4 HIGHLIGHTS OF HEEKLY INFORMATION REPORT WEEK ENDING JULY 18, 1986 Seabrookhit1 New England Coalition on Nuclear Pollution, Massachusetts Attorney General's Office and Seacoast Anti Pollution League have filed motions opposing the Applicant's motion for an operating license authorizing low power operation.

On a matter potentially affecting the full power license, FEMA has filed a motion with the Board, for changing the hearing schedule on emergency planning contentions from August 4, 1986, to October 20, 1986. FEMA noted that the

December 6, 1985 New Hampshire Emergency Plan (for which hearings were scheduled to begin August 4, 1986) has been substantially altered in a June 3, 1985, revision and will be again substantially altered in a revision to be submitted in August 1986. Therefore, FEMA considers litigating the December 1985 version

.maybe counter-productive to the resolution of the off-site emergency preparedness -

issues at Seabrook.

SeQuoyah Fuels Corporation, Gore. Oklahoma (SFC)

On July 14, 1986, representatives of NMSS and IE met with representatives of Sequoyah Fuels Corporation (SFC) to provide them with questions and comments on the information submitted relative to SFC's request for authorization to resume production of uranium hexafluoride. The revision of the Emergency Contingency Plan was also discussed. No major issues of contention were identified.

At the July 14 meeting, SFC indicated that there are some ancillary systems, not associated with the production of UF , that need to be operated irrespectiveofthestaffdecisiononth$restartauthorization. These systems include the operation of solvent extraction, evaporation, and raffinate treatment to handle liquid waste that has accumulated onsite since the accident. SFC was informed that they should make the request in writing and provide a justification for operation of this equipment.

Issuance of Renewal of the License to Possess-But-Not-Operate the Reactor Facility on the N.S. Savannah On July 15, 1986, the Office of Nuclear Reactor Regulation renewed license No. NS-1 for the continued possession-only of the nuclea~r reactor facility on the N.S. Savannah, which is located at Patriot's Point, South Carolina.

The license for possession of the N.S. Savannah facility is held jointly by the U. S. Maritime Administration and the Patriot's Point Development Authority.

The nuclear-powered ship was initially operated in 1961, and after several years and world-wide travel was shut down and defueled in 1970. The ship is moored at Patriot's Point, near Charleston, SC, where it is open to the public as part of an historical museum. The license for possession-only was renewed

'until July 15, 1996.

Comanche Peak The applicants have filed a Form 8-K with the Securities and Exchange Cennission explaining further delay in the corrective action program for Comanche Peak.

The process including hearings cannot now be completed in time for peak summer load in 1988 for Unit 1.

Millstone 3 Shift Advisors In a letter dated June 19, 1986, Northeast Utilities advised the staff that in view of the hot operating experience level now attained by regularly assigned shift personnel, shift advisors would be removed as of July 16, 1986. The staff has approved the licensees' plan to remove the shift advisors in a letter dated July 3, 1986.

San Onofre 1 Restart On July 10, 1986 the ED0 signed a Commission Information Paper informing the Commissioners that staff and licensee actions required for restart of San Onofre 1 are complete. The facility has been shut down for refueling and modifications since the November 21, 1985 loss-of-power / water hammer event.

The Region V Administrator is prepared to lift the Confirmatory Action Letter which requires NRC approval prior to restart. The plant is currently in Mode 4; scheduled to enter Mode 2 on July 12 and to synchronize to the grid on July 16, 1986.

JULY 18, 1986 ENCLOSURE B

, Shearon Harris Unit 1 Carolina Power and Light informed the staff that the fuel load date has slipped from late July to late August or early September,1986. Consnercial operation is scheduled for early 1987.

Fermi-2 On July 9,1986, Congressman Dingell issued a press release regarding Fenni-2

! in response to a 5 to 3 vote by the Monroe County Commissioners requesting Congressman Dingell to investigate the " Mark I Reactor" (sic). Specifically, Congressman Dingell states that "... I will be asking the NRC for a full report on its present actions and on current problems at the FERMI II reactor Nine Mile Point 2 '

- By a letter dated July 8,1986, Mr. William J. Donlon, Presideat of Niagara Mohawk Power Corporation, stated that Mr. B. G. Hooten will no longer be responsible for daily activities associated with Nine Mile Point Unit 2.

