HIGHLIGHTS OF WEEKLY INFORMATION REPORT

' WEEK ENDING APRIL 4, 1986

' Shoreham Eneroency Plannino The ASLAB issued on Wednesday, March 26, 1986 its decision on appeals of those aspects of the Shoreham Emergency Plannin Proceeding not related to the E-Plan drill or the " legal authort " issue. he Appeal Board remanded four issues, related to 1) plume E size, 2) role 4

conflict of schoolbus drivers, 3) evacuation plans for hospitals, and

4) use of the Nassau County Coliseum back to the Licensing Board for further action. However, it ordered the Board to not proceed until ordered to do so by the Commission, insofar as the Commission has the legal authority' decision before it. The Appeal Board also deferred ruling cn LILCo s non-legal authority appeals until the Commission rules on the legal authority issue.._

J TVA Employee Concern Program Related to QTC Records The contractual agreements between TVA and Quality Technology Company j (QTC) have been reduced to a state that the NRC is exercising its option to access the QTC records for copying. Staff representatives 4 are now at the Watts Bar site and, at the request of TVA, QTC is providing the clerical help to assist the NRC staff in copying of the files in accordance with the NRC order dated January 30 1986. We expecttohavetwosetsoftheserecordsinWashingtonInabouttwo weeks and will maintain a third set, under exclusive NRC control, at the Watts Bar site.

According to QTC, these files contain technical and Intimidation &

Harassment issues which they have not passed to TVA with sufficient

information to investigate the issue, due to risk of breeching

' confidentiality agreements. These issues may have safety related implications which are of interest to the NRC.

The staff is developing the program for review of these records.

NFS-Erwin On March 30, 1986, the strike at NFS-Erwin was settled. The company offer was accepted by the OCAW union workers by a four to one vote. NMSS and IE staffs held a joint meeting with NFS corporate management on March 31 concerning their plans to resume full operations. NFS will submit an information document to NMSS concerning their plans for bringing back the union employees i and performing essential retraining and requalifying before resuming full operations.

Title:

" Shuttle Bus Service"

Description:

The contractor shall provide regularly scheduled and extra shuttle services required to transport passengers, intra-agency mail and/or government-controlled materials / equipment to buildings occupied by NRC employees within the Montgomery County, Maryland Metropolitan Area.

ADMINISTRATIVE MATTER On April 1, 1986, a protest was filed with the Contracting Officer by Hirt Telecom Company (Hirt) under Solicitation No. RS-ADM-86-254 for cable installation. Hirt protests the NRC's decision to reject its bid as nonresponsive and requests that the bid be reconsidered for award. Award is being withheld pending disposition of the protest.

DIVISION OF ORGANIZATION AND PERSONNEL 4 ITEMS OF INTEREST Week Ending April 4, 1985 Interim Agreement with NTEU l

On April 3,1986, representatives of management and of the National Treasury Employees Union (NTEU) initialed an Interim Agreement. This agreement covers 48 of the 53 articles which have been subject to negotiation since June 10, 1985. -

We expect that the Interim Agreement can be made effective in early summer, following publication of the document and training for supervisors and Union Stewards.

The parties will soon begin negotiating the performance appraisal process.

New regulations governing performance appraisals were recently published by the Office of Personnel Management.

Substantive issues which have been the subject of negotiation but on which agreement has not been reached will be resolved by the Federal Labor Rela-tions Authority (FLRA) or the Federal Services Impasse Panel (FSIP). The FLRA has been asked to determine whether salaries and certain reduction-in-force rights may be negotiated. The FSIP will be resolving impasses re-garding hours of work.

A new comprehen:,ive Agreement will be put into effect when issues have been resolved by third parties and when negotiations have been completed. This will probably-not occur until the fall of 1986 or early 1987.

Shoreham Emergency Planning The ASLAB issued on Wednesday, March 26, 1986 its' decision on appeals of those aspects of the Shoreham Emergency Planning" Proceeding not related to the E-Plan drill or the " legal authority issue. The Appeal {

San Onofre 2 At 21:50 pst on March 26, 1986, while in Mode 5, San Onofre 2

' experienced a loss of both trains of shutdown cooling. The plant is shut down for refueling, and the RCS water level was being lowered to facilitate RCP seal replacement. Due to poorly calibrated water level instruments, the water level was allowed to go 3" below the planned level (the middle of the hot leg). This resulted in vortexing, air entrainment, and loss of shutdown cooling pus) (LPSI) suction. The

. water level was raised, the air vented, and stutdown cooling re-established within one hour and 15 minutes.

