SUBJECT: NRC INSPECTION REPORT NOS. 50-327/88-22 AND 50-328/88-22

This refers to the special Nuclear Regulatory Commission (NRC) heatup shift

coverage inspection conducted by the NRC's Sequoyah Restart Task Force on

March 19, 1988 thru April 2, 1988. The inspection included a review of the

activities associated with the heatup of Sequoyah Unit 2. At the conclusion

of the inspection, the findings were discussed with those members of your

staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. These

areas included extended control room observation and operational safety

verification. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with I

personnel, end observation of activities in progress. i

One unresolved item was identified concerning closed a auxiliary feedwater

block valve, 2-HCV-3-866, which rendered the turbine driven auxiliary ,

feedwater pump unable to feed the number 2 steam generator in the event of an !

accident condition. This issue is further addressed in paragraph 9 of the l

'

enclosed report. It is requested that you provide a response, within 30 days

of the date of this letter, that addresses the root cause of this occurrence

and the corrective action you plan to take.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a copy of this letter, its enclosures,

and any reply will be placed in the NRC's Public Document Room.

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL 96-511.