Senior responsibility for nuclear activities associated with Nine Mile Point

{ Unit 2 is now vested in Mr. C. V. (Chuck) Mangan, Senior Vice President. Mr.

J. Perry has been hired to permanently fill the position of Senior Vice

President of Quality Assurance.

! Catawba Unit 2 i

l On July 11, 1986 Duke Power Company's personnel successfully completed the loss of control room test for Catawba Unit 2. The reactor was tripped from j

- about 25% power and controls were transferred to the auxiliary shutdown panels.

The test was witnessed by the Catawba Project Manager (who was at the time participating in a Region II special inspection regarding Generic Letter 83-28) and a special startup testing team from Region II.

It should be noted that, on June 27, 1986, this test caused a severe de-

pressurization and cooldown transient because of multiple deficiencies in the i remote shutdown capability. Subsequently, Duke Power Company corrected the l procedures and the mislabeled controls at the shutdown panels and provided

' additional training to the operators before rerunning the test on July 11, 1986.

Point Beach Nuclear Plant Units 1 and 2 i

On July 14, 1986 a Director's Decision pursuant to 10 CFR 2.206 was issued regarding alleged equipment qualification deficiencies at the Point Beach Nuclear Plant, Units 1 and 2. Conments relating to these deficiencies were provided as comments to the proposed rule 10 CFR 50.49 by Stephen Proudman of

informad Mr. Proudman by letter dated January 4,1985 that the staff would consider comerts on the proposed rule relating to equipment qualification deficiencies at specific plants as petitions filed pursuant to 10 CFR 2.206.

l DAILY HIGHLIGHT - FOR THE THIRD TIME IN 1986 AN EARTHQUAKE WAS FELT AT OCONEE NUCLEAR STATION, UNITS NOS. 1, 2 AND 3 On July 14,1986 at 6:30pm, an earthquake was felt at the Oconee Nuclear Station, Units 1, 2 and 3 for the third time this year, and its magnitude was estimated at 2.2 on the Richter scale and lasted for 97 seconds. The -

earthquake did not damage the nuclear units, set-off any alanns, or trigger

. any of the strong motion instruments. However, the operators state that it '

was felt in the plant.

Previous earthquakes were felt on June 11, 1986 with a magnitude of 2.8 and on February 13, 1986 at 3.5.

The units did not trip and continue to operate.

Daily Highlight On July 15,1986, at 1:51 P.4 PDT, San Onofre Nuclear Generating Station Unit 1 achieved criticality in its startup following the loss of power / water hamer event that occurred on November 21, 1985. The licensee, Southern California Edison Company, expects the unit to be on-line by July 18, 1986.

Haddam Neck -- Unplanned Shutdown for Steam Generator Tube Plugging On July 16, 1986, Connecticut Yankee Atomic Power Company notified NPC that the Haddam Neck Plant was placed in cold shutdown to plug approxi-mately 100 additional steam generator tubes. During recent eddy current inspections, CYAPC0 detected undefined eddy current signals in the tube expansion region of the steam generator tube sheet. CYAPC0's ongoing analysis of these signals has indicated that 402 tube ends have approxi-mately 50% or greater through-wall cracks. After discussions with Westinghouse, CYAPC0 has calculated that adequate safety margins exist, except for those tubes where the cracks are in the upper one-inch of of the rolled expansion area of the tube end and are greater than about 50% throughwall. CYAPC0 has decided to plug the tubes with such indications. It is expected that plugging of these tubes will take two to three weeks.

CYAPC0 is continuing to perform an evaluation to identify the acceptance criteria for eddy current signals in the tubesheet region of the steam generator.

This analysis is to be completed by September 30, 1986.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR JULY 14 - JULY 20,1986

1. DEFUELING

- During the report period, the licensee continued core boring to acquire core stratification samples in positions G-12, K-9, D-8, and K-6. Encased stratification samples were removed and placed in defueling canisters for eventual shipment to Idaho National Energy Laboratories. An extended drilling operation was performed in the K-9 position on July 16, 1986 to characterize core debris found on

the lower reactor vessel head. Video inspections were made through the access ports left by the drilling operations. The video tapes from these examinations are presently being reviewed and analyzed. .