Haddam Neck Plant On March 26, 1986, Connecticut Yankee Atomic Power Company (CYAPCo) notified the NRC that during the performance of LOCA analysws for the Integrated Safety Assessment Program, a small range of break sizes in one loop of the reactor coolant system (RCS) was identified for which

-safety injection flow in the high pressure recirculation mode may be ,

insufficient to prevent core uncovery absent modification of facility

. operating procedures and/or system realignments. The specific break i sizes and locations involved are breaks of 0.045 ft to .02 ft in the cold leg reactor coolant pump discharge line for RCS loop 2 and breaks i less than .045 ft in the loop 2 charging line. The frequency of these '

breaks is estimated to be 8 X 10/ year. CYAPC0 is investigating corrective measures and will providing the specific details of the analyses and the proposed corrective measures in about a week. The ,

plant is continuing a planned refueling outage which began on January 4 of this year.

NRR will address the issue with the Full Committee April 10, 1986.

ENCLOSURE B APR 4 195 l

Millstone 3 Millstone 3 is currently shutdown for a one week scheduled maintenance outage.

The licensee performed the power coefficient test and load swing test at 75% power on Saturday, March 29. The large load reduction test was performed on Sunday, March 30, 1986. The unit went from 870 mWe in 20 seconds. During this test the atmospheric relief valves and turbine bypass valves operated as expected.

On Monday morning the licensee performed the MSIV closure test.

Following the MSIV closure test at 20% power the reactor remained on line at about 1% power. With the turbine on the turning gear, power was increased to 20% with the turbine bypass valves.

The loss of power test was performed at 1631 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.205955e-4 months <br /> from 20% power. By 1833 hours0.0212 days <br />0.509 hours <br />0.00303 weeks <br />6.974565e-4 months <br /> the buses were deenergized, the DG were operating, the turbine driven aux feed pump was operating and the reactor was being cooled by natural circulation. Following the successful completion of these tests the Unit was then shutdown for the scheduled maintenance outage.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT WEEK ENDING MARCH 28, 1986

1. DEFUELING

- As of. April 4, 1986, 37' debris canisters have been transferred from the reactor vessel to pre-shipment storage, net weight 43,800 lbs.

- .The program for resolution of the biologic growth in the Reactor CoolantSystem(RCS) continues.

- .The Temporary Reactor Vessel Filtration System, a swiming pool type (diatomaceous earth) filter, continues-to be used for RCS filtering.

Additional swimming pool filter capacity, pending resolution of the biologic growth problem, is being planned.

2. PLANT STATUS

- The reactor remains in long term cold shutdown, vented to atmosphere. Core cooling is by natural heat loss to ambient building atmosphere. The average of the incore thermocouple readings is 80*F. The reactor coolant temperature is 71*F.

- The airborne radioactivity on the defueling platform is about 9 E-8 uCi/cc Tritium and 3 E-11 uti/cc particulates, predominately Cesium-137. The platform is mounted above the modified internals indexing fixture which is mounted on the reactor vessel flange.

These provide water coverage of 151 feet over the core region. This water level is about 5 feet over the top of any debris canisters in the reactor vessel. .

3. WASTE MANAGEMENT.

The Submerged Demineralizer System (SDS) completed processing batch 130 from the "C" Reactor Coolant Bleed Tank (RCBT) to the monitor tanks then to the "A" RCBT. SDS total processed to date is 3,885,859 gallons.

- EPICOR II was in temporary shutdown this week. EPICOR II total processed to date is 2,854,371.

4.. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results ,

show TMI site liquid effluents to be in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.

- TMI water samples taken by EPA at the plant discharge (includes ,

TMI-1 and TMI-2) to the river consisted of 14 daily composite I

samples taken from March 8 - 22, 1986. A gamma scan detected no reactor related activity.

