follows:

On November 14, 1986, the NRC authorized Sequoyah Fuels Corporation to restart production activities. NRC provided 24-hour oversight of restart and production activities and continued the requirement for an Independent Oversight . Team (IOT) to provide a third-party examination of operations.

Both of these oversight activities have continued as the facility resumed production.

Page 38, A0 86-7. More information is needed to update this item. NRC proposed a civil penalty of $10,500. Did the licensee pay it? Was any part mitigated? Has there been a subsequent inspection? If so, what were the results?