The licensee is re-indexing the drilling rig over core position D-4.

2. SHIPMENT The first fuel shipping cask left the site by train during this report period. Prior to release, the NRC site staff verified that the shipment was in compliance with NRC and DOT radiological requirements.
3. PLANT STATUS The reactor remains in long term cold shutdown, vented to the atmos-phere. Core cooling is by natural heat loss to ambient building atmosphere. The average incore thermocouple reading is 81 F.

The airborne radioactivity on the defueling platform is about 5.0 E-7 uCi/cc Tritium and 2.9 E-11 uCi/cc particulates, predominately Cesium-137 and Strontium-90. The platform is mounted above the modified internals indexing fixture which is mounted over the reactor vessel.

These provide about 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters.

4. WASTE MANAGEMENT The Submerged Demineralizer System (SDS) began processing batch ,

S-135 from the neutralizer tank. 1 EPICOR II completed processing batch 300, 10,668 gallons from the I monitor tanks, and batch 301, 1,296 gallons from the Chemical Cleaning Building sump. The system began processing batch 302.

Total volume processed through SDS to date is 4,084,856 gallons, and the total volume processed E?ICOR II to date is 3,031,926 gallons, i

5. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

- The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken June 29 - July 5, 1986. A gamma scan detected no activity that could be attributed to TMI-2.

- TMI water samples taken by EPA at the plant discharge (includes

Units 1 and 2) to the river consisted of seven daily composite samples taken from June 28 - July 7, 1986. A gamma scan detected no activity that could be attributed to TMI-2. .

- The EPA analysis of the NRC outdoor air sample for the period ,

SFC was informed that they should make the request in writing and provide a justification for operation of this equipment.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending July 19, 1986 2D/3D Through ORNL and INEL, RES furnished some advanced two phase flow instruments to the German Upper Plenum Test Facility (UPTF) in return for test data. Some of these instruments have degraded in the course of the -

extensive facility shakedown testing which started over one year ago.

The degraded instruments include the upper tie-plate drag bodies which, combined with turbine flowmeters, measure the mass finw rates across the upper plenum and core interface, and loop drag disks which, combined with gamma densitometers, measure mass flow rates in the hot legs and cold legs. RES is currently looking into causes and formulating corrective actions. As part of this effort, RES is arranging a special review group to review the instrument problems to make sure that everything that 4;gg*

. should be done is being done so that these instrumentation problems will not cause any significant loss of data or cause any major impact on the UPTF testing schedule.

International Code Assessment and Applications Program (ICAP)

ICAP program meetings were held in Erlangen, F. R. Germany on 9-13 June, 1986. The first 21 days were devoted to a Specialist Meeting for code users, followed by a Technical Program Group meeting. The purpose of ICAP is to provide a quantitative assessment of the thermal hydraulic systems code TRAC-PWR, RELAP5. and TRAC-BWR. The NRC is obtaining 3

assessment reuslts through bilateral agreements with a number of different countries. Presentations of assessment results were made by representatives from Finland, Italy, Sweden, Spain, Switzerland, United Kingdom, and United States. There were 27 presentations covering separate effects experiments, integral tests, and plant applications.

The next ICAP meeting will be held on November 3-5, 1986.

2 Status of Bilateral Agreement on Thermal Hydraulics with Swiss Federal l Institute for Reactor Research (EIR)

A program review was held with EIR on 16-17 June, 1986. The current bilateral agreement between NRC and EIR was concluded in May, 1984 and has a duration of 4 years. It calls for 28 assessment studies using RELAP5 and TRAC-BWR. EIR has used RELAP5/ MOD 2 to calculate LOFT small break LOCA test SB-3 and large break LOCA tests LB-1 and L2-6. The calculation of SB-3 was greatly improved over a similar calculation using RELAP5/M001 in terms of conservation of mass and prediction of stratified flow in the hot leg and reflux condensation.