- The Lancaster water sample taken at the water works intake and analyzed by EPA consisted of a seven day composited sample taken from March 16 - 22, 1986. A gama scan detected no reactor related radioactivity.

APR 4 1986

- The NRC outdoor airborne particulate sampler at the TMI site collected a sample between March 26 and April 3, 1986. No reactor related radioactivity was detected. Analysis showed Iodine-131 and Cesium-137 concentrations to be less than the lower limits of detectability.

5. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending April 4, 1986 Spent Fuel Storage On March 28, 1986, the staff issued a letter of approval with a safety evaluation report for the Topical Report for the NUTECH Horizontal Modular System for Irradiated Fuel (NUH-001, Rev. 1). This design is referenced by Carolina Power and Light Company P(C.&L) in its application for an independent spent fuel storage installation (ISFSI) at its H. B. Robinson Steam Electric Plant site.

On March 31, 1986, the staff issued its Environmental Assessment and Finding

~o f No Significant Impact for the H. B. Robinson ISFSI license application submitted by CP&L. The application requests interim storage of up to 56 spent assemblies from the H. B. Robinson Steam Electric Plant Unit 2. Seven assemblies are stored in an inert atmosphere inside a stainless steel canister which provides confinement, shielding, criticality control and heat removal.

The spent fuel is loaded into the canister at the reactor. The canister is then transported onsite to the ISFSI inside an IF 300 shipping cask. The canister is then placed inside a concrete horizontal storage module that provides additional shielding. A total of eight concrete modules would be installed under the requested license. The first three modules are part of a cooperative demonstration program with DOE and EPRI.

Visit by Gun Bae Lee, Head, Site and Environment Department, Korea Advanced Energy Research Institute On March 24, 1986, members of FCMS and NMSS met with Mr. Lee to discuss siting criteria for a uranium fuel fabrication facility.

Sequoyah UF; Release-Health Effects Report Shortly after the January 4,1986, accident at Sequoyah fuels Corporation's (SFC) facility in Gore, Oklahoma, the NRC formed an Ad Hoc Interagency Task Force to evaluate the onsite and offsite health effects of the release which exceeded 29,000 pounds of Uranium Hexafluoride (UF 6 ) into the environment.

The Task Force was comprised of staff members from the NRC, EPA, HHS, USDA, LLNL, ORNL, and the University of Rochester. The technical report prepared by the Task Force, which assesses the public health impact of this accident has been released to the public.

Category I Export A Category I export shipment containing material authorized by five export licenses which originated at the Department of Energy Enrichment Plant, Piketon, Ohio has arrived at its destination in the Federal Republic of Germany. The international leg of this transfer was accomplished by Luftwaffe aircraft (German Air Force) crewed by German Air Force personnel.

Review of Regionalized Safeguards Programs The annual assessment visits to Regions I, II and IV were conducted in the last two weeks of February 1986. The draft reports on the annual assessments for all Regions were sent to each R~egional safeguards branch chief for review and comments on March 27, 1986.

RER (Safeguards Regulatory Effectiveness Review)

There will be an RER team at the Duane Arnold Energy Center, Iowa, the week of March 31 through April 4, 1986.

NFS-Erwin On March 30, 1986, the strike at hFS-Erwin was settled. The company offer was accepted by the OCAW union workers by a four to one vote. NMSS and IE staffs held a joint meeting with NFS corporate management on March 31 concerning their plans to resume full operations. NFS will submit an information document to NMSS concerning their plans for bringing back the union employees and performing essential retraining and requalifying before resuming fu l operations.

f. Emergency Preparedness (1) Members of the Emergency Preparedness Branch have participated on the Region inspection teams for the Arkansas and Limerick emergency plan exercises.

(2) Emergency preparedness exercise reports provided by FEMA for Wolf Creek and Cooper were forwarded by IE to Region IV for appropriate action. For Wolf Creek, the corrective action for a deficiency concerning the registration and radiological monitoring of evacuees is scheduled to be demonstrated at an upcoming remedial exercise. For Cooper, FEMA stated that there were four deficiencies identified in Nebraska and two in Missouri. Five of the deficiencies have been corrected and corrective action for the remaining deficiency in Missouri concerning the alert and notification system is scheduled for June 1986.