EIR has performed heat transfer experiments in the NEPTUN facility, which has 32 heater rods with similar geometry in LOFT including the same length (6 feet). The experiments are at low pressure under conditions of ,,

boiloff or reflood and at power levels characteristic of deca.v heat. -

Parameters varied were pressure, temperature, mass flow rate, and inlet subcooling. The experimental program is completed. These results are being;usedtoassessRELAP5/M002andTRAC-BWR.

NUREG-0956 A briefing was held on July 9, 1986 to inform the Commissioners of the status of NUREG-0956. The content of the final document, recent progress

in its preparation, and reviews of a draft copy by the national j laboratories most involved in the source term research were discussed.

In addition, future activities of the Office of Research in the area of source terms and severe accidents were reviewed.

Recent Earthquakes in the Eastern United States i

On July 11, 9186 at 8:26 a.m. a small earthquake, about 3.7 on the Richter magnitude scale, occurred in eastern Tennessee, approximately 10 miles east of Chattanooga. It was felt over a three county area including personnel on site at the Sequoyah NPP. The facility operators are checking a remotely recorded strong motion instrument for possible records. No damage was reported on site. No structural damage was reported in the immediate vicinity of the epicenter.

! On July 12, 1986 at 2:20 a.m., a moderate earthquake, about 4.2 on the I

Richter magnitude scale, occurred in western central Ohio within a few miles of the village of St. Mary's. This event was widely felt in Ohio and Indiana, with additional felt reports from Michighn and Kentucky. A polling of regional NPPs by the NRC Duty Officer showed that the event had not been felt at the NPPs. The earthquake had the se.me location as

! two microearthquakes of magnitude 1.4 and 1.7 on March 10, 1985. There had been no aftershocks in the first 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> after this event larger than the local minimum detectable magnitude 1.4.

to provide guidance to decisionmakers as to the appropriate role of information on core melt frequency, containment performance, source terms, and risk in making severe accident decisions.

Operational Safety Reliability Research The Operational Safety Reliability Research project includes case studies of five utilities to help identify reliability techniques that utilities have found to be effective and to identify attributes of successful reliability programs.

The U. S. Court of Appeals for the D.C. Circuit issued a decision on July 11, 1986, in the financial qualifications rule case (New Enoland Coalition on Nuclear Pollution, et al. v. NRC; D.C. Cir. No. 84-1514). The decision upholds the Commission's rule which exempts from staff review and adjudication the financial qualifications of regulated utility companies (and of utilities that set their own rates) applying for power reactor operating licenses.

The D.C. Circuit also dismissed a petition by the Callaway intervenors to reopen the OL hearing record on the financial qualifications issue. The i Callaway case (Coalition for the Environment. St. Louis Reaion. et al. v.

NRC. et al.; D.C. Cir. No 84-1313) was considered by the Court concurrently' with the financial qualifications rule case. The three-judge panel _ voted unanimously to uphold NRC's position in both cases.

Southeast Low-Level Waste Compact The Board of the Southeast Low-Level Waste Compact recessed its meeting in

. Columbia, South Carolina, Monday, July 14, 1986, without having selected the next host State. The Compact Commission requested its contractor to review further the data it collected and report back to the Commission in the next few months.

Engineering Evaluation Report, AE0D/E608 entitled, "Re-Examination of Water Hammer Occurrences," was issued on July 14, 1986. The study was prompted by the water hammer occurrence at San Onofre Unit 1 on November 21, 1985, as was intended to support the Office of Nuclear Reactor Regulation's response to staff action Item No. 2 enclosed with the February 4,1986 EDO memorandum:

" Staff Actions Resulting from the Investigation of the November 21, 1985 San Onofre Nuclear Generating Station, Unit 1 Event (NUREG-1190)". The study found that the underlying causes and general nature of the water hammer events which have occurred over the past five years do not appear to indicate any new generic concern not already identified and examined by the staff. Check valves were specifically identified as having caused only 2 of the 40 water .

events evaluated since 1969. Therefore, the study concludes that check valve leakage or failure is not a significant common generic cause of water hammer.

The group of experts would have a charter similar to that of the review group-on' steam explosions.

Chairman Zech emphasized the importance of obtaining input from different countries. He also requested the staff to clarify its plans to resolve the technical differences between the staff and the industry methods of calculating source terms.