(3) FEMA has provided their evaluation of the alert and notification system for Ginna, which was found satisfactory.

g. Performance Indicators The interoffice group on performance indicators met Tuesday, April 1, 1986 to discuss the first draft of the Commission Paper and set the basic direction for the program.
h. ACRS Briefings On April 9,1986, members of IE, NRR, and Region II staff will brief the ACRS Subcommittee on reactor operations and on April 10, 1986 the full ACRS regarding recent significant operating events at nuclear power plants.

Items of Interest Week Ending April 4, 1986 SCDAP/RELAPS The SCDAP/RELAPS code is a system code which analyzes recovered accidents in the reactor coolant system (RCS) of a nuclear reactor. It calculates core uncovery, core melt, fission product and hydrogen release, and core reflood and recovery with no vessel failure.

The SCDAP/RELAP5 code is currently under assessment and model validation by comparing calculations with severe fuel damage experiments. A copy of the code was recently sent to eleven foreign partners of the NRC-sponsored Severe ,

Fuel Damage and Source Term Research Program. User comments from those foreign partners will be used to improve the code.

Simplified RELAPS/ MOD 2 Decks for the Nuclear Plant Analyzer The RELAP5/M002 code is used to analyze thermal hydraulic phenomena occurring in the reactor system when a system transient occurs. The Nuclear Plant Analyzer i (NPA) provides interactive controls with a variety of user-friendly features and colorgraphics output. INEL has completed two simplified RELAP5/M002 decks for the Oconee-1 and H. B. Robinson plants for the NPA use. This permits NRC staff and its contractors to achieve "real-time" computational speeds in the NPA using the INEL computer system to simulate an operational system transient such as a loss of offsite power or an accident such as a medium size steam line break.

California Earthquakes of March 29 and March 31, 1986 On Saturday, March 29, 1986, at 9:24 a.m. local time a small earthquake of magnitude 4 on the Richter Scale occurred about 5 miles north of Oakland, 4 miles east of Berkeley, California, on the Hayward Fault. The. event was felt throughout the San Francisco Bay area but no damage was reported.

Another earthquake, with no apparent relationship, magnitude 5.3, occurred on ~'

, Monday, March 31,1986, at 3:56 a.m. local time about 15 miles east-southeast of Fremont. California. NRC Region V contacted Rancho Seco (the nearest NPP

i - about 60 miles away), University of California - Berkeley, Aerotest, and  :

j- Valecitos (non-power reactors), and they reported no damage. The only effects reported so far other than " felt reports" are power failures in San Jose and i

. . objects. falling from shelves around the Bay area. The U.S. Geological Survey  ;

4 has reported that since the main shock, it has recorded several hundred after- i e shocks, at least 3 of which were over magnitude 3. The seismicity that began i 4

on March 31 is reported not to be directly related to either the Hayward or-  !

Calaveras Faults which pass through the region. The seismicity is occurring on a fault that is not previously known to be seismically active.

These recent earthquakes are described to highlight the uncertainties that exist in the geologic sciences even for California whose seismicity is i' F reasonably well understood via the theory of geologic plates. 3 RES RULEMAKING ACTIVITIES General Requirements for Decommissioning Nuclear Facilities (Parts 30, 40, 50, 51, 70, 71) (PRM-50-22)

The proposal would provide more specific guidance on decommissioning criteria for production and utilization facility licensees and byproduct, source, and special nuclear material licenses. This action is intended to protect public health and safety and to provide the applicant or licensee with appropriate regulatory guidance for implementing and accomplishing nuclear facility decom-missioning.

Responses to approximately 90 percent of the public comments are completed and at CRESS, with the goal of issuing the draf t staff responses to comments for l division review by April 15, 1986, as per project schedule. The title of the rule has been changed to " General Requirements for the Decommissioning of Nuclear Facilities" to comply with the suggestion of the RES Independent Review Board that the title more accurately reflect the rule content.

Primary Reactor Containment Leakage Testing for Water-Cooled Power Reactors (Part 50) (Appendix J)

L The proposed rule would update and revise the1973 criteria for preoperational and periodic pressure testing for leakage of primary and secondary containment boundaries of water-cooled power reactors. The revision is needed to reflect l

current regulatory practice and current technology which are no longer being
